The LPA arrived at the facility at 10:00 a.m., met with Administrator Lori McKay and explained the reason for the visit. The LPA and the administrator conducted a tour of the physical plant at 10:30 a.m. The LPA checked for temperature readings recorded on the temperature thermostats found throughout the hallways in the facility on the first and second floor. At 12:30 p.m., the LPA interviewed residents, and staff. On the allegation that ‘Staff do not provide a comfortable temperature for the residents in care’; it is the concern of the complainant that the temperature in the facility has been very hot during the heat wave, and that the air conditioning unit has not been turned on even though the temperature outside has reached 107 degrees. On 7/20/2022, LPA Teresa Camara conducted an unannounced initial complaint visit. During the initial complaint visit LPA Camara conducted a brief facility tour upon arrival at 9:43 a.m. LPA interviewed Resident #1 (R1) at 9:51 a.m., interviewed Staff 1 (S1) at 10:40 a.m., and interviewed the Executive Director at 11:10 a.m. LPA obtained pertinent documents at 10:50 a.m. During the initial visit, it was determined that further investigation is needed to make a finding for the above allegations. Prior to conducting today's complaint visit, LPA Yee also conduct a telephone interview on 6/13/23 with a family member, identified as Family #1 at 11:52am. An initial unannounced complaint visit was conducted on 9/1/22 by LPA Teresa Camara. During the initial visit, LPA Camara conducted an interview with the Facility Manager at 10:18 a.m. and reviewed records at approximately 11:00 a.m. Resident 1 (R1) was at the hospital during LPA's visit. It was determined during the initial visit that further investigation was needed to make a finding for the above allegations. LPA Urena and Manager Joey Vitug conducted an inspection of the physical plant, and randomly selected and inspected 10 rooms from 1:10 p.m. to 2:20 p.m. The LPA interviewed staff at 12:55 p.m. The LPA interviewed the complainant on 05/11/2023 at 4:15 p.m. On the allegation that the ‘Facility staff are not cleaning resident's room’ it is the complainant’s concern that the facility staff spend a very short period of time cleaning the residents’ rooms, and that several furniture items are dusty and need cleaning. Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced subsequent complaint visit to the deliver findings for the allegation listed above. Upon arrival LPA met with staff and explained the reason for the visit. Executive Director Lito Vitug arrived shortly after. During the investigation, on 12/22/2020, LPA conducted a physical plant tour virtually, interviewed Executive Director as well as requested pertinent documentation relevant to the investigation. On 02/14/2023, LPA conducted a subsequent visit and conducted physical plant, interview Executive Director, nine (9) staff, as well as reviewed and obtained copies of additional pertinent documentation relevant to the investigation. During the investigation, LPA conducted a physical plant tour on 05/03/2021, as well as interviewed Executive Director and Administrator. On 02/22/2023, LPA conducted interviews with nine (9) residents and nine (9) staff during a subsequent visit investigating allegations on a separate complaint. It was reported that resident in care has scabies, as it was alleged that Resident 1 (R1), contracted scabies while in care. LPA’s interview with Lito along with records review of R1’s file revealed that R1 was first admitted into the facility on 01/19/2021. On 11/17/2022, the Department received a complaint regarding an allegation of Neglect/Lack of Supervision. It was alleged that upon admission to a skilled nursing facility for rehabilitation, Resident #1 (R1) was found to have an unexplained sacrococcyx pressure injury which measured 2.7cm x 1.5cm and was unstageable. LPA arrived at the facility at 10:25 a.m. and observed video feed of cameras in the following locations: front door, back entrance, lobby area, front desk area, and two medication rooms. LPA interviewed the ED at 10:48 a.m. who stated there are no operable cameras in the hallways or facility perimeter and there are no cameras placed in any residents' rooms. During LPA's visits to the facility LPA has observed the overall cleanliness of the facility and residents' rooms to be sufficient. R1 has an order for crushed medications. R1 requires staff to crush the medication and mix the medication in applesauce in the presence of R1. R1 also requires staff mix water in the applesauce cup two times to ensure all medications are received. Staff comply with R1's requests. In addition, staff come to R1's room to deliver medications on time but R1 will send them away and tell them to return later if R1 is busy doing something else. Staff try to ensure R1 is receiving medications timely but when R1 refuses the administration they cannot always ensure timely administration. The catheter was a condom catheter but due to anatomical changes it was no longer usable so R1 switched to a bed urinal. R1 indicated the staff assist with cleaning out the urinal but otherwise R1 is capable of using the urinal without assistance. Based on R1's statement that the facility staff did not need to assist with the catheter and R1 self-managed the condom catheter, the allegation staff did not observe issues with R1's catheter placement is deemed Unsubstantiated at this time. The LPA met with Lito Vitug & Lori McKay and explained the reason for the visit. On 11/20/2020, the Department received a complaint which alleged that due to lack of care and supervision, Resident #1 (R1) developed multiple pressure injuries. The initial visit was conducted virtually on 11/23/2020 from 12:45 p.m. to 1:30 p.m. by LPA Zabel Chochian, in which LPA Chochian interviewed staff and requested documents. LPA Ashley Smith conducted a subsequent visit on 03/15/2022, and the LPA conducted a tour at 9:20 a.m., conducted a file review at 10:20 a.m., and obtained documents. During today’s visit, LPA conducted a brief physical plant tour at 2:00 p.m. LPA interviewed staff at 2:00 p.m., 2:42 p.m. and 4:00 p.m. LPA interviewed Resident 1 (R1) at 3:05 p.m. Staff and R1 stated R1 does not take calls on the facility phone, R1 has a personal cell phone and prefers to take calls in R1's private room. If callers refuse to leave contact information, R1 cannot return the calls. Based on interviews with staff and R1 it was determined the allegation staff is not allowing R1 to receive telephone calls is unsubstantiated at this time. No deficiences noted.
