The department conducted an unannounced complaint visit on Monday, January 06, 2025, Upon arrival at the facility, The department conducted a risk assessment. Based on the evaluation, the facility is clear of COVID-19 infection. The department met with Administrator Gwen Craig and explained the purpose of today's visit. The investigation consisted of the following: Interviews were conducted with staff members S1-S2 and residents 1-6 (R1-R6). Residents' records were requested and reviewed. LPA Shirley and LPM Cifuentes arrived at the facility and spoke to Social Services Director Elizabeth Hernandez to explain the purpose of the visit it to investigate the allegation listed above. LPA and LPM were granted access to the facility. Administrator Gwendolyn Craig arrived later to join the visit. The investigation consisted of the following: On 5/16/2023 LPA Shirley and LPM Cifuentes met with Social Services Director Elizabeth Hernandez. LPA and LPM interviewed Resident 1- Resident 5 (R1-R5) and attempted to interview Staff 1-Staff 5(S1-S5). LPA identified himself and discussed the purpose of the visit and the elements of the allegation(s) with Administrator Gwendolyn Craig. LPA conducted interviews with Administrator (S1) and reviewed documents from the facility such as admission agreement, discharge report, and resident's financial detail statement including any other pertinent documents such as undelivered closing check dated 9/14/2022 remaining in the file and not given to resident at discharge. Resident was discharged on 11/18/2021 Findings: Based on interviews and records reviewed, the preponderance of evidence standard has been met. LPA finds the alleged violation did occur; therefore, the allegation is SUBSTANTIATED. The following deficiencies were cited on 9099-D, per Title 22 Regulations, Division 6.
