Type of visit: Required - 1 Year
On 11/7/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Licensee (L1) Nora Sanchez. LPA was granted entry. 0 clients are currently not placed in facility. LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. No food was observed in refrigerator or freezer because of no residents in facility. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 37 degrees F. First Aide kit observed, but did not contain a thermometer. Cleaning chemicals was observed stored and locked under kitchen sink. Fire extinguisher was observed without a purchase or service date. Fire drill currently not conducted because of vacancy of residents in facility. Clients' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 108.5 degrees F and 109 degrees F in 3 bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for clients. Medications not present in facility because of vacancy. Facility does have a Centrally Stored Medication Record designation in locked closet. Carbon monoxide and smoke detectors were tested and observed to be operational. No clients’ file reviewed because of vacancy in the facility. No staff or client files were reviewed as facility does not currently have staff. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 11/21/25: Lic 308, Lic 500, Lic 610D, Lic 9020 and Administrator Certificate. Type of visit: Post Licensing
On 12/4/24 Licensing Program Analyst (LPA) M. Garza conducted an unannounced Post-Licensing inspection. LPA met with Licensee, Nora Sanchez. LPA completed tour of the facility inside and out including the kitchen, bedrooms, bathrooms and laundry area. LPA observed the facility to be free of odor, clean and in good repair. LPA observed sufficient furniture and lighting throughout the facility. LPA observed the required 7-day non-perishable and 2-day perishable foods. Medications and knives locked and inaccessible to residents. Water temperature measured at 115.8 degrees F in the restroom #1. Fire extinguisher present and purchased 1/4/24. Review of resident/staff files and medication records. LPA observed the following posted in the facility: Resident Council Rights, See Something Say Something complaint poster, Reporting Requirements per AB40, Resident Bill of rights, Ombudsman Poster, Resident Personal Rights and facility license posted as required. The following issues were observed during today’s visit: The following issues were observed during today’s visit: Facility missing lighting and hand railing at front walkway stairs. Staff observed assisting residents without fingerprint clearance. R1 observed walking in hallway with missing pants. R1 observed sleeping on mattress on the floor without exception. Deficiencies and TV’s provided per Title 22. Exit interview completed with Licensee, Nora. Type of visit: Prelicensing
On 09/18/24 Licensing Program Analyst (LPA) M. Garza arrived for an announced pre-licensing inspection visit. LPA was met by Licensees, Nora Sanchez and Administrator, Divina "Grace" Petil. LPA introduced self, reason for visit and were permitted entry into the facility. Facility is in the process of pre-licensing. Currently there are no residents residing at the facility. Fire clearance was approved for 6 non-ambulatory residents. During visit on 9/11/24 facility was not ready to be licensed and corrections were needed. Todays visit is being conducted to clear issues previous observed. 1) Facility has ramp driveway for accessibility. Yard and stair lights have been added. Reflectors added at stairs for visually impaired residents. 2) All postings have been posted per regulations. 3) Grab bar was installed and sturdy. 4) Facility provided an electronic device (iPad) for resident use. 5) First aid kit is now complete with thermometer. 6) Gaps were filled in with mulch and rock. Gaps no longer observed. 7) Side gate had spring installed to make self latching. 8) Patio observed with umbrella covering seating area. 9) Debris has been removed and no longer observed in back yard. 10) Tools removed from facility and no longer observed. 11) Plan of Operations and Disaster Plan has completed. 12) Documentation added to the admission agreement. At this time the facility is ready to be licensed. LPA will inform Sacramento. Comp III completed with Licensee and Administrator. Exit interview completed with Licensee and Administrator. Type of visit: Prelicensing
On 09/11/24 Licensing Program Analysts (LPAs) M. Garza and R. Bruce arrived for an announced pre-licensing inspection visit. LPA was met by Licensees, Nora Sanchez and Administrator, Divina "Grace" Petil. LPAs introduced selves, reason for visit and were permitted entry into the facility. Facility is in the process of pre-licensing. Currently there are no residents residing at the facility. Fire clearance was approved for 6 non-ambulatory residents. Tour of facility inside and out was completed. Common areas observed with adequately furnishings and lighting. LPAs observed an extra supply of linens and personal hygiene/grooming products. Kitchen observed with dishes, plates and utensils. Cleaning supplies/chemicals observed in a locked garage cabinet. Medications will be kept in a locked closet in the hallway. Water temperature tested at 108 and 110 degrees F in resident restrooms. Fire extinguisher present and purchased 1/4/24. Smoke and carbon monoxide detectors are present and operational at time of visit. Exits free of obstructions. Facility has a functioning land line phone. Phone number is: (559) 800-8143. The following issues were observed during todays visit: Stairs in front entry observed without hand railing and lighting. Non-discrimination, Personal Rights of Residents in all Facilities, Additional Personal Rights of Residents in Privately Operated Facilities, visiting policy and complaint postings missing. Grab bar in bathroom #1 missing. No electronic device for resident use observed. First aid kit missing thermometer. Gap around air conditioning unit and right side yard observed with a 2-4 inch drop to level ground. Side gate is not self latching. No covered seating area. Debris observed in back yard in need of removal. Tools observed accessible and unlocked in backyard. Plan of Operation and Disaster Plan incomplete. Patients rights are missing from Admission Agreement. Due to corrections being needed, the Component III was not conducted during this pre-licensing visit. Pre-Licensing rescheduled for 09/18/24. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
