Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct the annual inspection. LPA met with Administrator Lita Salva during today's inspection. Current there are 6 residents in care, and 2 are receiving hospice care. LPA toured facility with administrator to ensure health and safety of residents in care. LPA toured 5 resident rooms, 1 staff room, 2 bathrooms, kitchen, common living spaces, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. LPA toured the backyard and all exits are accessible and unlocked. There is a locked storage for medications and toxins. Food supply is adequate for 2-day perishable and 7-day nonperishable. LPA observed an adequate amount of linens and found the first aid kit to be complete. LPA reviewed 3 of 6 resident files and 2 staff files. LPA reviewed medications of three residents comparing with Centrally Stored Medication Record and physician orders. A review of staff records indicates that all facility staff has received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current first aid certificates and training completed. LPA observed a copy of current liability insurance. Water temperature was measured at 139 degrees. Type of visit: Case Management - Other

Licensed Program Analysts (LPAs) Cassandra Mikkelson and Michael Hood arrived at the facility to follow up regarding concerns during last pre-licensing visit on 10/16/2024. Facility was inspected both indoors and outdoors. LPAs inspected 5 resident bedrooms, 2 bathrooms, 1 staff room, common living areas, kitchen, and outdoor areas. Outdoor area is free from hazardous debris. Outdoor exits are clear and accessible. Fire clearance was granted on 07/30/24 for 5 non-ambulatory residents and 1 bedridden resident in room near exit 3. Kitchen is clean, sanitary, and in good repair. LPAs reviewed one (1) resident record. Licensee will change admission agreement once License has been approved. Comp III was reviewed on 10/16/2024 with licensee. Licensee has fixed the window screens and the hole in the wall in the staff room. In addition, licensee has updated facility sketch to reflect current resident rooms. Application is pending and LPAs will forward findings to the Centralized Application Bureau (CAB) for final review and approval. CAB will further contact applicant on final status of application. Licensing Program Analyst (LPA) Bethany Mirlohi arrived to conduct a pre-licensing inspection. LPA met with Licensee/Administrator Lita Salva during today's inspection. Facility was inspected both indoors and outdoors. LPA inspected 5 resident bedrooms, 2 bathrooms, 1 staff room, common living areas, kitchen, and outdoor areas. Outdoor area is free from hazardous debris. Outdoor exits are clear and accessible. First aid kit was present in the facility. Centrally stored medications are stored in locked staff area near the front door. LPA inspected resident bedrooms and the bedroom had appropriate furnishings, adequate lighting and storage. Bathrooms are clean, sanitary, and in good repair. Grab bars and non-skid mat was present in all bathrooms. Smoke detectors and carbon monoxide detectors were checked. Fire clearance was granted on 07/30/24 for 5 non-ambulatory residents and 1 bedridden resident in room near exit 3. Kitchen is clean, sanitary, and in good repair. A working telephone has be set up for residents use. Comp III was completed today with licensee. Licensee needs to fix window screens and a hole in the wall in the staff room. In addition, licensee stated facility sketch needs to be updated to reflect current resident rooms. LPA will return on a later date to complete pre-licensing inspection.
