Type of visit: Required - 1 Year

On 07/30/2026 at approximately 09:30AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Residential Care Facility for The Elderly (RCFE). LPA was greeted by Licensee Rose Devera and Administrator, Dinah Belandres. Facility has an approved fire clearance for capacity of six (6) non-ambulatory residents. The facility currently approved hospice waiver for capacity of three (3) individuals. Upon arrival, LPA was informed that there were five (5) residents in care and two (2) staff members on-site. Facility is a 1 story building with eight (8) Resident bedrooms (In two (2) out of eight (8); one of the bedrooms used as an office, another bedroom used as a staff room), 2 bathrooms, and common spaces. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 11:00 AM, LPA and Licensee toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for resident use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. Staff were in the process of preparing lunch and cleaning up kitchen at the time of this inspection. Toxins are stored inaccessible to residents. Water temperature measured 116.2 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents. Fire Extinguishers found to be last charged in November 2025 at the time of visit. Carbon Monoxide and smoke detectors were present and in order. There was enough lighting in all common areas, resident rooms, and hallways. Medication is centrally stored and secure in a cabinet in the kitchen. Continued on LIC809-C page... Continued from LIC809 page… LPA reviewed four (4) resident records and found all residents have care plans, signed admission agreements, and physician's report on file. Resident medication records are thorough and contains doctor’s order LPA reviewed three (3) staff records. All staff members had current first aid and CPR certification and annual training are complete. LPA was presented with proof of current CPR & 1st Aid certification. Administrator Certificate for Dinah Belandres # 7032809740 expires on 02/17/2027. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility failed to conduct and documented a disaster drill quarterly. Last drill was conducted on 07/01/2026. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by due date of 08/10/2026: -LIC 308 Designation of Facility Responsibilities -LIC 500 Personnel Summary -LIC 610 Emergency Disaster Plan (Last page) -LIC 9020 Register of Facility Client’s/Resident’s -Copy/Proof of Updated Certificate of Liability Insurance No deficiencies were observed in the areas inspected, No citations were issued during today’s visit. Type of visit: Required - 1 Year

On 07/10/2025 at approximately 09:45AM, Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct 1-Year Required visit of this licensed Residential Care Facility For Elderly (RCFE). LPA was greeted by Administrator, Dinah Belandres. The facility currently provides care for 6 clients. Facility has an approved fire clearance for 6 non-ambulatory residents. Upon arrival, LPA was informed that there were 3 staff members on-site. The facility is a single-story building with 8 Resident bedrooms (one of the bedrooms used as an office, another bedroom used as a staff room), 4 bathrooms, and common spaces. At approximately 10:15AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 11:05AM, LPA and Administrator toured the building and grounds which were found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed outdoor area chairs and tables are not safe for residents’ use. License agrees to buy new chairs and table for outdoor sitting area. The amount of fresh and nonperishable foods is within regulation. The facility kitchen, refrigerators and freezers were clean, and food was stored properly. Toxins are stored in the laundry room. Water temperature measured 110.6 degrees F degrees which is within regulation between 105- and 120-degrees F at faucets accessible to residents. The facility has emergency lightning if need it. Fire Extinguishers found to be last charged on 07/10/2024 at the time of visit. Carbon Monoxide and smoke detectors were present and in order. There was enough lighting in all common areas, resident rooms, and hallways. Medication is centrally stored and secure in a cabinet in the kitchen. Continued on LIC809-C… Continued from LIC809… At approximately 12:05PM, LPA reviewed 3 resident records and found 3 of 3 residents have current care plans, signed admission agreements, and physician's report on file. Medication records are thorough and contain physician's orders for each resident. At approximately 12:25PM, LPA reviewed 2 staff records. 2 of the 2 records did contain documentation of completed training records as required. However, S1’s TB test results were not present on file. Administrator agrees to share TB results(Chest X-Ray) with CCL for S1 before 07/11/2025(Technical Advice Given). Evidence of current first aid and CPR training was current. LPA was presented with proof of current CPR & 1Aid certification for the whole staff. Administrator Certificate is for Dinah Belandres #7032809750 on renewal process and renewal application submitted on 04/25/2025. LPA reviewed the facility emergency disaster plan. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility conducted and documented a disaster drill on 07/02/2025. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by due date of 07/20/2025: LIC 308 Designated LIC 500 Personnel Summary LIC 610 Emergency Disaster Plan (If changed) LIC 9020 Register of Facility Client’s/Resident’s Copy/Proof of Updated Certificate of Liability Insurance No deficiencies were observed in the areas inspected, No citations were issued during today’s visit. Type of visit: Required - 1 Year

On 7/19/2024, Licensing Program Analyst (LPA's) Tobola & Loera conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Administrator, Dinah Belandres. Licensee, Rose Devera was contacted and arrived later in the visit. The facility currently provides care for 6 residents, two of which are receiving hospice services and some of which with a diagnosis of dementia. LPA continued with a tour of the facility with Administrator, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguishers found to be last charged on 11/9/2023 at the time of visit. Smoke and carbon monoxide detectors found throughout the facility, were tested and found to be functioning. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations, with food stored in the kitchen refrigerator found to have appropriate coverings. Staff provide residents with meals according to dietary restrictions with an ample amount of fresh and healthy foods observed. Cleaning supplies and other toxins are to be safely stored in locked cabinets in the laundry room and under kitchen sinks. Upon inspection LPA's observed a bottle of bleach located in resident restroom. Item was immediately removed and secured. Sharps and other kitchen supplies that could pose danger if available to residents were found secured in a kitchen drawer. There was a supply of hygiene products and paper products available for residents. All resident’s bedrooms have lighting & appropriate furnishings and bedding items. A half rail was observed in resident (R1) bedroom without a proper physician's order. Administrator immediately removed bed rail and is to contact R1's physician for clearance. Technical Violation issued. Residents that were awake during the inspection were observed interacting with staff in the common area, in their bedroom watching television or resting. The facility encourages regular family visits and utilizes outdoor areas for resident exercise and mobility. There is emergency exit located in the backyard which was found to be unobstructed. There is an outdoor patio with shade and large outdoor space for residents to utilize with exits equipped with ramps for accessibility. Administrator is to provide a device with internet access for residents. Administrator agrees to send photo proof of device to CCLD Technical Advisory issued. Continued onto LIC809-C LPA's conducted a sample file review for staff and found all staff to have appropriate annual training and 1st Aid & CPR certification on file. LPA's requested for Licensee to update dementia training to meet regulation. Technical Violation issued. All required positing and signeage at the front and hallway of the facility were found to be in order with information easily accessible for staff and residents. LPA's conducted a sample file review for residents and found all needs & service plans and physician's reports to be in order. Lastly, a spot medication check was conducted, medication supply and medication records were all found to be in order. Administrator, Dinah Belandres's Administrator Certification 7032809740 is valid through 2/17/2025. Deficiencies cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Appeal rights given. Failure to correct the deficiency and/or repeat deficiencies within a 12-month period may result in civil penalties.
