Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 11/04/25 to do the complaint investigation for above allegation. LPA met with administrator Andrea Yerby and explained the purpose of the visit. From record review, administrator interview, it has been concluded that R1 AWOL from facility on 10/31/25 sometime around 12AM and was outside the facility without staff's assistance. R1s LIC602 ,dated- 01/02/25 signed by their physician disclosed that R1 cannot leave facility unassisted. From all gathered information, it has been concluded that R1 had AWOL from facility unassisted and unsupervised on 10/31/25 due to lack and care from facility. It was learnt that emergency services were called but R1 was not transferred to hospital as there were no visible injuries. Based on interviews conducted by the department and records reviewed, the preponderance of evidence standards has been met. Therefore, the above allegation is found to be SUBSTANTIATED. Licensing Program Analysts (LPAs) Lavinia Muscan and Talwinder Bains arrived unannounced to complete and deliver findings to a complaint received on 3/28/23. LPA met with Administartor Andrea Yerby and explained purpose of visit..LPAs wore surgical masks during today's visit. LPAs interviewed (1) staff and (1) resident and reviewed medications for residents. LPAs also toured the facility and observed all residents on 4/4/23 and on 5/10/23. Licensing Program Analyst (LPA) Wolter arrived at the facility unannounced on 09/08/2021 to deliver complaint findings for the allegations listed above, LPA met with staff Shantel Watson and explained the purpose of the visit. Prior to initiating visit LPA completed daily self-screening questionnaire to confirm no symptoms of COVID-19, LPA was screened by staff upon entry and wore surgical mask and applied hand sanitizer. LPA contacted administrator via telephone to go over report as they were unable to come to the facility at time of visit, verbal permission was given for staff to sign report. Throughout the course of the investigation the department reviewed documentation and conducted interviews relevant to the allegations: facility staff did not follow reporting requirements, facility staff did not safeguard residents personal belongings, and resident was illegally evicted.
