Allegation investigated: Unlicensed care

Licensing Program Analyst (LPA) Angela Panushkina conducted a subsequent complaint visit to follow up on an Unlicensed Care Complaint. LPA met with the Operator, Irina Raskopina, and explained the reason for the visit. On 12/26/25, an initial complaint visit was conducted and interview with the Operator revealed that an application for Licensure “SUNNARA GOLD INC” # 197610839 was already submitted on 06/13/2025, with Community Care Licensing Division, and the Pre-Licensing visit was conducted on 11/13/2025, however, the license was still under the pending status. During the subsequent visit conducted on 12/30/25, interviews with staff revealed that five (5) out of five (5) individuals residing here require and were receiving elements of care and supervision. Elements of care and supervision include assistance with Activities of Daily Living (ADL), such as bathing, dressing, using the toilet, and arranging medical and dental services. Continue on LIC9099-C Substantiated Additionally, LPA reviewed STD850 "Fire Clearance" that was submitted on 10/15/25 and observed that on 10/22/25 the fire clearance granted for six (6) Ambulatory ONLY. Therefore, Notice of Violation of Law (NOVL) was issued, and Operator was advised to cease the operation. During today’s visit, LPA conducted physical plant tour and observed that there are currently four (4) residents residing at the facility. LPA conducted review of documents and observed that the license (#197610839) was issued on 01/29/2026 for capacity of six (6) ambulatory only. LPA also observed that a new Fire Clearance (STD850) request was submitted on 03/04/2026 for the capacity of six (6) residents of which two (2) can be Ambulatory, three (3) – Nonambulatory and one (1) – Bedridden. STD850 was granted as of 03/20/2026. New license will be issued to reflect the Fire Clearance approval. This complaint will remain Substantiated based on an initial finding. Exit interview conducted. Allegation investigated: Unlicensed care

Licensing Program Analyst (LPA) Angela Panushkina conducted a subsequent complaint visit to this house to follow up on an Unlicensed Care Complaint. LPA was greeted by Staff #1, who granted access to this home. LPA contacted Irina Raskopina and explained the reason for the visit. Staff #1 was assigned to sign this report. On 12/30/25, a visit was conducted and interviews with Mrs. Raskopina and staff revealed that five (5) out of five (5) individuals residing in this home required and were receiving elements of care and supervision. Although the pre-licensing visit was conducted on 11/13/25, the license was still under the pending status. LPA informed Mrs. Raskopina that Fire Clearance (dated 10/22/2025) was approved for five (5) ambulatory residents ONLY. Notice of Violation of Law (NOVL) was issued, and Mrs. Raskopina was advised to cease the operation and/or request a new Fire Clearance. Continue on LIC9099-C Substantiated During today’s visit, LPA was informed that request for a new Fire Clearance for five (5) Non-ambulatory and one (1) bedridden resident was already submitted to Sacramento Community Care Licensing Division (CCLD) on 01/14/2026. Although the Plan of Correction (POC) was submitted and cleared, based on complaint reviews, the Substantiated findings shall remain the same. LPA reminded Mrs. Raskopina that the application for Residential Care Facility for the Elderly (RCFE) license for this location is currently pending, and no new residents shall be admitted at this time. Allegation investigated: Unlicensed care

At 9:30am, Licensing Program Analyst (LPA) Angela Panushkina, along with Joycelynn Mantuano, from the Adult Protective Services, conducted an unannounced visit at the above location. Upon arrival the team was greeted by a Staff (S1), who granted access to home. S1 contacted the Operator, Irina Raskopina, and LPA explained the reason for the visit. The Department received information that this location provides care and supervision to individuals living in the home without a license. At 09:45am, the team conducted a tour of the home and observed the following: The home has three (3) bedrooms and two (2) bathrooms. Home appeared to be clean and there was a sufficient supply of food. Based on interviews conducted and information obtained during the visit, it was determined that five (5) out of five (5) individuals residing in the home require and were receiving elements of care and supervision. Continue on LIC9099-C Substantiated Elements of care and supervision include assistance with Activities of Daily Living (ADL), such as bathing, dressing, using the toilet, and arranging medical and dental services. Additionally, LPA reviewed STD850 "Fire Clearance" that was submitted on 10/15/25 and observed that on 10/22/25 the fire clearance granted for six (6) ambulatory ONLY. Therefore, the allegation of "Unlicensed Care" is Substantiated. A Notice of Operation in Violation of Law (NOVL) was issued. The Operator was advised that a retroactive civil penalty of $100 per day per tenant shall be assessed from the original date of the NOVL (December 30th, 2025) for the operation of an unlicensed facility. If the Operator has not ceased operation within 15 calendar days of the issuance of this notice, $200 per resident per day will be assessed beginning on the 16th day until the operation ceased. The LPA reminded the Operator not to take in individuals who are in need of care and supervision in which a license is required. The Operator informed LPA that an application for Licensure “SUNNARA GOLD INC” # 197610839 was already submitted on 06/13/2025, with Community Care Licensing Division, and the Pre-Licensing visit was conducted on 11/13/2025, however, the license is still under the pending status. Deficiency issued on LIC9099-D. Exit interview conducted. Plan of correction: The Operator already submitted the application on 06/13/25. However, the application is still pending and the fire clearance was approved for six (6) ambulatory only. Operator will also submit a new LIC200 along with the facility sketch requesting five (5) Non-ambulatory and one (1) bedridden. Therefore, five (5) individuals residing at this home must be relocated before POC date.

Type of visit: Prelicensing

Licensing Program Analyst (LPA) Jose Tan conducted a Pre-Licensing Inspection with the applicant representative Anton Gorskikh. The applicant is "Sunnara Gold Inc". A fire clearance was approved on 10/22/25 for six (6) non-ambulatory residents, two (2) of which may be bedridden on either Room #1, #2 or #3. The applicant has an approved hospice waiver for six (6) residents. The facility is a single storey house and has a total of three (3) bedrooms and two (2) bathrooms. One of the bathroom is designated for staff use only. A tour of the physical plant was initiated at approximately 12:15 PM and the following was observed: KITCHEN: The facility has a Kitchen that is equipped with a refrigerator, microwave, stove, dishwasher and sink. Knives is kept in a locked cabinet with the medication. Cleaning supplies and other toxins will be stored in the cabinet in the laundry area which was locked during visit. BEDROOMS: There are three (3) bedrooms designated for client's use. The applicant furnished the resident bedrooms with beds, night stand, chairs, dressers, bedding and linen. The bedrooms have sufficient lighting and storage. BATHROOMS: The facility bathrooms for clients' use was observed to have the proper fixtures, grab bars, and non-skid mats. The hot water temperature was measured at a range of 119.7°F (continued on LIC 809-C) LAUNDRY ROOM: The laundry room is located adjacent to the kitchen. Laundry detergents and other cleaning agents were locked in a cabinet in the laundry room. The laundry room was observed to be locked during visit. COMMON AREAS: These included the living room and the dining area. Living room was furnished with chairs and side tables. The living room area was furnished with a television, a sofa to sit the capacity of the facility. There were no visible immediate hazards. There is a working telephone line and internet accessible to residents. Dual smoke/carbon monoxide alarms were tested and observed to be operable. Fire extinguisher was last bought on 09/22/25 located in the living room area. MEDICATIONS: The medication cabinet is located in near the kitchen and was observed to be locked during visit. A complete first aid kit is located in the living room. Staff/Resident Records: Staff and resident records will be kept in the same medication cabinet SURROUNDING GROUNDS: The driveway, passageways and entrance to the home was clear of obstruction. All entry and exit doors have a functional auditory alert when the doors open. The backyard of the facility has a covered patio and backyard furniture to accommodate the residents. There is no garage at this facility only street parking There is another tool shed storage at the backyard where other toxins are kept. It was observed to be locked and inaccessible to residents. There is no body of water in the facility. Component III was conducted with the Licensee representative. This report will be sent to Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when the license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst.
