Allegation investigated: Uncleared staff providing care to residents

On 08/04/26, Licensing Program Analyst (LPA) M. Medina conducted an unannounced initial 10-day complaint visit. LPA stated purpose and allowed entrance into facility by direct care staff. Administrator, Joseph Gitti arrived a short time later to conduct the complaint visit with LPA. During the course of the investigation, documents were reviewed, interviews conducted, and information gathered. During review of documentation it was discovered that Staff 1 (S1), Staff 2 (S2) and Staff 3 (S3) are all fingerprinted, cleared and associated to facility. Staff 4 (S4) was fingerprinted and associated to facility on 7/27/25 and disassociated on 1/12/2026. S4 has not been employed at facility since approximately November 2025. This Department has found that the above allegations are UNFOUNDED, meaning they were false, could not have happened, and/or were without reasonable basis. We have therefore dismissed the complaint. On 12/17/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Epifane Ballena. LPA was granted entry. 6 residents were present during inspection. Administrator (A1) Harmeen Jhutti arrived shortly LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 48 degrees F. Fire extinguisher was observed with a purchase date of: 1/8/25. Fire drill last completed on 12/25. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature range of 118.5 and 118.9 degrees F. in 2 bathrooms. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. Samples of residents and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 12/31/25: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. Licensing Program Analyst (LPA) Katie Brown arrived unannounced to conduct the Annual Inspection. LPA met with and explained the reason for the visit with Administrator (AD) Joseph Gitti. During this visit, LPA toured the facility inside & out. Resident rooms and common areas were clean, in good repair and contained required equipment, furnishings and lighting. LPA observed required items in bathrooms which were clean with hot running water. LPA observed hygiene items, towels, extra bedding, and linens which were stored and available for use. The kitchen was found to be clean, in good repair with necessary items and appliances. LPA observed required food supply and paper products. Knives, cleaning/disinfecting supplies and chemicals were locked are stored separate from food. Medications are centrally stored in a locked cabinet in a hallway closet. Doors and passageways are unobstructed throughout the facility including outdoors. First aid kits contained required items. LPA walked the outdoors to find the grounds well-kept with clear walkways, sitting area and a gate with working latch. Fire extinguishers were found to be charged and were serviced 1/4/24. Smoke and carbon monoxide detectors were observed and tested. LPA conducted resident and staff file reviews and a medication audit. Emergency Disaster and Infection Control requirements were reviewed during the inspection. There were no citations during this inspection. An exit interview was conducted and a copy of this report was signed and provided. Licensing Program Analyst (LPA) Katie Brown arrived unannounced to conduct the Annual Inspection. LPA met with and explained the purpose of the visit with Facility Designee, Harmeen Juutti and Nina Nahrin. During this visit, LPA toured the facility inside & out. Resident bedrooms contained required furnishings and lighting. LPA observed required items in bathrooms which were clean with hot water temperature reading of 116 degrees. Resident hygiene supplies were properly stored and available. The kitchen was observed to be clean, in good repair with necessary items and appliances. LPA observed required food supply and paper products. Knives, cleaning/disinfecting supplies and chemicals are locked and stored separate from food. Medications are centrally stored and locked. First aid kit contained required items. Facility has designated visitation areas available inside and out. Outside of the facility toured. LPA observed a self-releasing gate and windows have screens in good repair. Smoke and Carbon Monoxide detectors present and in working order. Fire extinguisher date 1/13/23. LPA conducted resident and staff file reviews including medication audit and interviews. There were no citations during this inspection.
