Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Leslie Ngo-Castaneda conducted an annual required visit and inspection of the facility. LPA met with staff, Seolrock and Knarik Vardanyan, and explained the reason for the visit. Seolrock is not associated or fingerprinted to be at the facility. Deficiency will be cited in LIC 9099-D. Approximately, around 1:20 PM staff designee, Ovsanna Khayalya, arrived and was explained for the reason of the visit. At 1:30 PM, with the assistance of of staff designee, LPA took a tour of the physical plant. Required postings were observed in the entry area. At 1:59 PM smoke alarms were tested and are operational. There are carbon monoxide detectors that functions properly. The fire extinguisher is beside the kitchen. During the visit the facility is at 72 degrees Fahrenheit. The facility is fire cleared for six (06) residents of which two (2) ambulatory; three (3) non-ambulatory, one (1) bedridden resident in bedroom #2. Approve for four (4) hospice waiver. Kitchen: The kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found enough at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Knives were stored in a locked drawer in the kitchen. LPA observe R2 in refrigerator unlock and accessible to residents in the facility. Deficiency will be cited in LIC 9099-D. Bedrooms: There were five (5) bedrooms designated for clients' and staff use. Four (4) of the that are in use by residents were properly furnished with appropriate dresser, beddings, and linens with sufficient lighting. Room #1, bedroom #2, and bedroom #4 are properly furnished and is for private used. Room #3 is shared. Bedroom #5 is for staff use. Staff bedrooms are locked with no medication in sight. Continue to LIC 809-C Bathrooms: There are two (2) bathrooms designated for residents' and staff use. Bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured at 105 degrees Fahrenheit for bathroom #1 located in the hallway beside bedroom #3. Bathroom #2 is for staff used. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. Common Areas: These included the living room and dining area for residents. The common areas were properly furnished. The auditory alarms on all exit doors were on and functional at the time of the visit. Residents dining table fits enough for six (6). Surrounding Grounds: Entry and exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards. The laundry area and detergents are located by the laundry room in the locked garage. Cleaning supplies are being stored in an locked cabinet in the garage. The facility does not have a swimming pool or body of water. Office space is in the entrance area beside the entrance of the facility. Properly labeled medications were locked in the kitchen cabinets. The facility has a garage and is used for storage. Resident Files: LPA conducted a file review of resident records to ensure compliance of licensing forms. Staff Files: LPA also conducted a file review of staff records to ensure forms and training are up to date and compliance with licensing forms. S2 is missing LIC 503 (health screening and TB test). Deficiency will be cited in LIC 9099-D. Medications: Medication and Medication Records (MMR) were review for proper documentation. R1 does not have CSMDR. R3 medication does not match CSMDR. Deficiency will be cited in LIC 9099-D. Pursuant to Title 22 Division 6 of the CA Code of Regulations, deficiencies observed during the visit. Deficiency will be cited in LIC 809-D. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Michael Cava attempted to conduct a Required Annual visit at the facility. At arrival, LPA waited at the front door for approximately ten minutes but no answer. LPA called telephone number on file, and was able to get a hold of the administrator, Eduard Hakobyan. He stated his application for the Assisted Living Waiver (ALW) was just approved, but there are no residents currently in the facility. He adds he would like to continue to carry his license with the state, but facility is not operating at this time. LPA observed the home from the outside. Property appears to be vacant, and not operating. No cars parked in driveway, and trash was empty. LPA will make another visit to facility on a later date to complete annual. This report will be left at the front of the home, and will be mailed. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Eduard Hakobyan and explained the reason for the visit. The facility is currently and has been vacant since their license was approved. Although license is active, the licensee isn't operating at this time. Licensee is still awaiting process of the Assisted Living Waiver (ALW) application. Also, facility has gone under remodeling to replace/repair the floors in the living room and in room #1, caused by flooding. At approximately 09:15am, with the assistance of the administrator, LPA took a tour of the physical plant. The facility is a one story building. The smoke alarms are interconnected and battery operated. It is dual with the carbon monoxide detector. The fire extinguisher is located in the kitchen. Kitchen: The kitchen appliances and fixtures are functional. Because no residents at this time, licensee doesn't store any perishable and non-perishable food. Licensee advised to store a sufficient amount of perishable and non-perishable food items once they admit their clients. Bedrooms: There are five (5) bedrooms. Four (4) bedrooms are designated for residents' use, and one is for staff. Rooms #1 and #4 are private. Rooms #2 and #3 are shared. Room #1 is having the floors replaced due to flooding, therefore, it is not furnished at this time. Once floors are repaired, furniture will be put back in place. Rooms #2, #3 and #4 were properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two(2) bathrooms. One designated for staff and one designated for residents. The bathroom designated for residents was properly supplied and had functional fixtures. Hot water temperature was measured at 120 degrees Fahrenheit. No cleaning supplies stored in the bathrooms. Common Areas: These included the living room and dining area. Currently the living room floor is under repair due to flooding, therefore the furniture had to be moved at this time. Once repairs are completed, administrator will put back the furniture. The dining room area has a dining room table, large enough to seat up to six (6) individuals. No fireplace or open heaters. Air conditioning and heating unit is centralized. Internal temperature is 72 degrees during the inspection. Surrounding Grounds: Entry/exits were free of obstruction. Floors in the dining room, kitchen, hallway and bedrooms were mopped and clean. There was furniture appropriate for outdoor use. There is no swimming pool or any other bodies of water. The outdoor area was free of hazards. Garage: The garage is attached to the building. There is a passageway, through the kitchen. Passageway through the kitchen will be inaccessible to the residents. Garage will be used for extra storage for detergents and cleaning supplies. Laundry: The laundry area is also located in the garage. Resident Files: No files to review at this time Staff Files: No files to review at this time. Medications: No medications and medication documents to review at this time. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit.
