At 11:45a.m., Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an unannounced Required One (1) year inspection to the facility. LPA met with the staff who granted entry to the facility and explained the reason for the visit. At about 11:55a.m., Administrator joined today’s visit. At approximately 12:30p.m., LPA and Administrator conducted a physical plant tour inside and out. During the tour, LPA observed that the facility is a single-story home located in a residential community with six (6) bedrooms and three (3) bathrooms. Fire Clearance was approved on 12/15/2017 (6) non-ambulatory and (6) bedridden residents. Approved hospice for two (2). Fire Emergency drill was last conducted on 03/01/2026. The facility is currently occupying five (5) residents. Facility maintains a telephone land line and it was observed to be operational. Required postings were observed in the dining area. The dual fire sprinkler/smoke detector appeared to function properly located in each bedroom and dining area. The carbon monoxide detector is located in the dining room. The fire extinguishers are throughout the facility with service date of 01/21/2026. During the visit the facility is set at 73 degrees Fahrenheit. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Kitchen: LPA observed kitchen appliances and fixtures were functional. The kitchen has a working stove, freezer, refrigerator, microwave and dish washer. LPA found at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrapped and stored properly as well. Knives were stored in a locked drawer in the kitchen. Food storage and preparation areas are clean Cont. on LIC 809-C Cont. from LIC 809 and inaccessible to pests. Garbage cans have tight fitting covers. Toxic cleaning supplies were stored and locked away next to stove. Laundry Area: the washer and dryer located next to kitchen. LPA observed the staff office/room to be inaccessible to residents where the facility files and medication are kept. Medications: are in a centrally stored and locked medication cabinet in staff office/room cabinet, including over-the-counter medicines; medications are properly labeled and checked for expiration dates. Each centrally stored prescription and PRN medication has been logged in the medications logged with proper documentation from the resident’s doctor. First-aid has all proper items and were observed to be stored in dining area and an extra one in the medication locked cabinet. Bedrooms: LPA observed six (6) bedrooms designated for residents' and staff use. Bedrooms #3 - #4 are for private use, bedrooms #1 - #2 are shared and two (2) staff rooms. All the resident’s bedrooms were properly furnished with appropriate dresser, bedding, and linens with sufficient lighting. Facility is well lit. Linens are stored in bedrooms and observed to have ample supply of clean linen, comforters, and towels in facility. Bathrooms: LPA observed three (3) bathrooms to be clean, sanitary and with necessary supplies. The appropriate grab bars and mats in the shower. Hot water temperature measured at a range of 117.9°F to 114.9°F within the required range. Resident’s personal hygiene supplies are kept separate in plastic containers. Towels and washcloths are not shared. Common Areas: These included the dining area and living room for residents. Residents dining table fits six (6) residents. No obstructions and/or tripping hazards throughout the facility. There are no issues with Fire Clearance. Surrounding Grounds: Entry and exits were free of obstruction. The facility has appropriate outdoor furniture with a shaded covered area for residents and visitors. There is a ramp access with sturdy hand railings. There is a locked detached garage. The detached garage is used for storage supplies such laundry detergents, toiletries, PPE and incontinent supplies. The outdoor area was enclosed, and no bodies of water were observed. Staff Files: they have criminal record clearances and are associated to facility. Staff have current first aid and training documentation showing training completed. Administrator's certificate was observed to be current. Resident Records: All five (5) resident records were reviewed. Residents’ records are complete and current at this time. Residents were also interviewed. Facility is within CA code of Regulations Title 22 or Health and Safety Code. No deficiencies cited during today’s visit. Exit interview conducted. On 4/30/2025 @ 10:30am, Licensing Program Analyst (LPA) Leizl de la Cerra conducted the required 1 year annual inspection. LPA met with designated administrator, Rima Abelian. This is a single story residence consists of four (4) bedrooms and three (3) bathrooms. Fire Clearance was approved on 12/15/2017 (6) non-ambulatory and (6) bedridden clients. Approved hospice for two (2). A tour of the physical plant was initiated at approximately 11:00Am and the following was observed: The facility has two fully charged fire extinguishers inspected on 01/22/2025. One fire extinguisher is located in dining room area, and the other at the bedroom hallway. Facility maintains a telephone land line and it was observed to be operational. Required postings were observed in the dining area. The smoke alarms and carbon monoxide detector are hard wired and inter-connected. Staff tested the dual smoke/carbon monoxide detector @12:30pm and was observed to be operational. The facility maintains a comfortable temperature at 74 degrees Fahrenheit. The air conditioner is operational. Kitchen: The facility has a kitchen area that is equipped with a refrigerator, microwave oven and sink. There were adequate supplies of perishable and nonperishable food and dining ware to accommodate a maximum capacity of six (6). Knives are kept in a locked box. There is a staff workstation located at the corner of the kitchen. Continue to LIC809-C Bedrooms: LPA observed all four (04) bedrooms to be properly furnished with appropriate dresser, night stand, chair, beddings, and linens with sufficient lighting. Extra linens and beddings are stored in the hallway closet. Bedroom #1 is occupied and shared by two (2) residents. Bedroom #2 is occupied and shared by two (2) residents. Bedroom #4 is occupied by one (1) resident. Bedroom #3 is occupied and shared by two (2) residents. Bathrooms: LPA observed three (03) bathrooms at the facility, two (02) are for residents to use and one (01) is for staff use only which is kept locked. Resident bathrooms have hand washing signs, soap and paper towels. Proper grab bars and non-slip bath mats. The hot water temperature measured between 106°- 110°F. Outside: LPA toured the outside area. LPA observed a shaded sitting area for residents. There is a ramp access with sturdy hand railings. There is a locked detached garage. The detached garage is used as a storage room, used for storing supplies such laundry detergents, toiletries, PPE supplies, incontinence supplies and wheelchairs. The facility does not have any bodies of water. Staff Records: LPA Reviewed files for two (2) staff members between 12:30pm to 1:00pm. Files are maintained at the facility in a locked room. Residents Records: LPA reviewed files for six (06) out of six (06) residents between 11:30am to 12:30pm. Files are maintained at the facility in a locked room. The facility uses the Medication Administration Record (MAR) log to document medications given. Medications are administered as prescribed by the Physician. Medications are bubbled packed. Medications are in a locked room. Operational Requirements: Liability Insurance policy is valid and will expire on 6/13/2025. Fire drill was last conducted on 3/01/25. The First Aid Kit is complete and current. Fire inspection completed on 1/22/2025. No deficiencies cited. Licensing Program Analyst (LPA) Leizl de la Cerra conducted an unannounced required 1-yr inspection. LPA was allowed entry by Turlybek Sadibekov, caregiver at 11:00am and explained the purpose of the visit. At 11:45am, Maro Mary Karibian, Administrator/Licensee and Rima Abelian, designated back up Administrator arrived, LPA explained the purpose of the visit. LPA reviewed resident files, staff files and conducted a tour of the facility between 12:00pm to 3:00pm. Outside: LPA toured the outside area. LPA observed a shaded sitting area for residents. There is a ramp access with sturdy hand railings. There is a locked detached garage. The detached garage is used as a storage room, used for storing supplies such laundry detergents, toiletries, PPE supplies, incontinence supplies and wheelchairs. The facility does not have any bodies of water. Common Area: LPA observed all furniture to be clean and in good repair. The facility maintains a comfortable temperature at 77 degrees Fahrenheit. The air conditioner is operational. The facility smoke alarm system is hard wired and interconnected. The facility uses a dual Carbon Monoxide/Smoke alarm detectors all over the common areas of the facility. Sprinkler systems are hardwired and interconnected. At 3:00pm they were tested and deemed operational. Facility maintains a telephone land line and it was observed to be operational. Required postings were observed in the hallway. The are 2 fire extinguishers, on in the dining area and the other in the hallway, with inspection date of 11/13/2023. Food Service and Kitchen Area: The kitchen appliances were functional. The kitchen has a working stove, faucet, refrigerator, and microwave. LPA found enough food for at least three (3) days perishable and seven (7) days non-perishable which are properly stored. Knives were stored in a locked drawer in the kitchen. Food preparation areas are clean. Garbage can have a tight fitting cover. Kitchen cleaning supplies were stored in a locked cabinet. Residents' dining table accommodates six (6) people. Continued to LIC809-C Personnel Records/Staffing: LPA Reviewed files for five (5) staff members. Files are maintained at the facility in a locked room. Proof of staff training, health clearance, vaccinations and 1st Aid/CPR training are current. Staff employed all have criminal background clearance, fingerprint cleared and associated to the facility. Residents Records: LPA reviewed files for six (06) out of six (06) residents. Files are maintained at the facility in a locked room. Physician's Report (including TB and Ambulatory Status), Consent For Medical Treatment, and Admission agreements, Medical/Functional assessments, Needs and Services Plans, Personal rights. Medications were reviewed for two (02) residents to confirm medication is given as prescribed and is documented properly. The facility uses the Medication Administration Record (MAR) log to document medications given. Medications are administered as prescribed by the Physician. Medications are bubbled packed. Medications are in a locked room. Bedrooms: LPA observed all four (04) bedrooms to be properly furnished with appropriate dresser, night stand, chair, beddings, and linens with sufficient lighting. Extra linens and beddings are stored in the hallway closet. Bedroom #1 is occupied and shared by two (2) residents. Bedroom #2 is occupied and shared by two (2) residents. Bedroom #4 is occupied by one (1) resident. Bedroom #3 is occupied and shared by two (2) residents. Bathrooms: LPA observed three (03) bathrooms at the facility, two (02) are for residents to use and one (01) is for staff use only which is kept locked. Resident bathrooms have hand washing signs, soap and paper towels. Proper grab bars and non-slip bath mats. The water temperature is 112.3 degrees Fahrenheit and 111.5 degrees Fahrenheit. Operational Requirements: Liability Insurance policy is valid and will expire on 6/13/2025. Surety Bond (Tokio Marine/American Contractors Indemnity Company)is in effect with bond amount of $3,000.00. The fire clearance is approved on 12/15/2017 for (6) non-ambulatory and (6) bedridden clients. Fire drill was last conducted on 5/18/2024. The First Aid Kit is complete and current. Fire inspection completed on 5-18-2024. No deficiencies cited. On 12/14/2023 at 09:00 AM, Licensing Program Analyst (LPA) Christopher Alemoh conducted an unannounced Required – Annual Continuation Inspection and met with Gayane Agazayan Administrator. Four (4) residents and two (3) staff were present during this inspection. Facility is following covid protocols. LPA Alemoh's temp was taken and asked to sign in upon entry. Facility has Facility is licensed to serve six (6) non-ambulatory residents, six (6) may be bedridden in any room. The facility also has an approved hospice waiver for six (6) residents. The facility currently has 2 non-ambulatory residents. The Annual Licensing Fees are current. The home consists of 1 floor level with: 2 staff room, 4 resident rooms, 3 restrooms, kitchen, dining room, and laundry room and isolation room. Facility Temp is 76 degrees Fahrenheit. At 01:40PM LPA began physical plant tour. LPA observed two fire extinguishers in the facility locked at ends of main hallway. Charge date is Nov 3rd, 2023. Facility also uses a sprinkler system; facility is also cleared for fire doors. The administrator accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. There are no security bars or weapons on the premises. LPA observed that Medications were safe, locked and inaccessible in the staff quarters. All medications observed were labeled and maintained in compliance with label instructions and State and Federal law. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, hot water temperature properly measured between 109.8-110.2F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards, doorways were free of obstructions. LPA toured the kitchen area and observed a two day supply of perishable and a seven day supply of non-perishable food. Knives and toxics were locked and secured in a drawer next to the sink. LPA observed supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days in the Pantry near the kitchen. First Aid kit and manual secured hanging on the wall behind the kitchen door. LPA tested all carbon monoxide detectors and smoke detector located in the kitchen area. Both devices were functional. LPA observed that all bedrooms and hallways are equipped with a carbon monoxide and smoke detector. At 02:40 PM LPA conducted a file review. All facility files are locked and stored in the Staff Room. 5 staff records were reviewed, 5 out of 5 staff records had current first aid certificates and had required criminal record clearances or criminal record exemptions. 6 resident records were reviewed and, 6 out of 6 client records had Admission Agreements, Medical Assessments, Pre-appraisals (or Reappraisals) and/or Needs & Services Plans. Last Fire Drill conducted 09/15/2023. An exit interview was conducted, Plans of Corrections were reviewed and developed with the Licensee.
