Type of visit: Required - 1 Year

At approximately 3:45 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and met with Minerva Villegas, Administrator. Facility is a Residential Care Facility for the Elderly (RCFE) with 28 residents in care and serves assisted living and memory care residents. Community has a Hospice waiver for 12. LPA obtained a copy of the resident roster, facility census, and staff roster. LPA will return at a later date to complete this annual inspection. No Deficiencies are being cited during today's inspection. Allegation investigated: Personal Rights

On 04/21/2026, at approximately 12:15 PM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conclude investigation and deliver findings regarding LIC802 - Complaint Report 21-AS-20260123104046, which was received by Community Care Licensing (CCL) on 01/23/2026. Reporting Party (RP) alleges a personal rights violation. LPA met with Minerva Villegas, Administrator. On 01/23/2026, during visit, LPA conducted an interview with Resident 1 (R1) where R1 stated they are well taken care of, have no complaints and do not have concerns that they are being over medicated. R1 appeared clean, calm, and comfortable. During this same visit, LPA obtained a copy of R1's LIC602A Physician's Report dated 11/19/2025 which indicates a dementia diagnonsis with "behavioral manifestations," medication to address these behaviors and direction that R1 is unable to manage their own medications. Continued on LIC9099C... Unsubstantiated Continued from LIC9099... LPA also obtained a copy of R1's hospital discharge summary dated 11/19/2025 which indicates that R1 was assigned a public guardian, placed under a county conservatorship and was discharged to the facility under the care of a third party Hospice agency. Today, LPA obtained a copy of R1's medication administration records dated 11/19/2025 through current, which indicate that R1's medications are administrated in compliance with R1's physician's orders. Based on the information obtained, LPA received conflicting information. Based on interviews conducted, observations made, and records obtained, the allegation of a personal rights violation is UNSUBSTANTIATED. A finding that a complaint allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED. No deficiencies cited. Type of visit: Required - 1 Year

At approximately 10:15 AM, Licensing Program Analyst (LPA) Julie Florio arrived unannounced to conduct a required 1-year annual inspection and was greeted by Minerva Villegas, Administrator. Facility is a Residential Care Facility for the Elderly (RCFE) with 27 residents in care. Facility is approved for 46 non-ambulatory residents, 10 of whom may be bedridden, and has a Hospice waiver for 12. At approximately 11:15 AM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one story building, was a comfortable temperature, and passageways were free from obstructions. Facility has agreed to post warning signs at al faucets and showers that test above the allowable water temperature range of 105 to 120 degrees F per Title 22 regulations in order to bring the facility into compliance. LPA observed a supply of hygiene products, clean linens, paper products, and incontinent care briefs available for residents. Residents' bedrooms were inspected and observed to have appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. Facility has at least two days of perishable food and one week of non-perishable foods, as well as an emergency water supply. Medications were centrally stored and locked. There is a covered seating area in the two courtyards with outdoor space for activities. Facility has internet available to residents in care and the phone was tested an operational. Facility agrees to purchase an internet access device and have it available to residents in care. Facility's fire extinguishers were observed charged and were last serviced 07/2025. Facility Smoke and Carbon Monoxide detectors were tested during facility's annual fire inspection 05/2025. Continued on LIC809-C... Continued from LIC809... Facility conducts quarterly disaster drills with the most recent drill conducted 04/2025. LPA observed facility's infection control plan and emergency disaster plan which was last updated 05/08/2025. LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights for emergency preparedness. Facility has a generator. Administrator provided LPA with a copy of the facility's current liability insurance. At approximately 1:30 PM, LPA reviewed six (6) staff files and five (5) resident files. Six (6) of six (6) staff files reviewed have all of the required paperwork and proof of current CPR training. Staff 1 (S1) was missing proof of current first aid training. Administrator registered S1 for the training during today's inspection to ensure the facility is operating in compliance moving forward. Additionally, six (6) of six (6) staff files reviewed were missing proof of the required annual training and annual medication training hours per regulation, (see LIC809Ds). Five (5) of five (5) resident files reviewed have all the required paperwork. Administrator and residents' families coordinates medical and dental visits for the residents and transportation to and from their appointments. Facility contracts with a third party vendor to provide transportation services as needed.. At approximately 4:00 PM, LPA reviewed medications and medication records which are maintained and stored in compliance with regulation. Facility does not manage P&I for residents. Deficiencies are cited from the California Code of Regulations (CCRs), and/or the Health and Safety Code. Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, or repeat violations within a 12 month period, may result in a civil penalty assessment. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Jacky Macias arrived unannounced at approximately 9:20am to conduct an Annual Required inspection and was greeted by staff. Administrator Minerva Villegas arrived shortly after. LPA discussed the purpose of the visit to Administrator. Hospice care waiver approved for ten(10) residents. Facility has an approved dementia plan of operation. Facility has a required Infection Control Plan, which is part of the facility's plan of operation. Fire clearance approval is for 46 non-ambulatory residents, of which 10 may be bedridden. At approximately 10:00am, LPA and Administrator initiated a tour of the facility which included resident rooms, kitchen, food storage, dining room and common area used by residents. Facility was at a comfortable temperature and passageways were free from obstructions. Resident rooms were furnished per regulation. Water temperature in various restroom sinks accessible to residents in care were all within the range of 105 to 120 degrees F allowed per regulation. Extra hygiene products and linens were available. Cleaning supplies are locked in supply closet. Facility has at least two days of perishable and one week of non-perishable foods which appeared to be of quality, stored per regulation. There is also a daily activity schedule for residents. Some of the activities available for residents are: chair exercises, puzzles, coloring, book club, movie night, bingo, arts and crafts, dancing, and karaoke. Five staff files and five resident files were reviewed and found to be well organized, thorough, and contained all of the required documentation. Staff have required First Aid and CPR certificates. Administrator Certificate for Administrator, Minerva Villegas (7030490740) Exp 10/21/2025. Medications are centrally stored in a secured room and were reviewed. No deficiencies. Fire extinguishers were last serviced May 17, 2024. Facility has carbon monoxide detectors and a fire alarm system that is serviced by an outside vendor that was last inspected on May 7, 2024. Most recent fire/disaster drill was conducted 4/10/2024. Administrator to conduct the next fire drill on 7/30/2024. Continued on LIC809C...
