Licensing Program Analyst (LPAs) Renita Hall conducted an unannounced visit to deliver findings regarding the above-mentioned allegations. LPA was allowed entry by the Caregiver. LPA identified herself and disclosed the purpose of the visit and elements of the complaint to the Caregiver. On October 28, 2025, the Department received a complaint alleging the above concerns. LPA conducted a facility visit, which included a tour of the physical plant, observations, and interviews with staff, Resident 1 (R1), and the Power of Attorney (POA). During the facility visit, LPA observed the facility to be maintained in good repair. There were no observable odors, no cobwebs present in R1’s bedroom, and no visible leaks in the roof at the time of inspection. R1’s room appeared clean and organized. The POA confirmed LPA’s observations regarding the condition of the facility and R1’s bedroom.
