On 8/4/2026, at 1:30 PM Licensing Program Analysts (LPA) Reza Jamaly arrived unannounced at the facility to conduct the required annual inspection. LPAs met with staff on duty, and explained the purpose of the visit, administrator Lucia Boboc was notified and arrived shortly after. Facility is cleared for 6 resident non-ambulatory residents, waiver granted for hospice care for 6 residents, current census was 5, per administrator 2 residents left the facility for the family visit. Firstly, LPA Jamaly inspected the physical plant including but not limited to the common area, kitchen, pantry, dining area, resident's bedrooms, resident bathrooms, laundry room, and outside courtyards of the facility to ensure compliance with Title 22 regulations. LPA inspected 5 resident's bedrooms and observed bedrooms to be properly furnished with appropriate bedding and lighting. Resident's bathrooms observed to be cleaned, grab bars and non-slip mats were observed to be secure and in good condition. The hot water temperature measured 112.4 at one of the resident bathroom sink, which is within the required range of 105°F to 120°F. Facility temperature was checked at living room and observed to be 77 degree F. There are no bodies of water present. During the inspection, LPA toured the kitchen and pantry and observed multiple perishable and non-perishable food items that were either expired, spoiled, or marked with manufacturer "Best if Used By" dates that had passed. In the kitchen, LPA observed seven (7) loaves of bread with visible mold. The bread was labeled with "Best if Used By" dates in February, May, June, and July 2026. LPA also observed at least four (4) packages of wilted vegetables stored in the refrigerator. During the inspection, the Administrator removed the wilted vegetables from the refrigerator. Continues on LIC 809C on page 2 page 2 Additionally, LPA observed two (2) packages of beef labeled "Best if Used By 06/05/2026." The Administrator immediately discarded the beef upon identification. LPA inspected the pantry and observed numerous non-perishable food items, including cereals, biscuits, peanut butter, chips, sugar cones, raisin bran cereal, cheese sauce, snacks, and other food products with manufacturer "Best if Used By" dates that had already passed. Photographs were taken to document the observations. LPA reviewed the date labels to determine whether food items were being retained beyond the manufacturer's recommended quality date. Among the items observed, at least three (3) containers of crispy bread crumbs were labeled "Best if Used By 12/12/2020." LPA interviewed the Administrator regarding the facility's procedures for monitoring food quality and date labeling. Specifically, LPA asked how the facility monitors food expiration, how frequently food storage areas are inspected, and how often food is purchased. The Administrator stated, "I usually check the food quality and expiration dates on a monthly basis, but I haven't checked the food expiration dates for a couple of months." A deficiency was cited and documented on LIC 809-D. Administrator took them away and cleaned the pantry.. Sharp knives were secured in a locked kitchen cabinet and inaccessible to resident. LPA inspected 5 resident's bedrooms and observed bedrooms to be properly furnished with appropriate bedding and lighting. Resident's bathrooms observed to be cleaned, grab bars and non-slip mats were observed to be secure and in good condition. The hot water temperature measured 108.1 at one of the resident bathroom sink, which is within the required range of 105°F to 120°F. Facility temperature was checked at living room and observed to be 77 degree F. There are no bodies of water present. Smoke detectors and carbon monoxide detectors were found at the facility. One fire extinguisher, located in living room and in hallway, was last serviced on 8/4/2026. LPA observed that the facility maintained a public telephone in the living room and that all required licensing posters were properly displayed Cleaning supplies and other toxic substances were observed in laundry room and laundry door were locked, inaccessible to resident and administrator unlocked laundry door for LPA inspection. page 3 Medication were observed to be stored in a locked cabinet in the kitchen and inaccessible to residents. LPA reviewed 3 of 5 residents' centrally stored medication record, which were complete and consistent with physician's orders. The first aid kit was inspected and contained all required supplies. LPA requested resident and staff records for review. LPA reviewed 3 out 5 resident's file and found them to be complete. Two (2) staff files were reviewed and found to be complete. A review of personnel records confirmed that all staff members and other individuals required to undergo caregiver background checks had received criminal record clearances and were properly associated with the facility. The following documents were requested and will be emailed to the LPA by 8/17/2026 no later than 5:00 p.m.: LIC 308 – Designation of Administrative Responsibility Copy of the Administrator Certificate LIC 610 – Emergency Disaster Plan Proof of Current Liability Insurance LIC 500 – Personnel Report LIC 309 – Administrative Organization Based on today's inspection, the facility was found to be out of compliance with applicable Title 22 regulations. The deficiencies are documented on the attached LIC 809-D Deficiency Report. Licensing Program Analyst (LPA) Sommer Hayes arrived unannounced to conduct a Required - 1 Year visit on 07/17/25 at 8:45am. LPA met with Lucia Boboc, the Administrator, and stated the purpose of today’s visit. The most current emergency drill was conducted on 05/20/25 at 10:00am. 5 residents and 2 staff participated. The facility is licensed for a capacity of 6 residents. There are currently 5 residents. LPA toured and inspected the physical plant inside and outside to ensure there are no safety hazards to residents. LPA observed 2-day perishables and 7-day non-perishables. The temperature inside the facility was observed to be at 75* F which is within the required range of 68-85*F. The hot water temperature was measured at 106.1 F which is within the required range of 105-120 F. LPA observed a fire extinguisher, smoke and carbon monoxide detectors, and central heating and air in the facility. LPA observed the centrally stored medications area to be locked a with a “U” type child safety lock made by “Regalo” that does not use a key and can be bypassed and refrigerated medications were observed in the kitchen refrigerator making medications accessible to residents. The first aid kit contained the required items such as a guide, sterile dressings, bandages, adhesive tape, scissors, tweezers, thermometers, antiseptic solution and a guide. LPA reviewed 2 resident records and 5 staff records. LPA observed the backyard of the facility. There was a shaded area and a small garden for residents to enjoy. There were no bodies of water present. Per the California Code of Regulations, Title 22, Division 6, Chapter 8, 1 violation was cited during this visit. Licensing Program Analyst (LPA) Victoria Brown arrived announced on 8/19/24 at 10:00am and met with Applicant representative Lucia Boboc and stated the purpose of the visit. This visit is to conduct a Pre-Licensing Inspection. LPA was allowed entry into the home that will be licensed for a capacity of 6 non-ambulatory residents of which 6 may receive hospice care services. Administrator certificate expires 12/17/24. LPA and Lucia Boboc toured and inspected the physical plant inside and outside to ensure there are no health and safety concerns. LPA observed the kitchen area, dining area, bedrooms, bathrooms, storage areas, back yard, and laundry room. LPA observed knives/sharps area to be locked. LPA observed required furniture, and lighting throughout the facility. The hot water temperature measured at 115.3 *F which is within the required range of 105-120*F. The temperature inside the facility measured at 75*F which is within the required range of 68-85*F. LPA observed supplies of nonperishable foods for a minimum of one week and perishable foods for a minimum of two days maintained on the premises. The first aid kit included supplies such as sterile first aid dressings, bandages or roller bandages, adhesive tape, scissors, tweezers, thermometers, antiseptic solution, and guide. LPA observed area for centrally stored medications to be locked. LPA observed the fire extinguisher(s), smoke and carbon monoxide detector(s) and exit alarms on every door in the home. The facility has central heating and air. LPA observed the area for staff and resident files locked and to be made readily available for review. Component III conducted - There are no objections to licensure at this time. -Licensure pending.
