LPA Spaeth conducted an unannounced visit and was greeted by Administrator. LPA stated the purpose of the visit was to investigate allegations, facility did not seek medical attention in a timely manner, resident has lice, and resident's hygiene needs are not being met. LPA conducted a tour of the facility from 10:30 am until 10:50 am and did not observe any health or safety issues on 12/22/2021. LPA interviewed twelve residents, three staff members, the Administrator, and the Administrator Designee on 12/22/2021. Based upon LPA's staff inteviews, staff stated 911 was immediately called when R1 stated was feeling weak and needed medical attention. Residents' interveiws revealed that staff were attentiive to residents' needs to see a doctor or to call 911. Also staff reported R1 did not state to staff needed to see a doctor. However, staff did stated called 911 during the month of January, 2021 when R1 did state was feeling weak. LPA Spaeth reviewed resident's file at 10:45 am until 11:05 am. LPA requested a copy of the hospice provider, Tulip Hospice, Inc, Informed Consent form. LPA observed resident's signature which shows the resident did consent for the hospice services. Therefore this allegation is unsubstantiated. Exit interview was conducted, appeal rights discussed, and a copy of the report was given to the Administrator. Licensing Program Analyst (LPA) Wendell Smith conducted an unannounced complaint visit to investigate the allegation above. LPA met with administrator and explained the reason for this visit. LPA conducted a physical plant tour from 10:40-11am to ensure no immediate health and safety issues of which none were noted. Staff not meeting resident needs It is alleged that facility staff is not cleaning resident #1 (R1) bed sore on their left thigh daily as it is supposed to be cleaned. LPA conducted an interview with R1 from 11-11:30am regarding this allegation. LPA interviewed facility staff from 11:30-12pm regarding this allegation. LPA reviewed R1's hospice paperwork and obtained copies of pertinent information from 12-12:30pm. LPA interviewed the Hospice Supervisor from 12:30-12:50pm regarding treatment that R1 is receiving. Information obtained from interviews and record review show that R1 does not have a wound that is staged on their left thigh. LPA conducted a brief physical plant tour from 11:30-11:45 am to ensure no immediate health and safety issues were present. LPA did not observe any immediate health and safety issues. It is alleged that staff are not allowing resident (R1) to contact their physician. LPA conducted interviews with R1 from 11:45 to 12:30 pm regarding this allegation. LPA interviewed facility staff from 12:45 pm until 3:00 pm. Staff interviews revealed that R1 had not asked staff for assistance to speak to R1's physician. Based upon interviews of staff members, this allegation is unsubstantiated. It is alleged that staff are refusing to allow R1 to take medication. LPA interviewed R1 and R1 stated did not receive PRN medication on 7/12/2022 during evening hours. It is alleged that resident #1 (R1) is having their mail witheld by facility staff. LPA conducted interviews with R1 from 10:45-11:30am regarding this allegation. LPA interviewed facility staff from 11:30-12pm regarding this allegation. Interviews revealed that R1 had three different packages mailed to them by their friends and that they were sent on Sunday 7/3/22. The packages arrived at the facility on two different dates of 7/6/22 and 7/7/22. R1 confirmed that they received their packages in a timely manner on both of those dates. LPA explained the purpose of the visit was to present the findings regarding the allegations, resident sustained severe pressure injuries while in care and resident sustained multiple bruises while in care. A complaint was received on September 28, 2021 which alleged Resident #1 (R1) sustained severe pressure injuries while in care and that R1 sustained multiple bruises while in care. The complaint investigation was conducted by Investigations Branch (IB) Investigator Wilfredo Vasquez. The complaint stated that R1 sustained several pressure injuries and multiple bruises while in care. The Reporting Party (RP) stated R1 had been hospitalized for a bladder infection and bed sores; however, sores on R1’ hip and buttocks were ninety percent (90%) necrotic and required a plastic surgeon consultation. LPA Spaeth conducted an unannounced visit and was greeted by the Receptionist. LPA's temperature was recorded and COVID questions were answered. LPA was then greeted by Administrator at 10:35 am. LPA stated the purpose of the visit was to investigate a complaint which states facility is not safeguarding resident's personal belongings. The complainant stated it was reported some of the resident room keys could open other residents' rooms. LPA Spaeth and Administrator checked ten residents' rooms from 11:00 am until 11:45 am. LPA and Administrator checked the ten rooms with the resident's own room key and checked to see if a resident's key could also open another resident's room. Two designated room keys opened a different room. Therefore, it was determined the designated room keys were actually copies of the master facility key. LPA toured the kitchen from 11:45 am until 12:00 pm and observed the menu was posted outside the dining hall door. LPA was greeted by the Administrator and LPA stated the purpose of the visit was to investigate the complaint which states resident sustained wounds while in care and staff did not seek any medical attention for resident in care. LPA interviewed resident from 9:15 am until 9:40 am and interviewed two staff members from 9:45 am until 10:30 am. LPA conducted a tour of the facility from 10:30 am until 10:45 am and did not obsevve any health or safety issues at the facility. LPA reviewed resident's file from 10:45 until 11:00 am and obtained a copy of related documentation from resident's file. Resident sustained wounds while in care - Reporting party stated resident had two open wounds on R1’s body. LPA Spaeth made an unannounced visit to the facility and observed the COVID signs on the front door. LPA's temperature was taken and recorded. LPA explained the purpose of the visit was to investigate a complaint which states resident sustained scabies while in care and facility has bed bugs. LPA Spaeth conducted a brief physical plant tour from 2:15 until 2:35 to ensure no immediate health and safety issues were noted. LPA inspected twenty-two (rooms) from 2:45 uintil 3:45 pm. LPA checked all beds, baseboard area, and flooring with a flashlight and found no bed bugs. LPA Spaeth conducted an unannounced visit to the facility and was greeted by Administrator, Jessica Peleya. LPA stated the purpose of the visit was to continue the investigation of the complaint, residents' diapering needs are not being met. LPA confirmed with Administrator there are twenty-three (23) residents who receive diaper changes each day. On December 15, 2021, LPA interviewed ten residents from 12:30 pm until 3:00 pm. Six of the ten residents stated had to wait over thirty minutes for a diaper change on many occasion. Three residents stated only checked two times a day during the 7:00 am until 7:00 pm shift and one resident stated had to wait over two hours for a diaper change during the evening shift (7:00 pm until 7:00 am). On December 15, 2021, Administrator explained the caregivers are to check on those residents who need assistance with diaper change every two hours. LPA amended 9099 report of 12/16/2021 adding additional interviews conducted by LPA Spaeth as of today, 11/21/2022. Staff over medicated resident - LPA Spaeth interviewed the medical technician, Araceli F. (S1) at 3:30 pm on 12/08/2021 who stated R1 was not prescribed the medication Trazodone. LPA received a copy of R1’s centrally stored medication record and observed the medication was not on the list. S1 stated during the month of October, R1 refused to take medications. At 3:40 pm, LPA obtained a copy of the medication report and LPA observed the letter R in each designated day which indicates refusal. LPA Spaeth conducted a complaint visit and met with Tannya Quezada , Administrator Designee at 4:00 pm. LPA Spaeth discussed the purpose of the visit was to report the findings of the investigation regarding the complaint dated December 31, 2020. The complaint stated a resident sustained injury while in care. The complainant stated the resident had multiple large purple and red areas of bruising to the upper chest and stated the resident had long linear red areas of bruising along the neck. The complaint report was forwarded to the Investigative Bureau via fax. A full investigation was conducted by IB Investigator, Peter Zertuche, for the case. The IB Investigator interviewed the Complainant on January 25, 2021. Complainant stated noticed numerous marks and bruises on resident’s chest and shoulder. The complainant reported resident was intubated and resident was unable to answer questions. Licensing Program Analyst (LPA) Wendell Smith conducted an initial complaint visit regarding the allegation above. LPA met with the administrator and explained the reason for this visit. Upon entry to the facility LPA conducted a brief physical plant tour to ensure no health and safety issues were present. During the walk through LPA did not notice any health and safety issues. At approximately 10:30 am LPA conducted an interview with the administrator regarding the allegation. It is alleged that resident # 1(R1) was removed from hospice services and R1's disposable catheter bag was removed also. At approximately 11am LPA conducted an interview with R1 regarding the allegation. R1 stated that without warning he was discharged from hospice and that his hospital bed and disposable catheter bags were taken.
