Allegation investigated: Staff did not maintain adequate phone communication access Staff did not allow resident access to their personal cell phone.
Licensing Program Analyst (LPA) Jose Tan conducted an unannounced initial complaint visit at this facility to investigate the above allegations. LPA met with Administrator Daniel Alexandryan and explained the reason for the visit. LPA conducted a physical plant tour at 9:23 AM, requested copies of facility documents relevant to the investigation at 9:45 AM, reviewed records between 9:45 AM to 10:45 AM and interviewed staff and residents between 11:00 AM to 1:00 PM. Regarding the allegation that Staff did not maintain adequate phone communication access, it was alleged that the reporting party (RP) called the facility three (3) times today (12/08/25) and the phone rang at least ten times but no one answered. LPA's record review today between 9:45 AM to 10:45 AM revealed that there was only one (1) recorded phone call at the facility phone number from RP's phone number on 12/08/25 and there was no recorded phone call from RP number on Administrator's personal phone number on 12/08/25. (continued on LIC 9099-C) Unsubstantiated (continued from LIC 9099) LPA's interview with the Administrator today revealed that the Administrator talked to RP and threatened the Administrator two (2) weeks ago and called the police on the facility. The police came but nothing happened per the Administrator. LPA's interview with Resident #1 (R1) today revealed that R1 had no problem communicating with family and friends and always kept own cell phone on own pocket at all times Regarding the allegation that Staff did not allow resident access to their personal cell phone, it was alleged that RP was not able to reach R1's personal cell phone and believes staff are not letting R1 use own cell phone. LPA's interview with R1 today revealed that R1 always kept own phone on own pocket at all times and had no problem calling anyone. LPA called R1's phone only to find out that the cell phone was deactivated and unable to make or receive calls, as LPA tested it multiple times. LPA made contact to R1's family member (FM) and informed the family member of R1's phone issue. LPA's interview with the administrator today revealed that R1's phone is a prepaid phone and probably no more load that is why it was deactivated. R1's FM got back to LPA and was told to reboot R1's phone. With R1's permission, LPA rebooted R1's phone and now working properly. LPA's interview with R1 today also confirmed that no staff had stopped using or took R1's cell phone at any time. LPA's interview with the other four (4) residents today revealed that four (4) out of four (4) residents interviewed stated that no staff had stopped them from using their own cellular phone or the facility phone from calling anyone. Based on the information gathered during this visit, these allegations are deemed unsubstantiated at this time. Exit interview conducted. Type of visit: Case Management - Deficiencies
Licensing Program Analyst (LPA) Huma Rahimi met with Staff #1 (S1) who granted access to the facility. The Administrator was contacted and LPA explained the reason for the visit. The purpose of the case management visit is to address deficiencies observed during the course of investigation control # 31-AS-20251027152920. Deficiencies observed were not alleged however are related to the complaint During the course of the investigation control # 31-AS-20251027152920 the following were observed: LPA was informed that the Administrator observed Resident #1 (R1) experiencing cognitive decline approximately two months ago. However, R1’s physician was not notified, and R1’s Appraisal/Needs and Services Plan was not updated to reflect the change in condition. Therefore, it was determined that the Administrator did not ensure that a timely reassessment and ongoing reappraisal were conducted, nor was the Needs and Services Plan revised in accordance with the resident’s change in condition. Moreover, LPA was informed that R1 had a fall on 04/27/25 and a former resident #6 (R6) who no longer resides at the facility was throwing up and went unconscious on 05/17/2025. LPA was informed that for both incidents regarding R1 and R6, 9-1-1 was called and both residents were transferred to the hospital on the above mentioned dates. However, no incident reports were submitted to the Community Care Licensing Department (CCLD) in a timely manner. LPA reviewed all incident reports on a system and did not observe any Incident Reports regarding R1 and R6. Based on Title 22 Regulation: a written Unusual Incident / Injury Report shall be submitted to CCLD within seven (7) days of occurrence. LPA informed the Administrator that all staff members are mandated reporters and they are all responsible for reporting. During today's visit, LPA collected both incident reports regarding R1 and R6. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Jose Tan met administrator Daniel Alexandryan for a One (1) Year Required visit for this facility. LPA observed that all six (6) residents were at the facility during the visit. A tour of the physical plant was conducted at 3:18 PM and the following was noted: There is only one entrance being utilized at the facility, there is a required poster posted at the main door. The screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Infection Control and Mitigation plan. The facility has a total of five (5) bedrooms and three (3) bathrooms. One bedroom is located in the converted garage that is designated for staff use only. Another bedroom is designated for staff use. One (1) bathroom is designated for staff use only. The facility is fire cleared for six (6) non-ambulatory residents, one (1) of which may be bedridden. Hospice waiver for six (6) residents. Living and dining room furniture were also checked. Dual smoke/carbon monoxide detectors are hardwired and interconnected and observed to be operational. The fire extinguisher is located in the kitchen and observed to be full and last purchased on 04/30/25. The backyard of the facility has outdoor furniture, with a covered shaded area for clients. There is no body of water in the facility. (continued on LIC 809-C) (continued from LIC 809) There is no garage at the facility, only car port located at the front. The garage was converted into an office for staff use. It has access both outside and inside the house but it has lock for security purposes. There are two (2) tools shed located, one in the front driveway and the other at the backyard, both tool sheds were observed to be locked and inaccessible to residents. Laundry area is located adjacent to the kitchen. Laundry detergents, cleaning agents and other toxins are stored in a locked cabinet in the laundry area. Food Service/Kitchen, the facility has sufficient perishable food in stock for two (2) days and has non-perishable food for seven (7) days in stock. Knives and sharps are observed to be locked in a kitchen drawer and inaccessible to residents. The Clients' rooms are adequately furnished with appropriate furniture and lighting system. Hallways/passageways are lit. Linens and towels are sufficient in stock in the bedroom hallway cabinet. The bathrooms were checked for cleanliness and proper operation. LPA observed the appropriate grab bars in the toilet and shower. The hot water temperature was measured at 108.6°F Medications: The medication cabinet is located in the living room and was observed to be locked. There is a complete first aid kit located in the kitchen. Client records: All six (6) residents records were reviewed and observed to be complete and updated. Staff records: Three (3) staff records were reviewed and observed to be complete and updated. Disaster Drill was last conducted on 06/23/25. Required posting observed in facility (complaint hot line poster). There are no health and safety issues observed during this visit. Exit interview conducted. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Abeye Duguma met with Daniel Alexandryan for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 1:40 PM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of three (03) bedrooms for residents and three (03) bathrooms. The facility is fire cleared for six (06) non-ambulatory of which one (01) may be bedridden and a hospice waiver for six (06). The facility is currently occupying five (05) residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. The garage is currently being used for storage and office. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 74°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be fully charged and last inspected on 02/27/2024. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 110.8°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication to be locked and inaccessible to residents. Facility has complete first aid kit.
