Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to OC Villas- Linda Vista. The purpose of today’s visit was to conduct the Annual Required inspection. LPA was allowed entry into the facility and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents. Facility has an approved hospice waiver for two residents and there are two residents on hospice care today. Administrator Elsa Roman has an administrator certificate valid until 06/23/2028. Upon entry, facility appears clean, safe and sanitary. LPA toured the facility along with Caregiver Magracia at 10:35 AM. LPA toured the physical plant, checked food service, first aid kit and reviewed records. Facility is a one story home with 6 bedrooms, 2 restrooms, living room, family room, dining room, kitchen, and an attached 2 car garage. The garage is kept locked and used for storage. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. LPA observed two residents with half bed rails and two residents with full rails. Resident restrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 105.1 and 105.8 degrees F in all facility restrooms. Resident toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. LPA observed the first aid kit with all the elements including thermometer, tweezers and scissors as well as a first aid manual. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. There are no protective mechanisms on the cook top and four out of six residents are at risk if exposed to cook top devices per physician reports. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher is fully charged. The outside grounds were toured and there is ample shaded seating for residents. CONTINUED ON LIC 9099C DATED 09/01/2026 Exit gates are unlocked and operational. Emergency food and water was observed. The emergency disaster plan was reviewed during the visit. Facility provided documentation of last fire drill conducted on 06/04/2026 and drills are conducted quarterly. Facility provides activities in the form of exercise, games and music therapy. LPA reviewed resident and staff files. Resident files contained required documents including admission agreements, physician reports and resident appraisals. Staff files reviewed contained required documentation of medical clearance, training and criminal record clearance. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet. LPA observed staff are taking blood pressure for resident 2 (R2) with parameter medication orders (photos). None of the staff are skilled professionals. Medications appear to be administered per physician order. Based on the observations made during today's visit, deficiencies are being cited per Title 22 division 6 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Celine Rodriguez conducted an unannounced required visit to the facility for the purpose of conducting the required annual inspection. LPA Rodriguez explained reason for visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted facility administrator (AD) Elsa Roman about visit. For this visit, there are 2 staff members on duty, of which are background cleared and associated. The PUB475 "See Something, Say Something" poster was observed to be posted in the living room. LPA observed the Administrator's Certificate for Elsa Roman which expires on 6/23/2026. LPA toured the interior and exterior portions of the facility with staff on duty (S1). The facility is a one level structure and is licensed for 6 non-ambulatory residents, of which 0 may be bedridden and 2 may be on hospice. For this visit, there are a total of 6 residents in care, of which 1 is on hospice and 0 bedridden. There are a total of 6 resident bedrooms, of which all 6 are private resident rooms. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of 2 restrooms, of which were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature was measured to be at 115.8 degrees Fahrenheit. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguishers were dated and tagged for the date of 2025. LPA observed the emergency disaster and evacuation plan, which is posted at in the living room. Facility had back-up emergency food and water supply, located in the kitchen and in the garage. LPA observed that First Aid Kit had all the required components. Medications and toxins were also observed to be locked and inaccessible to residents in care. For the exterior portion, LPA observed patio furniture under shading, and the grounds were free of any hazards. There are 2 gates in the backyard, which were self-closing and self-latching. No bodies of water were observed. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. No citations were issued. Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced visit for the Required 1 Year Inspection. LPA explained the purpose of today’s visit, and was greeted and granted entry by Caregiver Angelamari Locsin. Administrator (AD) Elsa Roman arrived shortly after. For today’s visit, LPA observed a total of five residents in care and two staff members on duty. LPA observed the Administrator's Certificate for Elsa Roman which expired on 6/23/24, however, AD provided proof of payment to show that the certificate is in renewal process. Renewed certificate has not yet been received and is pending. LPA reminded Licensee to post certificate once received. LPA Ramirez toured the interior and exterior portions of the facility with caregiver. The facility is a single level structure and is licensed for six non-ambulatory residents, of which two may be on hospice and zero bedridden. There are a total of six bedrooms, of which six are resident bedrooms. LPA Ramirez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector and auditory exit alarms were tested and operational. There are a total of two restrooms. Restrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature tested between 105.9-107.2 degrees Fahrenheit. During today's visit LPA observed as a caregiver was reading a book to the residents in the living room. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to residents in care. Fire extinguishers were charged and one was located by the kitchen and one by the residents' bedroom hallway. CONTINUED ON LIC809-C.. LPA observed the emergency disaster and evacuation plan which is located by the entrance seating area. Facility had back-up emergency food and water supply. LPA observed that First Aid Kit had all the required components. LPA observed that medications and toxins were locked and inaccessible to residents in care. For the exterior portion, LPA Ramirez observed a shaded area, patio furniture, and the grounds were free of any hazards. There are two gates in the backyard, which both are self-closing and self-latching. No bodies of water were observed. LPA reviewed five resident files and two staff files. LPA interviewed residents and staff present. For today's visit no deficiencies were issued per Title 22 Division 6 of the California Code of Regulations.
