On 10/13/2025 at 1:25 PM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility to investigate and deliver the findings of the above allegations. LPA Serrano met with administrator Loreta Tuparan to explain the purpose of the visit. The investigation consisted of file review, interviews with facility staff, resident and related party as well as facility observation. Allegation #1: The licensee violated resident's personal rights – Based on observation and interview of staff and resident, Resident #1 (R1) stated that R1 has no complaints about this facility, this is a good facility, the administrator is very good. R1 stated that the administrator never speaks to R1 inappropriately. The information received during the investigation did not corroborate with the allegation. Licensing Program Analyst (LPA) Sarina Ramirez conducted an unannounced visit to the facility to conduct a complaint investigation on the above allegations. LPA met with Administrator Lori Tuparan and explained the purpose of the visit. Regarding the allegation #1, LPA Ramirez conducted 2 staff interviews. 2 out of the 2 staff informed LPA they reposition hospice residents every two hours. LPA Ramirez conducted 2 hospice resident interviews. 2 out of the 2 residents stated staff reposition them every 2 hours and staff are available if reposition is needed prior to the two hour mark. Regarding the allegation #2, LPA conducted 2 staff interviews. 2 out of the 2 staff informed LPA residents are fed three times a day and snacks in between meals throughout the day. Allegation investigated: Facility did not notify the responsible person of a change of condition for the resident
Licensing Program Analyst (LPA) Tricia Danielson met with Licensee Loreta Tuparan via telephone to conclude an investigation into the allegation listed above. The facility does not currently have residents and there are mitigating circumstances that deem Ms. Tuparan to be unable to meet with LPA at the facility. Regarding the allegation "Facility did not notify the responsible person of a change of condition for the resident", it was alleged that the facility did not notify Resident #1's (R1's) responsible person that R1's eyes were swollen and gooey and having trouble walking as noted on R1's last day at the facility. Interview with Licensee Tuparan indicated no notice was made to anyone because there had not been any change in R1's condition. Tuparan stated R1 had been on a visit with their sister three (3) days prior to their last day at the facility and R1's sister did not report any concerns. Licensing Program Analyst (LPA) Tricia Danielson met with Licensee Loreta Tuparan via telephone to conclude an investigation into the allegation listed above. The facility does not currently have residents and there are mitigating circumstances that deem Ms. Tuparan to be unable to meet with LPA at the facility. Regarding the allegation "Resident left in soiled diapers", it was alleged that Resident #1 (R1) was found to be in a soiled diaper during a visit. Interview with a visitor of R1 revealed R1 was taken on an outing on 3/10/20, R1 was observed to wearing an incontinent garment which was caked in dried feces. The garment was removed, R1 was cleaned and a new garment was provided by the visitor. Interview with Licensee/Administrator Loreta Tuparan indicated R1 left the facility with a clean incontinent garment as well as an additional garment and a change of clothes for any accidents which may have occurred during the outing.
