On March 26, 2026, at approximately 10:00 AM, Licensing Program Analyst (LPA) Antonine Richard conducted a subsequent unannounced complaint visit. The LPA met with the Administrator, Svetlana Calamaro (A1), and explained the purpose of this visit. The investigation included the following: The LPA conducted interviews with the Administrator (A1), one staff member (S1), and three residents (R2-R4). The LPA also reviewed and obtained several documents, including the Client Roster, Staff Roster, and CPR/AED/First-Aid Certificates for five staff members. Additionally, a copy of (R1)’s face sheet, the Admission Agreement dated March 6, 2021, (C1)’s Physician’s Report for Community Care Facilities (dated March 19, 2023), Medication Administration Records (MAR) from May 2023, and records of hospital visits and discharges (dated February 22, 2023) were examined. On 4/17/2025 at 12:10 PM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility to deliver the findings of the above allegations. LPA Serrano met with administrator Svetlana Calamaro to explain the purpose of the visit. The investigation consisted of file review, interviews with facility staffs as well as facility observation. Allegation #1: Resident do not have appropriate laundry services – Based on observation of laundry room (washer and dryer), and information received during interviews LPA was unable to corroborate the allegation. During today’s visit, LPA observed the facility’s washer and dryer are currently operating. Allegation investigated: Staff speaks to resident in an inappropriate manner. Staff did not accord resident personal privacy with visitors.
On 08/31/2023, Licensing Program Analyst (LPA) Chinwe Nwogene conducted an unannounced visit to investigate the above allegations. LPA met with Licensee, Svetlana Calamaro who was informed of the purpose of the visit. During the visit, staff and resident were interviewed. Regarding the allegation “Staff speaks to resident in an inappropriate manner” it was alleged staff yelled at resident and was aggressively speaking to resident. Resident was interviewed, the interview revealed staff yells at resident. Licensee was interviewed who acknowledged speaking to resident and resident’s daughter in a raised voice because licensee heard the facility caregiver names mentioned during resident and resident daughter's conversation with a Kaiser Nurse (Substantiated). Regarding the allegation “Staff did not accord resident personal privacy with visitors” it was alleged staff was ear dropping on resident and resident daughter's conversation with nurse.
