Type of visit: Required - 1 Year
On 1/29/2026, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA introduced self, stated purpose of visit, and allowed entrance by Direct care staff. Licensee/Administrator Ofelia Nebrida also present at time of LPA arrival. There are currently five (5) residents in care. Three residents present at time of visit, two residents were at day program. Residents observed to be relaxing in their bedroom, and some to be engaged in activities in the living room with staff. Facility tour conducted with Administrator, facility observed to be a comfortable temperature, clean, odor free, and well lit. Facility has 5 private bedrooms, all bedrooms observed to have required furnishings available. Bathrooms toured, showers observed to have shower chairs, grab bars, and skid resistant mats available. Toilet area also observed to have grab bars. Water temperature measured in bathroom at 115 degrees F. Kitchen toured, facility observed to have a 2-day supply of perishable food and a 7-day of non-perishable food available for residents in care. All sharps observed to be locked and secured in kitchen drawer. Medications observed to be locked and secured in kitchen cabinet. Medications observed to have original labels and to be administered as prescribed. Facility and staff files reviewed during inspection, Fire extinguisher present with a service date of 6/23/2025. Carbon monoxide detector and smoke detector observed operational during inspection. Last fire drill conducted on 12/23/25 according to facility records. Outside of facility toured. Facility has sufficient seating and a shaded patio area available for all residents. Emergency exits open free of obstruction. No hazards observed. LPA received a current copy of liability insurance during inspection. No deficiencies observed during inspection. On 5/29/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management. LPA met with Administrator, Ofelia Nebrida to conduct visit. LPA conducted visit to follow up on a self reported incident for R1 that occurred on 05/09/25 where are R1 had an unobserved fall. LPA conducted interview and obtained additional documentation during case management visit. . R1 was observed to be seated in recliner participating in day program activity with staff observing. Type of visit: Required - 1 Year
On 2/12/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection visit. LPA arrived, stated purpose of visit, and allowed entrance by Administrator, Ofelia Nebrida. Currently, 5 residents in care. Four (4) residents were present at the start of today's inspection. Facility tour conducted with Administrator. Facility observed to be well lit, odor free, and comfortable temperature. LPA observed residents engaging in activities with staff during inspection. Facility living room and dining room observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. Resident bathrooms toured, water temperature measured at 113 degrees F. Bathrooms observed to have grab bars in the shower and near toilet. Shower also has shower chair and non-skid mat available. LPA observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. Facility observed to have a 2-day supply of perishable food and a 7-day supply of non-perishable food available. Medications are centrally stored and locked in kitchen cabinet. All medications observed to have original labels and be administered as prescribed. Outside of facility toured. Facility has an outdoor shaded seating area available for residents. Exits open free of obstruction. Fire extinguisher present with a service date of 07/02/2024. Smoke and Carbon Monoxide detectors present and observed operational. Last Fire drill conducted 12/02/24 according to facility records. LPA reviewed resident and staff files, and Disaster Binder. No deficiencies were cited during this inspection. On 2/05/2024, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Required Inspection. LPA introduced self, stated purpose of visit, and allowed entrance by Direct Care Staff. Currently, there are five (5) residents in place. All residents were present during inspection. Residents were observed participating in various activities during facility visit. Facility tour conducted with Administrator. Facility observed to be clean, odor free, and comfortable temperature. Facility tour began in resident rooms, all bedrooms are private. All rooms observed to have all required accommodations. Resident bathrooms observed to have grab bars near toilet. LPA observed showers to also have grab bars, non-skid mats, and shower chair available. Water temperature measured at 115 degrees F. All common areas have adequate seating available for residents. Kitchen toured, LPA observed facility to have adequate food supply for residents in care. Knives observed to be locked, secured, and inaccessible to residents. Cleaning supplies observed to be locked and secured under kitchen sink. Medications observed to be locked and secured in kitchen cabinet. Medications observed to have original labels, and administered as prescribed. Smoke detectors and carbon monoxide detectors present and observed operational during inspection. Facility is equipped with pull station and sprinklers. Fire Extinguisher present with a service date of 7/31/2023. Last fire drill conducted on 12/10/23 according to facility records. Outside area toured. All exits open free of obstruction. No hazards observed. LPA reviewed staff and resident files. LPA received the following during inspection: Administrator certificate, LIC 308, LIC 500, LIC 9020, and certificate of liability insurance.
