At approximately 11:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct an investigation into the above allegations. LPA met with Administrator Nadia Roop and reviewed records. Based on records reviewed and interviews conducted, LPA found evidence to support the allegations listed above. The investigation revealed, staff left a small cup of medication containing pills in a different residents room. The cup was discovered by a residents family member and given to staff. Based on records reviewed, the morning medication on 01/23/2026 was documented as given, but interviews conducted showed R1 refused the medications and they were put in a plastic baggy and given to the bus driver to take to the day program. Staff of the day program were not given any documentation for the pills and did not administer. The pills were properly destroyed. Allegation investigated: Staff sexually abused resident in care
At approximately 1:15PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to deliver finding from an investigation conducted by the Department into the above allegation. LPA met with Administrator Amanda Mitchell and interviewed staff. Based on interviews conducted by the Department, R1 denied ever being raped at the facility and denied ever saying they were raped. Staff interviewed stated since this incident they have adopted two person assistance for R1 when assisting them. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated. Allegation investigated: Staff are verbally abusing residents and physically intimidating them
At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to deliver findings from an investigation into the above allegation. LPA met with Jessie Regnier. Based on interviews conducted and documents reviewed, there was no evidence that staff verbally abused or intimidated resident. On the contrary, based on records reviewed and interviews conducted, R1 was verbally abusive to staff and when asked to stop, would accuse staff of being assaultive. R1 would threaten to hit staff if they did not do what was asked. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated. Allegation investigated: Residents needs not being met
At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to deliver the results of an investigation into the above allegation. LPA met with Amanda Mitchell. The concerns of this investigation were in relation to Resident 1, R1, leaving the facility in disarray. R1 allegedly was covered in food scraps from breakfast, including on their face, on their shirt and pants and hands. LPA reviewed video footage from the morning in question and although the video did not provide a close up of R1, LPA did not observe food on their person. Interviews conducted with staff on duty, S1, on the morning of the incident, informed LPA that morning was a bit rushed. The bus driver arrived early and was honking their horn for the resident to be brought out. S1 could not recall if they cleaned R1 after breakfast, because R1 was just finishing when the bus arrived.
