Licensing Program Analyst (LPA) Angela Barutyan arrived at the facility unannounced to conduct an initial complaint investigation for the allegation listed above at 01:25PM. LPA was greeted by staff. Entrance interview conducted. Reason for the visit was explained. During today's visit, LPA conducted a brief physical plant tour, interviewed two (2) staff and three (3) out of six (6) residents between 01:32PM-02:10PM, and reviewed and obtained copies of pertinent documents. It was alleged that staff do not respond to Resident #1 (R1)’s call buttons timely. Per regulation, facilities licensed for 16 or more or having separate floors/buildings shall have a signal system. Allegation investigated: Resident fractured their hip due to lack of care from staff

Licensing Program Analyst (LPA) Angela Barutyan conducted a subsequent complaint visit to deliver investigation finding. Upon arrival, LPA met with staff who called Administrator Michelle Mauer. Entrance interview conducted. On 01/30/2025, Community Care Licensing Division received a complaint alleging Resident #1 (R1) sustained a hip fracture due to staff neglect. The complaint was referred to the Community Care Licensing (CCL) Investigations Branch (IB) and assigned to Investigator Edward Hector. On 01/31/2025, LPA T. Camara conducted an initial complaint visit to this facility for the above allegation. During the visit, LPA Camara spoke with staff at 11:23AM and spoke with the co-administrator at 11:25AM, reviewed and obtained pertinent documents at 11:37AM, conducted a brief tour of the facility/health and safety check at 11:49AM and interviewed one (1) resident at 11:50AM. Licensing Program Analyst (LPA) Ashley Smith arrived unannounced to conduct an initial complaint visit. The LPA met with staff Pam Hashemi and explained the reason for the visit. Today, the LPA obtained documents, interviewed Resident #1’s responsible party at 12:35 p.m., and interviewed staff at 12:10 p.m., 12:20 p.m., and 12:50 p.m. Regarding the allegation, it was alleged that Resident #1 (R1) suffered a laceration between two fingers, yet there were questions as to how R1 sustained the injury. The LPA interviewed staff whom were with R1 when R1 sustained the cut. On 11/10/2022 at approximately 3:45 p.m., Staff #1 (S1) was assisting R1 with a diaper change. Staff claimed that when S1 attempted to put the diaper on R1, R1 grabbed the diaper, and in the process, R1’s fingers went in between the diaper and suffered somewhat of a paper cut. S1 said that R1 had attempted to take the diaper off, which is when R1 sustained an injury. Licensing Program Analyst (LPA) Ashley Smith arrived unannounced to conduct an initial 10-day visit. The LPA met with staff and explained the reason for the visit. To investigate, the LPA conducted a tour at 9:10 a.m., interviewed residents at 9:30 a.m. and 9:33 a.m., and interviewed staff at 9:45 a.m. It was alleged that staff in the facility were not wearing masks while in close contact with the residents. Upon arrival to the facility at 9:10 a.m., the two staff on site (Staff #1, Staff #2) were observed not wearing masks. The LPA observed that at approximately 9:30 a.m., S1 and S2 were wearing masks. Yet information obtained from a credible witness confirmed on 5/10/2022 and 9/21/2021, the credible witness noted that S1 and S2 were observed not wearing masks while in close proximity of residents. Based on the investigation, there is sufficient evidence to support the claim that staff were not wearing the required personal protection equipment (PPE).
