At approximately 8:45 a.m. on 06/06/25 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced complaint visit. LPA met with staff and disclosed the reason for the visit. Prior to this investigation, LPA reviewed the eviction notice of Resident #1 (R1) at approximately 4:00 p.m. on 05/01/25. Licensing Program Analyst (LPA) Tihesha Smith made an unannounced complaint visit to this facility at 11:20 am to deliver findings. LPA Smith met with facility staff and disclosed the purpose of this visit. The administrator was present at the facility. Personal Rights: Due to neglect in care and supervision resident sustained multiple fractures The initial visit was conducted by Licensing Program Analyst (LPA) Tihesha Smith on 06/30/2023, at which time LPA Smith conducted a physical plant tour at around 11:00 am and conducted an interview with the administrator at approximately 10:30 am. On 07/03/2023 this case was referred to the Community Care Licensing Investigations Branch (IB). Investigator Olivia Spindola continued the investigation by conducting records review and interviews on 07/14/2023,07/24/2023, 08/07/2023, 09/26/2023,09/27/2023 and 09/28/2023. LPA met with staff Michelle Egan and explained the reason for the visit. LPA conducted physical plant tour at 9:00 AM, requested copy of facility documents relevant to the investigation at 9:38 AM and interviewed residents and staff from 10:00 AM to 1:30 PM. Regarding the allegation that Staff failed to obtain medical treatment for resident, it was alleged that Resident #1 (R1) was ill for two (2) weeks and was not taken to the doctor. LPA's record review on 05/17/22 at 10:15 AM and today at 11:00 AM revealed that R1 was consistently checked upon by medication technicians whenever R1 was given medication. LPA met with Executive Director Gina Grundeis and explained the reason for the visit. LPA conducted physical plant tour at around 9:30 AM, requested copy of facility documents relevant to the investigation at 10:00 AM and interviewed staff between 11:00 to 1:00 PM. Regarding the allegation that the facility failed to properly prepare food in result of E. Coli, it was alleged that Resident #1 (R1) acquired E. Coli from the food served at the facility. LPA's record review today at around 10:15 AM revealed that the facility had no case of E. Coli since August of 2019 up to the present time. Further review also revealed that all the key kitchen staff were trained in food safety/handling. Regarding the allegation that facility staff fails to meet resident's hygiene needs, it was alleged that Resident #1 (R1) was not cleaned properly and left in soiled diaper for extended period of time. LPA's record review today at 9:15 AM revealed that R1 was capable self care including but not limited to toileting and dressing and needed assistance on bathing.
