Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with facility Administrator Gurmit, Aulakh Continual Administrator's Certification expires 02/26/2026. There are currently 5 residents who reside at this home and there is 2 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA reviewed a sample of staff files, a sample of resident files, Emergency Disaster Plan, and Infection Control Plan. LPA observed hospice care plan for resident 1 is not accurate. LPA observed dust throughout facility in light fixtures, and vents. The facility does not have Plan of Operation available for review. Resident 2's Needs and Services Plan is not updated annually as required. The facilities Infection control plan does not have all required information. The facility does not have required liability insurance available for review. The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. On 12/17/24 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Evaliza Romero. LPA was granted entry. 5 residents were present during inspection. Administrator Gurmit Aulakh (A1) was called and arrived shortly after LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -7 degrees F and refrigerator temperature was maintained at 29 degrees F. Fire extinguisher was observed with a purchase date of: 2/19/24. Fire drill last completed on 10/6/24. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 106.8 in bathroom 1, 107.7 in bathroom 2 and 108.1 degrees F. in bathroom 3. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. Samples of medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. All residents and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. The following documents requested to be updated and submitted to Fresno CCL by 12/31/24: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. An unannounced Annual visit was conducted on the date & time indicated above by Licensing Program Analyst (LPA) K. Mcclurg. LPA met with Administrator (Admin) Gurmit Aulakh. Physical plant toured. Living, dining & bedrooms sufficiently furnished with adequate lighting. Resident bathrooms observed to appear clean with no unpleasant odors. Hot water measured @ 117.5 degrees F. Fire extinguisher service date: 2/13/2023. Carbon monoxide & smoke detectors operational. Auditory alarms operational & on exits. Knives & stove knobs inaccessible to residents.
