Licensing Program Analyst (LPA) Alviso conducted a Required- 1 Year visit, on 07/28/26 at approximately 1:40pm, and met with Administrator Emma Mutunga. LPA observed a caregiver on duty, Faith Jepchumba. All four clients were at day program services. Fire clearance is approved for four (4) non-ambulatory, and two (2) ambulatory residents. Bedrooms #1 and #3 are each approved for two (2) non-ambulatory residents. Licensee will be submitting a floor plan sketch with some revisions to two (2) bedrooms, one (1) of them being a resident room. Once LPA receives the revised sketch, the LPA will request a new fire clearance inspection. Once an approval is received from the local fire department, the LPA will notify the Licensee. Administrator agreed to submit the revised sketch with cover letter of what is being requested, by 7/30/2026. Facility has an approved dementia plan of operation. There is an approved hospice waiver for two (2) residents. Facility has a required infection control plan, and a required emergency and disaster plan. Facility has current required liability insurance; Licensee will submit copy with updated forms requested. The facility does have emergency food, water, and supplies to meet the "72 hour shelter in place" requirements. Emergency disaster drills are being conducted as required. All required rooms have a smoke alarms working properly, including carbon monoxide detector. Fire extinguisher was serviced and tagged as required. All exits were free and clear from obstruction. LPA reviewed four (4) resident files. All medication records were complete and medications were stored and locked up as required. Continued on LIC809C... LPA reviewed four (4) staff files. All staff had required first aid certification and CPR certification. All staff had required criminal record clearance. LPA reviewed staff required training, including medication training for those staff that assist residents' with medications. The LPA toured the home with the Administrator. The exits were all free and clear of obstructions. The fire extinguishers were serviced and tagged as required. All medications were locked up and inaccessible to residents in care. All cleaners/disinfectants were locked up and inaccessible to residents in care. The facility had a sufficient supply of food. Sufficient supply of hygiene products, linens, soaps/cleaners/disinfectants, furnishings, paper products, and personal protective equipment (PPE) supplies. All bathrooms had non-slip mats/non-slip flooring for resident use. All bathrooms had grab bars for residents use. All common areas, resident rooms, hallways, and bathrooms had sufficient lighting for resident's use. Hot water was measured at 116.7 degrees Fahrenheit in resident bathroom. Licensing Program Analyst (LPA) Alviso conducted a pre-licensing inspection at approximately 9:45am on 5/13/25, and met with Applicant Emma Sila Mutunga. Emma Sila will be the facility's, Sila Home Care, Administrator. This application is a change of ownership, and is currently licensed as "Alvarez Family Home" #496800549. Emma Sila Mutunga is the current Administrator for Alvarez Family Home. Applicant will be submitting a hospice care waiver request to the application packet being processed by the application unit. Applicant has an approved fire clearance for four (4) non-ambulatory residents, in room #1 & #3, and two (2) ambulatory residents in room #2, effective 10/19/2024. LPA observed the front door foyer area to be clear of most of the boxes; The Administrator has agreed to ensure boxes remaining stay stacked out of the walking pathway, boxes stacked at waist height only for safety, and to ensure the front double doors are never blocked from opening fully. All items that were gathered and stored in the middle of the garage space were removed; The garage has a clear pathway to exit as needed. LPA observed new fencing that was installed on the side of the front yard in front of the house. The LPA will forward the pre-licensing report, LIC809, to the assigned Application Analyst in the Department's application unit. Licensing Program Analyst (LPA) Alviso conducted a pre-licensing inspection at approximately 9:45am on 5/13/25, and met with Applicant Emma Sila Mutunga. Emma Sila will be the facility's, Sila Home Care, Administrator. This application is a change of ownership, and is currently licensed as "Alvarez Family Home" #496800549. Emma Sila Mutunga is the current Administrator for Alvarez Family Home. Applicant will be submitting a hospice care waiver request to the application packet being processed. Applicant has submitted a dementia plan with their application. Applicant has submitted a required emergency disaster plan, and a required infection control plan. Applicant has an approved fire clearance for four (4) non-ambulatory residents, in room #1 & #3, and two (2) ambulatory residents in room #2, effective 10/19/2024. There is a carbon monoxide detector in the facility that was working properly during the inspection. All required rooms have a smoke alarms as required by the local Fire Department. Fire extinguisher were serviced and tagged as required. All exits were free and clear from obstruction. All exits had auditory alarms. The LPA toured the facility with applicant. Bathrooms had grab bars and non-slip mats for the showers for resident use as needed. Hot water was measured at 115.5 degrees Fahrenheit. All resident rooms were clean, orderly, and had all required accommodations. Facility had sufficient lighting in resident rooms, bathrooms, and common areas. Medications were centrally stored in a locked storage cabinet. Continued on LIC809C... All disinfectants/cleaners/soaps were locked up and inaccessible. Facility had a supply of food, perishable and non-perishable. Facility had a sufficient supply of linens, dishes, and furnishings for resident use. The fire exit ramps were free and clear of obstruction, one is off of resident room #3 in the backyard area, and the other one is in the front of the house, from the exit door of the facility's laundry room. All outside walkways/pathways were free and clear of obstruction as required. Component III orientation was held with Applicant Emma Sila Mutunga, on 5/13/25. The following items need to be corrected/completed: Facility has a large amount of boxes in the front door foyer area, which are full of incontinent supplies. The boxes cover one whole side of the entry's double front door, and continues along the side of the wall. The applicant stated the boxes are being moved to storage in the garage that will be available soon. LPA obtained photos. Facility had a large portion of the garage full of open boxes with tools/items, furniture items, and miscellaneous items; Applicant stated they are needing to be taken and/or picked up for donation or to the dumps. Some paint cans, caulking, paint brushes, and some miscellaneous items were put in a locked storage cabinet by the applicant. LPA obtained photos. LPA observed in the front of the house, the front yards side fencing had a large portion missing due to the fence falling down and/or breaking down due to age of the fence. There is a bit of a drop from the fence line, where the ground is uneven and goes down,which may be a trip hazard to residents in care. Applicant is installing a barrier/fencing/gates to ensure the front side area is not a health and safety hazard/risk to residents in care. LPA obtained photos. Applicant is dusting, and cleaning all windows, window screens, and glass doors.
