On 7/22/26, Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced and met with the Administrator, Mirasol Mungcal, to conduct a Required-1 Year Inspection. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are five (5) bedrooms and three (3) bathrooms for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 106-109 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed knives, cleaning products and other toxins to be locked away and inaccessible to residents. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke detectors and carbon monoxide detectors are operational. Fire extinguishers and first aid kits are maintained and ready for emergency use. Facility has a pool which was found to be locked and secured during inspection. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed two (2) residents files and also reviewed two (2) staff files and found all required documents. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Exit interview conducted. Allegation investigated: Resident sustained skin issue due to lack of care from staff

On September 30, 2025, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Mirasol Mungcal . During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: **Report continued on 9099-C** Unsubstantiated Resident sustained skin issue due to lack of care from staff The department reviewed records, interviewed witnesses, staff, administrator and residents to investigate this allegation. Four staff and administrator interviews reflected that they were aware of resident, R1 health care needs who have been residing at the facility since 12/04/24. Staff interviews indicated that R1 was alert and orientated. R1s home health staff interviews indicated that R1 was getting appropriate care at the facility and there were no reportable concerns. Record review reflected that the facility was documenting all care notes per R1s care plan and R1 was receiving care from home health once a week and from other agencies and there were no issues to address. It was also noted that R1 had skin issues on different parts of their body prior to admission and all parties were aware about addressing those skin issues. Based on the information gathered, it has been concluded that even R1 was having skin issues which were noted during their hospital visit on 08/12/25, it was not due to lack of care from staff, therefore this allegation was found to be UNSUBSTANTIATED. Licensing Program Analyst (LPA) Melissa Parks arrived on Thursday August 8, 2024 to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA Parks reviewed resident (6) and staff files (2). All resident files contained the required paperwork. Staff files contained the required paperwork and training. LPA Parks and Administrator Mirasol toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, living room, kitchen, and backyard. In the areas toured, there were no health or safety violations observed. Facility was clean and well organized. Facility was current on fire drills. First aid kit was fully stocked. All required posting were observed. All knives/sharps were kept locked and inaccessible to residents. LPA obtained current copies of the following: LIC500, current liability insurance, and LIC610E. No deficiencies cited.
