Allegation investigated: Toxins and medications are accessible to residents in care.
On this day, Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to the facility for the purpose of continuing the investigation of the allegation listed above. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. An initial investigation visit was conducted on August 24, 2022 by LPA Jenifer Tirre. During the visit, LPA Tirre accompanied by staff conducted a tour of the physical plant. LPA Tirre conducted interviews, reviewed files and requested pertinent documents. The investigation determined as follows: regarding the allegation toxins and medications are accessible to residents in care, it was reported that toxins and disinfectants are located under the kitchen sink and unlocked, accessible to residents in care. It was revealed during the initial investigation on August 24, 2022, LPA Tirre observed bleach and other toxins under the kitchen sink unlocked and accessible to residents.
