Licensing Program Analyst (LPA) J. Duarte arrived unannounced to conduct an Annual inspection. House Manager Lyn Garcera answered the door and LPA explained reason for today's visit and toured the facility with the house manager. Lyn contacted Licensee Helen Houck, who arrived shortly after. There are three residents at the facility. LPA observed residents sleeping and watching television in the activity room. The facility was observed to be clean and the temperature was set at 73 degrees F. Common areas were furnished well with adequate seating and lighting available. The kitchen was toured and it appeared clean and safe for food preparation. LPA observed a two-day supply of perishable and a seven-day supply of non-perishable food. The sharps were observed locked in a kitchen cabinet. Chemicals were stored in a locked cabinet under the kitchen sink. LPA toured the facility office. The medication was stored in a locked cabinet in the office. A complete first Aid kit with all requirements was stored in a locked office closet. LPA toured the resident bedrooms. The bedrooms had all required furnishings to include chair, dresser, and lighting. Beds were observed to have clean linen. The master bedroom hot water measured at 109 degrees F, the hallway restroom hot water measured at 109 degrees F. Restrooms had grab bars and slip resistant mats in the showers. LPA observed three out of five oxygen tanks in one of the resident's bedroom were not secured in a stand or to the wall. The garage was toured and LPA observed a refrigerator with an additional supply of food. Additional chemicals were observed stored in a locked garage cabinet. The facility has a washer and dryer in the garage. Detergent to wash was stored in a locked cabinet in the garage. The backyard has a patio and a gazebo with seating available. Continued in LIC 809-C. Continued from LIC809 Fire extinguisher were observed with a service date 05/06/2026. Smoke detectors and carbon monoxide detectors observed operational during today’s inspection. Facility has a pull station fire alarm and a sprinkler system. A fire prevention inspection was conducted by the Bakersfield Fire Department on 10/17/2025. The facility conducted a fire drill on 04/23/2026, per staff records. LPA reviewed client records, medications and staff records, and conducted a pill count. MARs reflected medication is being administered as prescribed. A deficiency is being cited. See LIC 809D. On 06/17/2025, Licensing Program Analysts (LPA) J. Duarte and M. Medina conducted an unannounced annual required inspection visit. LPAs met with House Manager Mya Lwin. Licensee, Helen Houck was contacted by telephone and arrived shortly after. There are currently four residents residing in the facility. LPAs inspected the interior and the exterior of the facility with Administrator. Facility observed to be well lit, odor free, and comfortable temperature set at 74 degrees F. LPAs observed residents in the living room watching television, building puzzles and/ or sleeping, at time of inspection visit. Facility living room and dining room observed to have adequate seating available. Resident bedrooms toured, bedrooms observed to have all required furnishings. Resident bathrooms toured, water temperature measured at 105.4 degrees F. Bathrooms observed to have grab bars in the shower and near toilet. Showers have shower chair and skid resistant mats available. LPAs observed kitchen to have all sharps and utensils locked and secured in kitchen drawers. Facility observed to have and adequate supply of food for the residents in care. Medications are centrally stored and locked in a kitchen cabinet. All medications observed to have original labels and be administered as prescribed. Outside of facility toured. All exits open free of obstruction. No hazards observed. Facility has seating and a shaded area for residents. Fire extinguisher present, one located in the hallway and one in the kitchen, both were serviced on 05/14/2025. Smoke and Carbon Monoxide detectors present and observed operational. Last Fire drill conducted 05/09/2025 according to facility records. LPA reviewed resident and staff files. Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Licensee, Helen Houck, Continual Administrator's Certification expires 10/22/2024. There are currently 3 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 106 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA reviewed 3 client files, and 5 staff files. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Allegation investigated: Physical Abuse

Licensing Program Analyst (LPA) Darius Williams conducted a follow up visit to deliver findings. LPA met with House Manager Florencia Topete. The Department as investigated the allegation regarding physical abuse. Based on interviews, observations, and record reviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Type of visit: Case Management - Deficiencies

Licensing Program Analyst conducted a case management visit in response to a visit conducted by the Department on 10/24/2023. LPA Williams met with House Manager, Florencia Topete and discussed the purpose of the visit. On 10/24/2023 Staff 1 and the Administrator reported no training had been completed regarding assisting residents with mobility and transfers. The Administrator reported training would be conducted regarding mobility and transfer of residents. Based on the Departments interviews a deficiency is being cited on the attached LIC 9099D page.
