Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to deliver findings on the allegation listed above. LPA met with Administrator Emmanuel Torres Garcia and House Manager Priscila Castanon and explained the purpose of the visit. The investigation consisted of staff and client interviews and request of documentation. For the allegation, Staff did not prevent client in care from being assaulted at the facility LPA interviewed Client #1 (C1) who stated C1 has not been assaulted at the facility. Licensing Program Analysts (LPA) Beena Singh conducted an unannounced visit to the facility to deliver findings on a complaint alleging Staff hit Resident #1 (R1) LPA Singh met with Staff-Ashley castanon, facility representative, and was granted entry into the facility. The facility Manager-Emanuel Torres arrived during the visit. The investigation conducted by LPA Singh consisted of interviews and records review. Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to conduct a complaint investigation regarding the above allegations. LPA Prieto met with Administrator Robert Cantoria and explained the elements of the complaint. Regarding the allegations that the facility staff not assisting client with dental hygiene needs; LPA Prieto interviewed resident #1 (R1) in question, who stated that they can handle their own dental hygiene needs and does so on a daily basis. R1's medical and assessment records indicate that R1 is able handle their own hygiene needs, including dental, with no assistance from staff. R1 has a dental service and LPA Prieto obtained those records to show that dental services are being provided. This unannounced visit by Amy Goldenberg, Licensing Program Analyst (LPA), is to initiate the 10 day visit to investigate the above-mentioned complaint allegation. LPA met with caregiver Angie Zalazar and discussed the elements of the allegation. During this investigation visit LPA interviewed staff, reviewed R1's medications, medication records/orders, and Indivual Program Plan. LPA received copies of medication treatment records, and three incident reports dated 10/24/2022, 11/19/2022, 11/24/2022. It is alleged that prior to R1's most recent hospitalization 12/20/2022 it is believed the client was not receiving the correct dosage of medication prescribed. Investigation revealed the following: LPA review of the medications on hand and the medication treatment record show that staff are dispensing medications and signing out each dose.
