On July 22 2026 Licensing Program Analyst (LPA) Sarah Benson arrived at the facility to conduct a case management concerning staff without criminal background clearance. Licensing Program Analyst (LPA) Sarah Benson conducted an audit of criminal record clearance for S1 and facility roster on Guardian. Based on LPA's file review and Guardian, S1 does not have a criminal background clearance and is associated to the facility. LPA toured the facility reviewed staff rosters and schedules to confirm all staff have criminal background clearances. When asked how long has S1 worked at the facility the administrator reported S1 has not worked at this facility. The administrator stated S1 worked at our other facility in February for a few days believing the guardian site reported S1 as eligible. When staff didn't receive the clearance in the mail we called licensing to discover S1 was not cleared. As soon as it was discovered he was not background cleared we removed him from employment. Type of visit: Case Management - Incident

On May 20 2026 Licensing Program Analyst (LPA) Sarah Benson arrived unannounced to conduct a case management visit. During today's inspection LPA met with Administrator Krystle Love. LPA arrived to obtain further information regarding incident reports submitted on 4-6-26 and 5-5-26. During today's inspection LPA interviewed Administrator and requested resident documents. In addition, LPA interviewed 1 resident in care. No deficiencies were cited during this visit. Department will follow up further if warranted. Exit interview conducted. Type of visit: Required - 1 Year

On May 8 2026 at 11:54pm Licensing Program Analyst (LPA) Sarah Benson arrived at the facility unannounced to conduct a Required-1 Year inspection. LPA met with administrator Betty Cain (cert #7004026740 exp.03-02-28) and explained the purpose of the visit. Administrator certificate is current. LPA Benson and administrator toured the facility together to ensure the health and safety of residents in care. Areas toured include but are not limited to fifteen (15) resident rooms, common areas, nineteen (19) bathrooms, kitchen, storage areas and back yard. Staff and resident files were reviewed. Medications were also reviewed. Medication is locked in a locked closet. The common area was clean, odor-free and in good repair. All bedrooms had required furniture, bedding, lighting and windows with screens. The bathrooms were clean and in good repair. The kitchen was clean and in good repair. Cooking/dining equipment and utensils were present. Food appears to be stored and prepared properly. Facility has required (7) seven-day non-perishable and (2) day perishable supply of food. The facility was observed to be at a comfortable temperature. First aid kit fully stocked and ready for emergency use. Fire extinguisher fully charged. Smoke detectors are all operational. Hot water temperature measured within required Title 22 regulations of 105 degrees F and 120 degrees F. All employees requiring background checks are cleared. There is a schedule of activities planned for the clients. All required postings are displayed within the facility. No pools/bodies of water are on the premises. No firearms are on premises. The last disaster drill was conducted and documented on 04-20-26, the facility has been conducting drills every 3 months. LPA interviewed three residents and two staff. The facility is in compliance. Type of visit: Case Management - Incident

On February 19 2026, Licensing Program Analyst (LPA) Sarah Benson arrived at the facility unannounced to conduct a case management visit. LPA Benson met with House Manager Hannah Hunt who phoned Administrative Director Crystal Love and Licensees and explained the reason for the visit. LPA Benson and staff toured the facility together to ensure the health and safety of residents in care. The department is following up about an Incident Report (IR) sent by facility on 02-12-26 stating that a resident had a fall with an injury. LPA Benson conducted interviews with Licensee, staff, the resident and performed a file review. (see attached 812 for interview). At this time, this case is under review and the department will do follow up as needed. No citations were issued per Title 22 Regulations. LPA met with Administrator Krystle Love acting for Administrator Betty Cain(cert #7004026740 exp.3-2-26) and explained the purpose of the visit. Administrator certificate is current. LPA Benson and administrator toured the facility together to ensure the health and safety of residents in care. Areas toured include but are not limited to fifteen (15) resident rooms, common areas, nineteen (19) bathrooms, kitchen, storage areas and yard. In the areas toured no immediate health, safety, or personal rights violations were observed. Staff and resident files were reviewed. Medications were also reviewed. Type of visit: Case Management - Health Checks

On 7-16-24 at 10:00AM Licensing Program Analyst LPA Sarah Benson arrived at the facility to perform a health and safety check of residents in care regarding flooding of the facility. LPA Benson met with Licensee Betty Cain. Licensee Betty Cain and LPA Benson toured the facility together. LPA Benson observed residents throughout the front of the facility relaxing in recliners and performing daily activities. Administrator reported contacting all residents responsible persons. Licensee stated six resident rooms were affected by the water. Licensee reported the affected resident room floors have been tested with a moisture meter and are drying out nicely. Licensee reports the residents have access to their rooms at this time. Licensee reports that one resident was moved to another room after consulting with the residents family. The licensee states the work that needs to be done in the resident rooms can be done during the day while the residents are busy in the front half of the facility with daily activities. The licensee states the facility is waiting for the insurance company claim before the final inspection will be performed. LPA met with Jennifer Gama Resident Care Director acting for administrator (cert #6014210740 exp.03-02-2026) and explained the purpose of the visit. Administrator certificate is current. LPA Benson and resident care director toured the facility together to ensure the health and safety of residents in care. Areas toured include but are not limited to four (4) resident rooms, common areas, two (2) bathrooms, kitchen, storage areas and back yard.
