Allegation investigated: Staff neglect resulted in a resident sustaining a burn. Staff did not provide reasonable accommodation to the resident.

Licensing Program Analyst (LPA) Leizl de la Cerra conducted an unannounced subsequent complaint visit to this facility to investigate the above allegation and deliver findings. LPA met with facility manager, Susanna Avetian and explained the reason for the visit. Regarding the allegation: Staff neglect resulted in a resident sustaining a burn. To investigate this allegation LPA de la Cerra conducted an initial visit on 12/16/24 and LPA requested pertinent documents relevant to the investigation at 11:00AM, conducted and interviewed resident and facility staff member between 11:15AM – !2:15PM. On 02/12/25, at 12:30PM LPA de la Cerra asked for the census, LIC500 and LIC9020 upon arrival. LPA interviewed additional residents, conducted a physical tour, gathered additional pertinent documents. It was alleged that while bathing R1 on 12/11/24, staff failed to check the temperature of the water which resulted in R1 getting burns on her back and lower back side. Allegation investigated: Staff unlawfully evicted a resident

Licensing Program Analyst (LPA) Rosaura Valenzuela conducted an unannounced visit for the above noted allegation, LPA met wiith Manager Susanna Avetikyan and explained the reason for the visit. It was reported that staff unlawfully evicted a resident. To investigate this allegation on 05/29/24 between 10:45am and 11:00am, staff interviews were intitiated. Interviews revealed that staff did not evict Resident #1 (R1). R1 was sent to the hospital on 5/19/24, due to expolosive diarrhea, and once there the hospital refused to disclose their diagnosis. Facility requested many times for R1's information to be provided, but it was not. Staff told hospital that they could not accept R1 until their diagnosis was disclosed. On 5/21/23, R1's responsible party came to the facility and collected their belongings. Based on interviews, there is not sufficient information to support this allegation. Licensing Program Analyst (LPA) Rosaura Valenzuela conducted an unannounced visit for the above noted allegatoin. LPA met with Manager Suzanne Avetian and explained the reason for the visit. It was reported that staff did not ensure resident's toileting needs were properly met. Resident #1 (R1) has had uncontrolled diarrhea and vomiting for approximately two weeks. To investigate this allegation on 5/21/2024, between 10:30am and 11:15am, staff interviews were initiated. Interviews revealed that R1 has been a resident at the facility for less than a month. About two weeks ago, R1 developed uncontrolled diarrhea. Staff has been doing there best to meet R1's toiletting needs by changing the diaper as often as needed and cleaning them. On 5/19/2024, R1 was sent to the hospital with the consent of their next of kin. R1's responsible party refuses to disclose theiir medical diagnosis to staff. Licensing Program Analyst (LPA) Cynthia Chan conducted a subsequent complaint visit to deliver findings for the allegation listed above. LPA met with Staff and explained the purpose of the visit. LPA spoke to Administrator, Viktorya Hayrapetyan, via telephone and read the report. The investigation consisted of the following: On 7/15/22, LPA Chan conducted the initial visit and toured the facility. LPA did not observe any health and safety concerns. There was at least 2 days of perishable and a week of non-perishable food supply. LPA obtained copies of the staff roster, resident roster, and documentation pertaining to Resident #1. Interviews were conducted with the Administrator and staff. The investigation revealed the following: Allegation - Resident sustained multiple pressure injuries while in care.
