Licensing Program Analyst (LPA) Victoria Brown arrived unannounced to investigate the above mentioned allegations on 7/10/24 at 11:00a. LPA met with Administrator Grace Bernardino and stated the purpose of the visit. LPA conducted a tour of the facility, resident rooms, and furniture. LPA conducted interviews of residents, staff and Administrator during this visit. Regarding allegation, "Staff did not ensure that facility was kept free of infestation", LPA observed food items in the drawers with roaches in room 6 during this visit. LPA also observed blue powder along the floor boards of the bathroom that Licensee is using to rid the facility of any pests. During visit, Administrator assisted resident with putting opened food items in a zip lock bag. Licensing Program Analyst (LPA) Bruce Jacobs arrived at the facility and met with Facility Administrator Grace Bernardino to conclude and deliver the investigation findings on the above allegations. This investigation was conducted by the Investigations Branch and LPA Jacobs and consisted of site inspections to the facility to conduct interviews with the facility administrator, staff, residents and other witnesses. The Investigator obtained and reviewed copies of the resident's (R-1) files, medical reports, Law Enforcement report and additional documentation. This investigation concluded after several inspections, interviews and record reviews that the facility did not neglect a resident (R-1) resulting in worsening of pressure injuries and did not fail to seek timely medical attention for the resident (R-1). On 10-06-2021 at 8:25 am, Licensing Program Analysts (LPA) Avelina Martinez conducted an unannounced facility visit in regards to a complaint investigation with the above allegations. LPA Martinez met with Grace Bernadino and explained the purpose of today's visit. During the course of the investigation, LPA Martinez conducted interviews, toured the facility, and reviewed facility documents. LPA Martinez reviewed R1's medical file. R1's medical file did not contain a home health service plan or a written care responsiblity agreement between the licensee and home health agency. Moreover, R1's facility records do not indicate R1 had open wounds or pressure injuries. LPA Martinez interviewed staff 1 (S1), and S1 reported R1 did not have open sores or pressure injuries.
