Allegation investigated: Resident in care was not provided meals in a timely manner. Staff did not provide proper medication assistance to resident in care.

Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to conclude a complaint investigation regarding the allegations made in this complaint. LPA Prieto met with Ms See and explained the elements of the complaint. Allegation #1 - LPA interviewed resident #1 (R1), R2, R3, R4 at the facility, all stating that they are receiving three (3) meals a day and those meals are being served in a timely manner. Interview with staff #1 (S1), states that meals are served during appropriate hours for breakfast , lunch and dinner. Allegation #2 - LPA interviewed resident #1 (R1), R2, R3, R4 at the facility, all stating that they are having their medication dispensed accordingly and as prescribed. Allegation investigated: Resident sustained unexplained bruise. Staff not maintaining residents grooming needs.

Licensing Program Analyst (LPA) Ryan Gardner made an unannounced visit to investigate and deliver findings for the allegations listed above. LPA stated the purpose of the visit and was granted entry and met with Licensee/Administrator Marilou Panaligan. The investigation consisted of resident interviews, staff interviews, and document review. For allegation, Resident sustained unexplained bruise: Interviews with residents and interviews with the staff revealed that there was no evidence of staff causing bruising to the residents in care. The residents denied being hit, pushed, physically injured, and or bruised by the staff. The staff denied hitting, pushing, causing injuries, and or bruising the residents. Document review of R1’s facility records did not indicate that R1 had bruising. LPA was not able to speak to R1 due to R1 passing away in November of 2020. Licensing Program Analyst (LPA) Javier Prieto arrived to the facility to initiate a complaint investigation regarding the above-mentioned allegations. LPA met with staff Marineth Reyes and Henry Reyes. There was not administrator at the facility during time of visit, with designated administrator qualified to perform the duties of an administrator for approximately 30 days prior to visit. Review of licensing records show that the faciity has not reported any incidents occuring at the facility since June 2022. LPA observed resident #1 (R1) with an injury with no record of R1 visiting the hospital for that injury. Based on LPA observations, interviews which were conducted and records review, the preponderance of evidence standard has been met. Therefore the above allegation is found to be SUBSTANTIATED. Review of resident records does not reveal any resident with a prohibited health condition. LPA met and observed all residents at time of visit. LPA observed the food supply and observed a sufficient supplies of perishables and non perishables with food being served of good quality and ample servings. Based on the information obtained there is not enough evidence that the facility is over capacity, facility is serving food not of good quality and the facility does not maintain sufficient food and supplies for residents and facility retaining resident's with prohibited health conditions. Therefore, the allegations that is deemed UNSUBSTANTIATED at this time.
