Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today at 09:10am. Upon arrival, the LPA was greeted by staff who then contacted the Administrator and informed them of the visit. Administrator Richard Soriano arrived at approximately 09:25am and the reason for the visit was explained. Entrance interview conducted. Beginning at 09:28am, the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed: Kitchen: The LPA inspected the kitchen/food service area at approximately 09:32am. Knives and sharps were observed in a kitchen drawer locked and inaccessible at the time of the visit. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. Cleaning supplies were observed under the kitchen sink inaccessible to residents at the time of the visit. At 09:35am, the kitchen sink was checked for hot water temperature, and it measured at 105.4 degrees Fahrenheit. Common Areas: This includes the living room and dining room. Furniture in the common areas was observed to be in good condition at the time of the visit. The facility maintained a comfortable temperature. Required postings were observed throughout the common space. Fire extinguisher was observed to be new with a purchase date of 01/26/2026. A complete First Aid Kit and Manual were observed at the time of the visit. No hazards/obstructions observed inside or out. Report Continued on LIC 809C... Report Continued from LIC 809... Garage: The garage is detached from the facility. The LPA observed an adequate amount of emergency food and water. Laundry: Washer and dryer were observed by the main hallway. Detergents and toxins were observed locked at the time of the visit. Restrooms: There is one (1) restroom for residents’ use. Bathroom was clean and sanitary and in operating condition with grab bars and non-skid surfaces. The bathroom was sufficiently stocked with supplies and paper towels. The hot water temperature was measured within the required range of 105 – 120 degrees Fahrenheit at the time of the visit. Bedrooms: There are three (3) bedrooms for residents’ use. All bedrooms are designated as shared / double occupancy. Bedrooms were observed to be furnished appropriately with appropriate furnishings, and sufficient lighting. Additional clean linens and towels were observed by the main hallway. Staff bedroom observed on premises inaccessible to residents at the time of the visit. Backyard/Front Yard: There are two (2) sheds used for storage purposes in the backyard. Sheds were observed locked and inaccessible to residents at the time of the visit. The front yard has a patio umbrella with patio furniture for residents use. There is one (1) side gate with latching mechanisms. All passageways were observed to be clear of any obstructions. No bodies of water noted at the time of the visit. Records: Record review began at approximately 09:55am Two (2) resident files were reviewed for, but not limited to, the following: signed admission agreements, current medical assessments with TB results, preplacement appraisal, consent for treatment form, and updated/current appraisal & needs and services plan/IPP. Four (4) personnel files including the current Administrator’s file were reviewed for, but not limited to: personnel records, health assessments with negative TB test results, criminal record clearances, first aid/CPR training, and the appropriate yearly training. Although the facility had a designated training binder, the LPA was unable to determine the number of hours completed per regulation for the past 12 months. The Administrator’s Certificate is valid until 03/17/2028. Facility is vendored through the South Central Los Angeles Regional Center (SCLARC) as Level 4E. Emergency Disaster Planning: During today’s visit, the LPA reviewed the facility's emergency disaster plan, which was observed to be complete and recently reviewed/updated. Emergency disaster drills are being conducted quarterly as per regulation. Medications: Medications review began at approximately 12:05pm. Medications are centrally stored and kept in a locked cabinet by the kitchen. All medications including PRNs were labeled, stored, and locked inaccessible to residents in care. Medications appear to be administered as prescribed at the time of the visit. No citations issued at this time. Exit interview conducted. On 05/19/25 Licensing Program Analyst (LPA) Yolanda Rosser conducted an unannounced annual inspection visit to the above facility using the CARE tool. LPA was met by Francis Soriano, Administrator, and the purpose of today’s visit was explained. The facility is licensed to serve six (6) residents aged 60 and above. All three client bedrooms are fire cleared for six (6) non-ambulatory residents. No hospice waiver. No dementia plan of operation. Census is 2 and fees are current. The facility is a single-story home located in a residential neighborhood. It consists of three (3) resident bedrooms, of which only two (2) are being used by residents, and one (1) staff bedroom, two (2) bathrooms, living room, dining room, kitchen, laundry area, and a detached garage used for storage. LPA Rosser and Milagros Soriano toured the physical plant. There were no bodies of water or obstructions on the premises. All resident rooms were inspected. Bedrooms had the required furniture and in good condition, bed linens and closet/drawer space to accommodate each client comfortably. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were clean and operational and found to be within Title 22 regulations. The water temperature properly measured between 107.0 F and 120.0 F. LPA observed the facility to be clean and appropriately furnished with clear passageways. A comfortable temperature was maintained in the facility. The kitchen was inspected and there is sufficient perishable and non-perishable food supply and maintained adequately. Sharps, toxins, cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. A landline was observed. Continued on LIC 809C Fire extinguishers were fully charged. A stocked First Aid kit along with manual was available. There was extra supplies in the garage, bed linens, sheets, cleaning supplies, toiletries. LPA observed a second freezer with an abundance of perishable foods. During this inspection LPA did not observe any deficiencies, therefore no citations were issued at this time. On 5/20/24 Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual inspection visit to the above facility using the CARE tool. LPA was met by Milagros Soriano, Administrator, and the purpose of today’s visit was explained. The facility is licensed to serve six (6) residents aged 60 and above. All three client bedrooms are fire cleared for six (6) non-ambulatory residents. No hospice waiver. No dementia plan of operation. A comfortable temperature was maintained in the facility. The kitchen was inspected and there is sufficient perishable and non-perishable food supply and maintained adequately. Sharps, toxins, cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. A landline was observed. Exits/ Walkways around the facility were free of debris and hazards. Smoke detectors and carbon monoxide were tested and are working properly. Fire extinguishers were fully charged. A stocked First Aid kit along with manual was available. Continued on LIC809-C During the visit, LPA observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents. There are sanitizing stations in common areas and restrooms. All mandated inspection control posters were posted. During this inspection LPA did not observe any deficiencies, therefore no citations were issued at this time.
