On 03/28/2024 Licensing Program Analyst (LPA) Melissa Spaeth conducted a subsequent complaint investigation at the above facility to address the following allegation(s). LPA met with the caregiver. LPA explained the purpose of this visit was to deliver the findings for this complaint. The investigation consisted of the following: On 08/05/2022, then, LPA Ruiz conducted a physical plant tour and collected resident documentation. During the 3/27/2024 visit, interviews were attempted with all five (5) residents of which one resident was interviewed at 11:15 am. The other four (4) residents were unable to participate due to their cognitive function. LPA Spaeth interviewed a staff member (S1) working at the facility at 11:15 am until 11:40 am. LPA reviewed the residents’ medications at 11:50 am until 12:15 pm. LPA received copies of residents' records and the facility menu. LPA Spaeth interviewed two staff members as of today, 3/28/2024 at 9:30 am and at 10:00 am via phone call. On 03/27/2024 Licensing Program Manager (LPM) Troy Agard and Licensing Program Analyst (LPA) Melissa Spaeth conducted a subsequent complaint investigation at the above facility to address the following allegation(s). LPA and LPM met with Administrator and Licensee, Kesha Richardson. LPA explained the purpose of this visit was to gather information, conduct interviews and deliver findings for this complaint. The investigation consisted of the following: on 05/24/2022, then, LPA Ruiz investigated and unsubstantiated six (6) of the eight (8) allegations. On 03/27/2024 from 9:45am- 10:15am LPA conducted a facility tour and interviews. LPM Agard reviewed records from 10:15am -11:01am. LPA and LPM collected R1’s Hospice records, Medication Administration Record (MAR), Needs and Services Plan, Physician Report, Staff and Resident Roster. At 10:15 a.m., Licensing Program Analyst (LPA) Evelin Rios conducted an unannounced initial complaint visit at the facility mentioned above. LPA was greeted by caregiver Sandy Iraheta. Sandy called the Administrator Kesha Richardson and LPA informed the Administrator the purpose of the visit and an entrance interview was conducted with Administrator over the phone. Administrator could not meet LPA at the facility and designated caregiver Sandy to sign this report. At approximately 10:30 a.m., LPA and Sandy conducted a physical plant tour to insure the health and safety of the residents in care. LPA did not observed any health and safety issues or concerns. While conducting the tour LPA interviewed Sandy. At 10:50 a.m. LPA reviewed and collected documents relevant to this investigation. At 11:32 a.m. LPA interviewed two (2) out of the two (2) residents present. At 11:47 a.m. LPA contacted the Administrator with follow-up questions. At 9:30 a.m., Licensing Program Analysts (LPAs) Melissa Ruiz and Angela Panushkina arrived at the facility to conduct an unannounced complaint visit. LPAs were greeted by staff and later met with the Administrator, Kesha Richardson. The purpose of the visit was explained, and an entrance interview was conducted. To aid this investigation, LPAs toured the facility at 9:50 a.m. conducted interviews with the Administrator, three (3) staff and two (2) out of two (2) residents from 9:40 a.m., to 12:00 p.m. LPAs requested and reviewed pertinent documents related to this investigation. File review consisted of but was not limited to resident care plans, medication logs, hospital records, etc. Allegation: Facility is in disrepair. Upon conducting a physical plant tour, LPAs did not observe the facility to be in disrepair. Interviews with staff and Administrator revealed that there was never a time where the facility was in disrepair.
