Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with staff, Sonny Tan and Melita Cabarles, and explained the reason for the visit. At approximately 8:39am, with the assistance of staff, LPA took a tour of the physical plant. The facility is a two story building. The first floor only, carries the license for the Residential Care Facility for the Elderly (RCFE). The second floor, is living quarters for staff and family only. No resident access. Required postings were observed in the open by the kitchen area. The smoke alarms and carbon monoxide are hardwired and interconnected. The fire extinguisher is located in the dining room. It was charged on December 27, 2025. Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives are stored and inaccessible to the residents. Cleaning supplies are kept in the garage. Bedrooms: There are four (4) bedrooms, three (3) designated for residents' use, and one (1) for staff. Bedrooms, in use by the residents are shared. Bedrooms were observed to be properly furnished with appropriate beddings and linens with sufficient lighting. Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures, grab bars and non-skid mats. Hot water temperature was measured between 115 and 117 degrees Fahrenheit. No cleaning supplies were observed in either bathrooms during the day of the inspection. Common Areas: Common areas include the living room and dining room. The living room is furnished with sufficient seating of couches, chairs, tables and television. The fireplace is properly screened with a chain screen and glass door. The dining room table is large enough to seat up to six (6) individuals. Furniture in common areas are observed to be in good repair. Floors were mopped and clean. Entry/exits, hallways and passageways were clear. Surrounding Grounds: The backyard of the facility has a shaded area, furnished with chairs and a table. There is sufficient space in the backyard for outdoor activities. There is no swimming pool or any other bodies of water. Laundry area: The laundry area is located in the garage. Resident Files: Resident files are kept in the medication closet. LPA conducted a file review of resident records to insure compliance of licensing forms. Staff Files: Staff files also kept in the medication closet. LPA also conducted a file review of staff records to insure forms and training are up to date and compliance with licensing forms. Medications: Medications are stored in a locked closet by the living room. Closet is kept locked and requires a combination code to gain access. Medication and Medication Records were reviewed for proper storage and documentation. Garage: The garage is attached to the house. Access is through staff room, which is inaccessible to the residents. Access to the second floor of the building is only through the garage. Pursuant to Title 22 Division 6 of the CA Code of Regulations, there were no deficiencies observed during the visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Abeye Duguma met with caregiver, Melita Cabarles, for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at 9:30 AM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of four (04) bedrooms, of which one (01) is for staff and two (02) bathrooms for both residents and staff. The facility is fire cleared for six (06) non-ambulatory residents and a hospice waiver for three (03). The facility is currently occupying three (03) non-ambulatory residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. The garage is currently being used for storage. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (2) days perishable and seven (7) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. (continued on LIC 809-C) The living and dining room are neat and clean. The facility maintains a comfortable temperature at 75°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located near the kitchen, observed to be full and last inspected on 12/16/2024. The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 110.3°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication and first aid kit to be locked and inaccessible to residents. No health and safety hazards noted during the visit. Exit interview conducted. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication and first aid kit to be locked and inaccessible to residents.
