On 12/11/2025, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit at the facility. LPA met with staff Carolina Castro and explained to Carolina the purpose of the visit. LPA was granted entry and conducted a tour of the facility alongside Carolina. LPA observed the following during the visit: Facility is a single-story structure which consists of (5) five resident bedrooms, (1) staff bedroom, (2) two bathrooms, living room, dining room, laundry room, kitchen, garage, and backyard. The facility kitchen was observed to be clean and free of debris. The facility met the required (2) two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. LPA observed an additional refrigerator in the garage that housed overflow foods. The facility maintains a sufficient stock of emergency food and water to meet all resident needs. Knives and other sharp items were maintained in a locked cabinet inaccessible to residents. Centrally stored medication was stored in a locked cabinet in the hallway, inaccessible to residents. Facility provides their own laundry services; all cleaning materials, disinfectants, and detergents were observed to be in the locked laundry room. LPA observed a shaded outdoor area that was fully furnished. Indoor/Outdoor passageways were free from obstruction. Resident bedrooms were equipped with the required bedding, furniture, and functional lighting. Bathrooms were equipped with the required grab bars and slip resistant mats. The facility is maintained at 75 degrees for the client’s comfort. Water temperature measured 109.2 degrees Fahrenheit. There were no bodies of water. Per Carolina, there are no firearms and/or ammunition on the premises. Resident records review included but not limited to preplacement appraisals, dental, medical assessment, admission agreements, and needs and service plans. Staff records review included but not limited to required training's, health screening, background clearance, personnel reports, and valid First-Aid/CPR Certification. Licensing Program Analyst (LPA) Kathleen Banrasavong arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction and visit purpose. Upon arrival LPA learned that five (5) residents live at this facility, however there was only four (4) residents at home during the inspection. There was two (2) staff members present. The Caregiver, Carolina Castro was advised of the annual and conducted and completed the facility tour. Client Records/Incident Reports/Clients Rights Information: LPA reviewed client records. Five (5) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans, placement, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. Personnel Records/Training/ Staffing/ Administration: LPA reviewed employee records. Two (2) records were reviewed. LPA reviewed employee records for first aid certification, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrative organization. Abigail V Mejares, Administrator’s certificate expiration date was 12/18/2025. Food Service: Food prep areas are clean and organized. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. Emergency food and water supply is present. There is a location for sharps in the kitchen. Physical Plant and Safety of Environment/Operational Requirements: LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The facility is maintained at 75 degrees for the client’s comfort. Lighting is sufficient for safety. Water temperature measured 108.0 degrees F. Laundry is done in the laundry room. There is a locked location for storing laundry soap, cleaning supplies and chemicals in the closet in the garage. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. LPA dialed the facility’s landline number, which rang and was operable. The LIC 610, emergency disaster plan is maintained. There are no firearms at this facility. There are one (1) secured fireplace at this facility. There are zero (0) pools at the facility. There is one (1) secured gate that has a self-latching lock located on the northeast side of the house. LPA observed emergency supplies and one (1) first aid kit. The facility conducted the last emergency drill on 08/01/2024. Infection Control: The LPA observed the hand washing stations in the facility restrooms and kitchen had hand hygiene supplies and hand washing signs. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements. LPA reviewed staff records and found that staff had infection control training. Medications/Health Related Services/Incidental Medical Services: The medications are centrally stored. There is a locked cabinet allocated for medication storage. Centrally stored medication and destruction logs are maintained separately. LPA reviewed medication logs and observed that they were dispensed accurately. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. LPA observed four (4) dual smoke detectors and carbon monoxide detectors throughout the facility. There were two (2) fire extinguishers on site, date charged was 02/08/2024. Pursuant to Title 22 of The California Code of Regulations Division 6, there are zero (0) deficiencies observed. On October 04, 2023, Licensing Program Analyst (LPA), Venus Mixson arrived to the facility unannounced in order to conduct the required annual inspection. LPA Mixson spoke with the Lead Caregiver, Carolina Castro introduced herself, and stated the purpose of the visit. LPA Mixson toured the facility, and inspected the inside and outside of the facility, and there were no obstructions to indoor and outdoor passageways currently at the time of this visit. The facility is a single story home, located at 23276 Spring Meadow Dr, Murrieta, CA. 92562. Physical Plant: The facility phone number is 951-600-2894, and is operable. The LPA observed the resident bedrooms, and they are equipped with required furniture as per Title 22. The LPA inspected the facility bathrooms, and the hot water temperature tested within regulations. Bathrooms were clean and appliances were operating appropriately currently at the time of this visit. The facility is equipped with operating smoke detectors, carbon monoxide alarms, and fire extinguishers. The LPA observed required postings such as; the ombudsman poster, "If you See Something, Say Something" and the "Personal Rights" postings were posted in a common area. The cleaning supplies, sharps, and other dangerous items were kept locked and inaccessible to the residents. There was a designated storage space for resident/staff files. Medications: were reviewed, and were locked and inaccessible to residents. The overall facility is clean, the furniture is in good repair. The facility air conditioning and other appliances were operable currently at the time of this visit. Food Service: Non-perishable and perishable food supply is sufficient per regulations, and there are a variety of food types available for residents. Dishes and utensils were in sufficient supply and stored properly. Care & Supervision: Facility has sufficient staff, and staff were engaging the residents during this visit. Record Review: The LPA reviewed five resident files, one staff file, and the required CCL reports from previous visits. There were no Title 22, Division 6 Regulation violations observed and/or cited during todays visit.
