Licensing Program Analyst (LPA) Ryan Gardner arrived at the facility unannounced to investigate and deliver findings for the above complaint allegation. LPA met with Administrator Ebraheem Hamed and explained the reason for the visit. During today’s visit, LPA toured the facility, interviewed residents, and interviewed staff. For allegation, Staff mishandled resident’s medications while in care: During facility tour, LPA observed three (3) plastic containers on the kitchen table with the resident’s medication stored in the plastic containers. Interviews conducted with the staff revealed that the medication was removed out of the original medication containers and placed in the plastic containers. The staff planned to leave the plastic medication containers on table until the resident’s ate dinner. The staff storing the resident’s medication in plastic containers on the kitchen table poses an immediate health, safety, or personal rights risk to persons in care. Licensing Program Analyst (LPA) Paola Guerrero conducted an unannounced visit to deliver findings on the allegations listed above. LPA met with Facility Assistant Administrator Sabrina Saenz and explained the purpose of the visit. The investigation consisted of staff and resident’s interviews. First allegation: Staff is drinking alcohol at the facility. Regarding the first allegation, Staff is drinking alcohol at the facility. LPA Guerrero conducted in-person interviews with Resident #1, Resident #2, and Resident #3, who all stated that they have not witnessed Staff #1 drink alcohol while providing care. LPA interviewed Staff #1, who stated that they work at the facility Mon-Fri and arrive at the facility at random hours and has not witnessed Staff #1 consume alcohol while providing care. Licensing Program Analyst (LPA) Anna Bueno conducted an unannounced subsequent visit to continue the complaint investigation of and deliver findings on the above allegation. LPA met with care provider Lanny Latta who was informed of the reason for the visit. Administrator Ebraheem Hamed was phoned and administrative staff Sabrina Saenz arrived shortly. LPA conducted resident and staff interviews and reviewed records. The allegation is Staff physically assaulted client (C1). All resident and staff interviews revealed that they do not know C1 or have heard of C1. Resident interviews deny that staff hit or physically assault them. Interviews with staff revealed that they don't hit any of the residents and that there has been no report of physical assault by any staff on any resident. LPA reviewed records confirming that staff have completed elder abuse training.
