At 9:20am, on 8/26/2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Licensee/Administrator Arlene Ramos Losbog, announced who he was and the reason for the visit. Licensee and LPA conducted a full tour of the facility. This facility is a single story residential home with four (4) resident bedrooms (two are dual occupancy) and there are two full bathrooms (one is en-suite to resident room #4 and one is for public/shared use). There is a living room, a kitchen with dining space and office area. Access to the laundry room and garage is through locked doors for resident safety. LPA noted that the backyard has seating and shade for residents and visitors. LPA noted fresh fruit and snacks in the kitchen for residents to enjoy freely. The facility has battery operated smoke detectors in each room that are all working, the carbon monoxide detector is by the kitchen and functioning normally. LPA observed fire extinguishers throughout the facility that were in the green compression range. LPA tested facility hot water at 116.4°F, within regulation temperatures 105-120°F. The forecast temperature today in Paso Robles is 101°F and the facility temperature per the thermostat in the hallway is 78°F, within regulation and residents state they are comfortable. LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility has no obstructions in hallways, doorways or exits and the front and backyard lawns appear recently trimmed. Medications are locked in a cabinet in the office area. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records. LPA conducted a staff and resident file review. LPA and Licensee conducted a review of the annual care tool modules. There were no deficiencies cited at this time. Allegation investigated: Staff do not ensure the facility is properly maintained.

At 9:50am, on 5/5/2026, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to investigate the allegation of this complaint. LPA met with Licensee/Administrator Arlene Ramos Losbog, announced who he was and the reason for the visit. On the allegation, staff do not ensure the facility is properly maintained; it is alleged that the exterior grounds of the facility are in a state of serious and ongoing neglect, specifically regarding weeds that have grown to knee height across the property. It is alleged that the current condition of the exterior grounds is not an isolated occurrence but reflects an ongoing lack of property maintenance. (Continued on LIC9099-C) Unsubstantiated Upon arriving to the facility today, LPA observed that the lawn in the front yard appeared to be recently manicured with a couple of small areas of thinly scattered tall growth. Walking paths remain accessible, and the overgrowth does not impede resident movement, attract pests, or create unsafe conditions. Along the east side of the property LPA noted a landscaped area with planted trees, bushes, and bark ground covering, with some weeds scattered throughout. At the corner of the front of the property near the fire hydrant LPA noted short patches of plant growth possibly not intentionally planted and some drought resident plants placed by the facility. The bushes on the property appeared to have been shaped previously, though they showed signs of recent growth. Staff interviews revealed that staff trimmed the front yard lawn yesterday and approximately two weeks prior was the last time it was trimmed. Staff stated it rained recently causing the weeds to grow more quickly. Review of local rainfall records indicated that the area received rain approximately two - three weeks prior to today’s inspection. Staff use a battery operated device to provide the maintenance and state the battery runs low quickly. They are looking into purchasing additional batteries or other devices. Resident interviews reveled that the residents feel safe at this facility and have never observed or encountered obstacles blocking interior or exterior pathways or exits. LPA and Licensee toured the inside and outside of the entire facility. The backyard landscaping was found to be partially maintained; bushes trimmed, but some weeds scattered throughout. None of the landscaping was blocking passageways or cause unsafe conditions. The backyard has space and shade for residents to enjoy with no obstructions. Additionally, the facility’s interior and the remainder of the exterior were observed to be in good overall condition. Based on these observations, the condition of the front yard does not appear to reflect the general maintenance and cleaning practices of the facility as a whole. LPA observed the interior to be clean and organized. A review of the facilities submitted Plan of Operation revealed no violation. LPA provided technical assistance, reminding the Licensee of the potential hazards that overgrown landscaping can have on resident and staff safety. As well as, how the maintenance of landscaping can impact the overall reputation of the facility. At 09:30am, on 08/06/2025, Licensing Program Analyst (LPA) Haner-Tomasko arrived at the facility unannounced to conduct the annual facility inspection. LPA met with Licensee/Administrator Arlene Losbog, announced who he was and the reason for the visit. Licensee and LPA conducted a full tour of the facility. This facility is a single story residential home with 4 resident bedrooms (two are dual occupancy and two are single occupancy), two full bathrooms (one is on-suite to the primary bedroom and one is for public/shared use). As of this visit LPA noted the primary bedroom is being used by staff. There is a living room, a kitchen, dining area and office area. LPA noted a piano in the living room for residents, staff or visitors to play. Access to the laundry room and garage is through a locked door for resident safety. LPA noted that the backyard has seating and shade for residents and visitors. LPA noted fresh fruit and snacks in the kitchen and dining area for residents to enjoy freely. The facility has wired smoke detectors in each room that are all working, the carbon monoxide detector is located high up on a wall in the hallway out of reach to test. Licensee stated they would purchase another carbon monoxide detector to place in an outlet so they can test it regularly. LPA observed a fire extinguisher near the kitchen and another in the hallway that was tagged current and in the green compression range, purchased on 7/29/2025. LPA observed at least 2-days of perishable and at least 7-days of nonperishable foods. LPA noted that the facility is clean with no obstructions in hallways, doorways or exits. Medications, staff and client files are locked in a cabinet in the office area. LPA conducted a sample medication audit and reviewed the facilities Centrally Stored Medication Records, finding no violations. LPA conducted a staff and resident file review. Type of visit: Case Management - Incident

At 8:00am on 12/30/2024, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct a case management visit related to elopement of Resident 1 (R1) on 12/24/2024. LPA met with Administrator Licensee, Arlene Ramos Lasbog announced who he is and the reason for the visit. Administrator Licensee, Arlene Ramos Lasbog and LPA conducted a advisory review of the incident that took place on 12/24/2024 of Resident R1. Administrator Licensee, and LPA reviewed facilities pre admissions appraisal process. Administrator Licensee, Arlene, and LPA reviewed staff training records and staff schedule. LPA noted that he observed 10 hours of dementia specific training and more than 20 hours of annual training for Staff 1 (S1) . Administrator Licensee, and LPA conducted a verbal advisement on resident acuity and staffing. Administrator, agreed to write a policy for pre admissions appraisal for residents, that includes staff education on new residents. Administrator also agreed to develop and implement a charting system for residents that includes daily changes in behaviors and Activities of Daily Living (ADL) needs. Administer will email a copy of pre admissions policy to LPA Jeffries by January 13th, 2025. LPA will forward general courtesy information on reporting, staff and resident files, and inspection resource material which includes link to Community Care Licensing web site. LPA advised Administrator on conducting a thorough pre admissions appraisal on all new residents with concern of resident acuity levels. Type of visit: Prelicensing

At 11:18am on 07/22/2024, Licensing Program Analyst (LPA) Jeffries arrived at the scheduled time to conduct the facility pre licensing inspection. LPA met with Licensee Arlene Losbog and Care Giver Eric Bunte (S1). LPA announced who he is and the reason for the visit. This facility is a 4 bedroom, 2 bathroom, with kitchen, dining room, living room, laundry room, and 3 car garage. There is a large back yard with seating and covered patio for shade. Bedrooms 1-3 are designated for double resident occupancy, and residents share bathroom 1. Bedroom 4 is designated for staff use and has an on suite bathroom. Medications, first aide kit and staff and resident files are located in a locked cabinet in the dining room area. LPA noted that all chemicals and hazardous materials are located in the garage and laundry room behind locked doors. LPA noted that there is a refrigerator in the garage as well as the kitchen that are operating normally. LPA noted that all appliances in kitchen and bathroom are operating normally. All tested for function by LPA at time of facility physical tour. LPA noted that the facility bathrooms are stocked and meet regulation requirements regarding infection control standard practices. LPA noted that the facility has ample linin for 6 resident and staff. LPA noted that there is a large supply of Personal Protective Equipment (PPE) stored in the garage and incontinence supply in the hallway closet. LPA tested facilities smoke detector and carbon monoxide detectors to be functioning properly. LPA observed two fire extinguishers to be primed in the green range indicating primed to function. LPA noted that the facility was clean and in good repair. LPA noted that facility required posting were posted in the facility hallway and the facility has been instructed to contact the local Ombudsmen's Office to obtain Ombudsmen's poster. Licensee, S1 and LPA conducted the Component III orientation and a full review of the Pre Licensing Care Tools module. LPA noted a screen needed to be repaired on the front living room window. LPA noted that the Emergency Disaster Plan and Infection Control plan both meet regulation requirements. LPA noted that dementia care is address as a separate packet to the Plan of Operations. LPA noted that all Pre Licensing documentation has been reviewed. Licensee agreed to have screen fixed by 07/23/2024 and send proof to LPA by end of day 07/23/2024. LPA noted that the facilities could not present proof of current liability insurance Licensee with forward the liability policy when they have possession of the liability insurance contract.
