Allegation investigated: Reporting Requirements

On 05/13/2025, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to deliver complaint 21-AS-20250218132605 investigation findings regarding the above allegation and met with Estrella Valendo, Licensee. Reporting Party (RP) alleges that the facility did not comply with reporting requirements when Resident 1 (R1) fell on two separate occasions. LPA Florio conducted 10-day complaint investigation visit on 02/19/2025 and obtained documents, made observations, and conducted interviews. During this visit it was revealed through an interview with the Licensee and obtained “Resident Daily Narrative” charting notes that R1 fell on two occasions but sustained no injuries, did not require assessment by a medical professional, Hospice was notified, and Licensee states R1’s family was notified. During investigation LPA interviewed staff, residents, responsible parties and various outside agencies, made observations, reviewed resident records and took pictures. The complainant alleged Faciltiy staff are not repositioning resident. LPA received statements from resident's R1 and R2 hospice agency, interviewed staff and made observations. Statments and records reviewed did not reveal that staff failed to reposition residents. Based on documents and statements the allegation is UNSUBSTANTIATED. The complainant also alleged that Facility staff failed to performe oral care. LPA conducted interviews, made observations of resident. Resident R1 seems to have poor oral health, but LPA is unable to deterimined when this has began. Licensing Program Analyst (LPA) Walters arrived unannounced for the purpose of delivering findings regarding the allegation listed above. LPA met with Administrator/Licensee, Estrella Valendo (EV). Assistant Administrator, Normita Subala arrived later. During investigation LPA interviewed staff, residents and various outside parties, LPA conducted virtual and in-person visits to the facility on 03/19/21 and on 06/24/21, made observations and took pictures. The complainant alleged that the faciltiy is serving expired food to residents. Based on interviews and observations LPA learned the following: On 3/19/21, LPA toured the facility virtually with EV and NS. LPA observed expired can foods dated 2018 in the garage and expired decayed food in the fridge. (pictures taken) LPA asked that EV remove all expired food from the refrigerator and throw away expired canned foods. Interviews confirmed that food was being given to residents.
