At 9:00am, on 6/23/2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Lead Staff Mylene Cabello, announced who he was and the reason for the visit. At 9:05am Lead Staff and LPA conducted a full tour of the facility. This facility is a single story residential home with five resident bedrooms (one is dual occupancy), three and a half bathrooms (one is en-suite to the dual occupancy resident room, two are for public use, and the half bathroom is for staff and located off the laundry room). There is a living room, dining room, and kitchen. A staff bedroom is located off the dining room. Access to the laundry room and garage is through a locked door for resident safety. LPA noted that the backyard has seating and shade for residents and visitors. LPA noted fresh fruit in the kitchen for residents to enjoy freely. The facility has battery operated smoke detectors in each bedroom and hallway leading to bedrooms that are all working, the carbon monoxide detector is in the hallway and functioning normally. LPA observed a fire extinguisher near the kitchen that was tagged current and in the green compression range, purchased on 2/28/2026. LPA tested facility hot water at 111.4°F, within regulation temperatures 105-120°F. LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility has no obstructions in hallways, doorways or exits. Medications, staff and resident files are locked in a cabinet in the dining room. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records. LPA conducted a staff and resident file review. LPA and Lead Staff conducted a review of the annual care tool modules. There were no deficiencies cited at this time. LPA met with Lead Staff Mylene Cavello (S1), LPA and S1 called administrator Maynard Marcos by phone who provided permission for S1 to sign for inspection visit. LPA noted that S1 has a LIC308, Designation of Facility Responsibility for this facility on file. S1 and LPA conducted a full tour of the facility. This facility is a single-story residential home with 5 resident bedrooms (one is dual occupancy), three and a half bathrooms (one is on-suite and two are for shared/public use). A staff bedroom is located off the dining room and the half bath located near the laundry room is for staff use. There is a living room, dining room, and kitchen. Access to the laundry room and garage is through a locked door for resident safety. LPA noted that the backyard has seating and shade for residents and visitors. LPA noted fresh fruit just outside the kitchen for residents to enjoy freely. Near the dining room there is a piano for residents, visitors, and staff to play. During visit LPA observed residents engaged in coloring and brain stimulating activities. LPA also observed multiple Home Health and Hospice staff visiting residents. The facility has battery operated smoke detectors in each room that are all working, the carbon monoxide detector is in the hallway and functioning normally. LPA observed a fire extinguisher near the kitchen that was in the green compression range, purchased on 03/10/2025. LPA tested facility hot water at 107.3*(f), within regulation temperatures 105*-120* (f). LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility has no obstructions in hallways, doorways or exits. (Continued on LIC809-C) Medications are stored in a locked cabinet. Staff and client files are locked in a cabinet in the dining room. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records, finding no violations. LPA conducted a staff and resident file review. LPA and S1 conducted a review of the annual care tool modules. There were no deficiencies cited at this time. At 9:30am on 07/12//2024, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the annual facility inspection. LPA met with care giver Mylene Cavello (S1), who called administrator Maynard Marco by phone who provided permission for S1 to sign for inspection visit. LPA noted that S1 has a LIC308, Designation of Facility Responsibility for this facility on file with CCLD. S1 and LPA conducted a physical tour of the facility. LPA noted that all exits were free and clear of hazards. LPA noted that all rooms have a working combination carbon monoxide/smoke detector unit that are all hardwired and currently working. LPA noted that all rooms are properly finished with correct bedding linin, personal storage and lighting in accordance with regulations. LPA noted that there is ample linin and personal hygiene and incontinence supplies for all 5 residents presently at the facility. This facility has 5 resident bedrooms, 4 are single occupancy and 1 in double occupancy. The facility has 3 bathrooms all meeting regulations standards. Water in the facility was tested at bathroom 1 and bathroom 3 and found to be within the regulation temperature range of 105*-120* (f). LPA observed there to be more than 2 days of perishable foods and more than 7 days of non-perishable foods for the 6 residents and staff. LPA observe the outside grounds of the facility and noted that patio tables have table umbrellas for shade when residents are utilizing outdoor area of facility. LPA noted that the facility to be clean and in good repair with no hazards present on facility physical tour. LPA found no technical, violations or citations during the physical tour at this time. S1 and LPA conducted a full review of the control tools modules. It was discovered that annual staff training was not completed per regulations for the past 12 month period and a citation was issued for staff annual training (Health and Safety Code 1569.625(b)(2)); It was also discovered that there had been no emergency drills conducted over the past 12 months and no documentation of drills (Health and Safety Code 1569.695(c)). LPA issued two citations, with plan of corrections for two violations. LPA concluded the annual facility inspection.
