Licensing Program Analyst (LPA) Christine Yee conducted an unannounced required Annual Inspection using the complete CARE Inspection Tool. LPA Yee was let into the home by Gohar Hovhannisyan, Staff. Siranush Bagdasarian, Administrator was contacted by staff and she arrived at 10:55am to conduct the visit. The facility is a single storey family home consisting of a living room, a dining room, a kitchen, 3 resident bedrooms, 2 common bathrooms and a attached garage. The facility is fire cleared for 5 non-ambulatory and one bedridden resident. Bedroom #3 is the room designated for the one bedridden resident. No bodies of water were observed on the property. On today's visit all 12 domains of the CARE Inspection Tool was reviewed, 6 resident files and 7 staff files were also reviewed. A tour of the physical plant, inside and outside, was conducted. The following was observed on today's visit: The living room, dining room and kitchen were all appropriately furnished for 6 residents and equipped with the appropriate appliances for food preparation and cooking meals. The fire place located in the living room had the required fire screen. The sliding glass door in the living room opens on to a ramp. All 3 bedrooms are shared rooms and contains 2 of each: beds, night stands, lamps, dressers, chairs and a built in closet. Bedroom #3, designated as the bedridden room has an outside exiting door that leads onto a ramp. continued on LIC809-C Page 2. The facility has 2 full bathrooms. The common bathroom located in the resident hallway has a bathtub that includes a shower head, a vanity with a single sink and a toilet. Grab bars were observed in the shower stall and the by the toilet. A shower chair and a slip resistant mat was also observed. The water temperature was tested and read 109.2 degrees Fahrenheit. The second full bathroom located by the kitchen also has a shower stall, a toilet and a vanity with a single sink. The second bathroom is designated for staff use and does not have grab bars. The attached garage accessible through the kitchen is primarily used for storage of the non-perishable foods, extra perishable foods, water supply, hygiene products, disinfectants and cleaning solutions also houses the washer and dryer and is kept locked. Sufficient perishable foods for a minimum of 2 days were observed in the kitchen and garage refrigerator and non-perishable foods for a minimum of 7 days were observed maintained on the premises. An emergency container with 120 servings of dehydrated foods was also observed. Disinfectants and cleaning solutions are also stored in a locked cabinet under the kitchen sink. Sharp knives are stored in a locked kitchen drawer The linen closet located by the kitchen contains towels and extra bed linens. The locked closet located by the front door is used to secure the centrally stored medications. The facility has 2 fire extinguishers, one in the resident hallway and one in the dining room, were last serviced on 7/14/26. The only carbon monoxide detector located in the dining room was tested and was operational. The first aid kit was reviewed and contained the required tweezer, scissors, thermometer, ointments, gauze and dressings. A first aid manual was observed. Per file review, all stall have current First Aid/CPR training. The facility has current general liability insurance that meets Title 22 limits. The auditory devices mounted on the front door, kitchen door, bedroom #3 and back sliding glass door were all tested and were operational. The hardwired smoke detectors located in all 3 resident rooms, resident hallway, by front door, living room and dining room were tested and were operational. continued on LIC809-C Page 3. The backyard has a covered patio furnished with wicker chairs and a coffee table. The two side gates located on each side of the home were observed to be locked from the inside and allowed residents to exit. The backyard was observed to be well maintained. the trash cans stored in the front yard were observed to be tightly sealed. The front yard was also observed to be clean and well maintained. The facility is a single storey family home consisting of a living room, a dining room, a kitchen, 3 resident bedrooms, 2 common bathrooms and a attached garage. The facility is fire cleared for 5 non-ambulatory and one bedridden resident. Bedroom #3 is the room designated for the one bedridden resident. No bodies of water were observed on the property. The fire place had the required fire screen. all three bedrooms were all equipped with 2 each of the following: hospital beds, night stands, dressers, lamps, chairs and a built in closet. All the windows had blinds for privacy. The common bathroom by the resident rooms is equipped with a tub/shower, grab bars, shower chair, slip resistant mat, a toilet, another grab bar and a single sink vanity. Water temperature was tested and read 111.4 degrees Fahrenheit. continued on LIC809-C the second bathroom, located by the kitchen, is equipped with a shower stall, a toilet and single sink vanity and is designated for staff use. Water temperature was tested and read 115 degrees Fahrenheit. the attached garage is used primarily for storage and also houses the washer and dryer. Detergents, disinfectants , hygiene products and cleaning solutions are stored in a locked cabinet. the perishable and non-perishable food supply stored in the garage and in the kitchen refrigerator were reviewed and met Title 22 requirements. The facility also has a single bucket containing emergency food supply. Per information provided, it contains 120 servings. the facility has 2 fire extinguishers purchased on 8/17/25 and they are located in the the dining room and in the resident hallway. the only carbon monoxide detector is located in the dining room. It was tested and was operational the first aid kit was reviewed and met Title 22 requirements. First aid manual was observed. the smoke detectors located in the 3 resident bedrooms, hallway and living room were tested and were operational. the auditory devices on the four outside exiting doors - Bedroom #3, front door, kitchen door and the sliding glass door in the living room were operational. All residents in the home were visually observed to be doing well. The backyard has a covered patio with wicker chairs and a coffee table. The backyard was observed to be well maintained. the trash cans stored in the front yard were observed to be tightly sealed. The front yard was also observed to be clean and well maintained. The facility is a single storey family home consisting of a living room, a dining room, a kitchen, 3 resident bedrooms, 2 common bathrooms and a attached garage. The facility is fire cleared for 5 non-ambulatory and one bedridden resident. Bedroom #3 is the room designated for the one bedridden resident. The following domains were reviewed on today's visit: Infection Control, Operational Requirements, Staffing, Personnel Records/Staff Training, Resident Rights/Information, Planned Activities, Food Service, Incidental Medical and Dental, Resident Records/Incident Reports and Disaster Preparedness. Due to time constraints, the following domains will be reviewed on a return visit: Physical Plant/Environmental Safety and Residents with Special Health Needs. Also reviewed on today's visit were 6 resident files and 5 staff files. Deficiencies noted during the reviewed domains and file review are being cited under California Code of Regulations, Title 22, Division 6, Chapter 8. Any deficiencies not addressed on today's visit will cited on a return visit. CIVIL PENALTIES WERE ASSESSED on today's visit.
