Type of visit: Post Licensing

On 12/08/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced for a Post Licensing Inspection. LPA was greeted and granted entry by Licensee/ Administrator Gurbinder “GB” Sodhi. LPA introduced self and stated the purpose of the visit. All one resident were present during inspection observed sitting in the living room. LPA toured facility with Licensee. Facility observed with video surveillance cameras with no audio outside the facility and in common areas. All resident and staff files observed with required records. A fire extinguisher was observed with purchase date 06/30/25. LPA toured the facility with Licensee. Facility was free from ground obstructions and odor free. Common areas were observed to have adequate seating and lighting available. Kitchen was toured. Knives and medication were observed locked in kitchen shelf. Adequate nonperishable and perishable food observed. Cleaning supplies and chemicals were locked under kitchen sink. LPA observed bed linens and towels in hall closet. Bedrooms were observed to have the required furnishing. Bathroom hot water temperature tested at 105.4 degrees F and at 106.3 and 108 degrees F in master bathroom. Outside of facility toured. Exits were open and free of obstructions and debris. Adequate outdoor seating was observed to be available for residents. Smoke detectors and carbon monoxide were observed to be operational during this inspection. No deficiencies issued during this inspection. Exit Interview conducted. Type of visit: Prelicensing

On10/09/25, Licensing Program Analyst (LPA) Yang arrived to the facility announced to conduct the Pre licensing visit. LPA Yang met with Licensee Gurbinder “GB” Sodhi and Licensee’s brother in law Shamsher Sangha who granted LPA entry into the facility. The facility is a 4 bedroom, and 3 bathroom home and fire clearance were granted for 3 non-ambulatory and 3 Ambulatory for a total capacity of 6. Facility observed with video surveillance cameras with no audio outside the facility and in common areas. A fire extinguisher was observed with purchase date 06/30/25. There are no residents present during this inspection. LPA toured the facility with Licensee. Facility was free from ground obstructions and odor free. Common areas were observed to have adequate seating and lighting available. Kitchen was toured and observed to have dishes, plates, and utensils. Knives and medication will kept locked in kitchen shelf. Refrigerator temperature maintained at 40 degrees F and freezer temperature maintained at 0 degrees F. Perishable and nonperishable food will be purchased upon first resident admission. Cleaning supplies and chemicals were locked under kitchen sink and garage cabinet. LPA observed an extra supply of bed linens and towels in hall closet. Bedrooms were observed to have the required furnishing and are ready for occupancy. Bathroom hot water temperature tested at 110.4 degrees F, range between 110.9 and 111.2 degrees F in master bathroom. Outside of facility toured. Exits were open and free of obstructions and debris. Adequate outdoor seatings available for residents. First aid kit was observed and contained all required items. Smoke detectors and carbon monoxide were observed to be operational during this inspection. Facility phone number will be 661-827-6430. Component III was conducted during pre-licensing visit with Applicants. I have found that applicant has met all pre licensing requirements. COMP II by CAB successfully completed Method: Phone Call at CAB Applicant/administrator participated in COMP II at CAB telephone call with analyst at CAB. Identification of the applicant and administrator was verified by presenting photo ID via phone. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1. Facility operation: License type, client/resident populations, and program 2. Staff qualifications and responsibilities 3. Applicant and Administrator qualifications 4. Program policy: Abuse, admission agreement, medication management, reporting incidents to CCL, restricted & prohibited conditions 5. Grievances, Complaints, Community resources 6.
