Type of visit: Required - 1 Year
At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a Required-1 Year inspection. LPA met with House Manager Shadie Aragon and explained the purpose of the visit. Facility has a Hospice waiver for 4 residents. At approximately 8:15AM, LPA toured the facility to ensure the health and safety of residents in care. The facility was observed to be at a comfortable temperature. Areas toured include but are not limited to resident rooms, common areas, bathrooms, kitchen, storage areas and back yard. In the areas toured no immediate health, safety, or personal rights violations were observed. Fire extinguishers were fully charged. Smoke detectors are all operational. Carbon Monoxide Detector was present. The common areas, bathrooms and kitchen were clean and in good repair. All bedrooms had required furniture, bedding, and lighting. The kitchen contained cooking/dining equipment that was clean and orderly, utensils were present. Food appears to be stored and prepared properly. Refrigerators and freezers were maintained at the proper temperature. Facility has required seven-day non-perishable and two-day perishable supply of food. Emergency food stores and water was present to ensure facility can be self-sufficient for 72 hours. Facility has a generator to supply power in an emergency. Emergency lighting devices were present. First aid kit was present. All employees requiring background checks are cleared. No pools/bodies of water are on the premises. Facility has been conducting Emergency drills quarterly. At approximately 8:45AM, LPA reviewed 5 of 10 resident files. All resident files contained the required documentation. Reappraisals were all conducted within the last 12 months. Documentation of physician visits within the last 12 months were present. At approximately 9:30AM, LPA conducted a review of medications. Medication is locked and not accessible. Continued on LIC809-C... At approximately 9:30AM, LPA reviewed 5 staff files. Staff files reviewed contained evidence of completed annual training. First Aid/CPR certification was current. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC610- Emergency disaster plan LIC500- Personnel Report LPA received Evidence of Liability Insurance during visit. Allegation investigated: Personal Rights Licensee did not report verbal abuse
At approximately 2:00PM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to deliver investigative findings from an investigation into the above allegations. LPA met with Administrator Heather Craven. Based on interviews conducted and records reviewed, LPA did not find evidence to support the above allegations. LPA interviewed all staff working at the facility and learned no instances of verbal or physicial abuse took place and staff enjoy working with their co-workers. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. Allegation investigated: Facility staff are not repositioning resident overnight resulting in injury Facility staff are leaving residents soiled for an extended period of time
At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to open an investigation into the above allegations. LPA met with House Manager Shadie Aragon and reviewed records. Based on records reviewed and interviews conducted, Resident, R1, is able to reposition themselves in bed. R1 needs assistance with transfers and some with mobility. There are no orders to assist with turning or repositioning in bed. Facility keeps a daily communication report that documents how often residents are to be checked on and details which staff assisted resident with bathroom and showers. LPA reviewed records and observed facility was in constant communication with R1's physician regarding swelling and blisters on their left arm. R1 was seen at the Hospital on 05/12/2025 and was presribed antibiotics. LPA reviewed daily communication logs and observed residents are checked every 2-3 hours and assisted with personal needs as they arise. Based on records reviewed, LPA did not find evidence to support the above allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated. Allegation investigated: Licensee is not addressing a contagious outbreak at the facility.
At approximately 11:45AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to open an investigation into the above allegation. LPA met with Administrator Heather Craven, conducted interviews and reviewed records. Based on records reviewed and interviews conducted, resident, R1, went to the hospital on 01/31/2025 for a planned surgery. R1 returned the next day and stayed for a couple days. Facility staff observed resident was continuing to have issues that were not related to a contagious disease, and notified the physician. On 02/07/2025, R1 was re-admitted to the hospital for IV antibiotics. Licensee was informed on 02/14/2025, by the responsible party, that R1 was dianosed with scabies while in the hospital. Based on documents reviewed, Licensee informed all staff of the situation and ensured proper PPE supplies were available for use. There are no other residents with rash like symptoms in the facility. On 02/18/2025, Licensee contacted local Public Health for guidance and notification of the situation. Facility staff increased monitoring of residents and increased cleaning of bedding and surface areas. Licensee notified the Department on 02/17/2025. Based on the Departments investigation, Licensee followed their Infection Control Policy. This agency has investigated the above allegation. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. At approximately 8:00AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced to conduct a Required-1 Year inspection. LPA met with Administrator Heather Craven and explained the purpose of the visit. Administrator certificate is current. LPA toured the facility to ensure the health and safety of residents in care. Areas toured include but are not limited to resident rooms, common areas, bathrooms, kitchen, storage areas and back yard. In the areas toured no immediate health, safety, or personal rights violations were observed. Staff and resident files were reviewed. First Aid/CPR certification was current. Medications were also reviewed. The common area was clean and in good repair. All bedrooms had required furniture, bedding, and lighting. The bathrooms were clean and in good repair. The kitchen was clean and in good repair. Food appears to be stored and prepared properly. Facility has required (7) seven-day non-perishable and (2) day perishable supply of food. Medication is locked and not accessible. The facility was observed to be at a comfortable temperature. First aid kit fully stocked and ready for emergency use. Fire extinguishers were fully charged. Smoke detectors are all operational. Carbon Monoxide Detector was present. Facility has fire sprinklers throughout. All employees requiring background checks are cleared. No pools/bodies of water are on the premises. Disaster drills are conducted monthly. Based on this inspection, the facility is in compliance.
