Type of visit: Case Management - Deficiencies

On Tuesday June 30, 2026 Licensing Program Analyst (LPA) Nancy Guillen conducted an unannounced Case Management- Deficiencies visit. The visit is being conducted in conjunction with complaint control number 22-AS-20260627090909. Administrator(AD) Regat Kebede was notified via telephone and later arrived to assist with the inspection. During the investigation into the complaint, LPA toured the facility, requested documentation and did a health and safety check on the residents in care. At approximately 8:30am, LPA observed the medication cabinet to be unlocked and accessible to residents in care. LPA asked Administrator to lock the cabinet. AD called Staff 1(S1) to lock the medication cabinet. S1 proceeded to look for keys. While S1 looked for the keys LPA asked S1 how long the cabinet had been unlocked for. S1 stated it was unlocked for "about 20 minutes." The key was found and the medication closet was locked with LPA present. At approximately 8:40 am, LPA asked to see the staff room. LPA observed an individual (ID) siting in the room with the lights off. LPA asked the individual for their name and AD responded stating this individual was only there for an interview. LPA asked to speak to the individual privately. During the interview the individual stated, they had been at the facility since 6:30 am and are scheduled to leave at 6pm. They also stated the visit for today was to "do the job and they see how I do." LPA verified on Guardian that the application is currently pending. The individual was asked by the AD to leave the facility and ID left immediately after. Continued on LIC809-C At approximately 8:46 am LPA observed WD-40, Lysol Disinfectant Spray, Probiotic Plus & Cranberry and medication Escitalopram in an unlocked staff room. LPA asked AD to lock the door, however the door does not have the capability to lock. LPA then had AD remove the items from the room. At approximately 8:49am, LPA observed Saline Solution, Pain Relieving Cleansing Spray and medication Latanoprost in bedroom for Resident 1. LPA asked AD to remove the items. Items were removed and placed in the locked medication closet. Based on the information gathered during the visit, deficiencies are being cited on the attached LIC809-D page. An exit interview was conducted with Administrator Regat Kebede. Type of visit: Required - 1 Year

On March 16, 2026, at 2:00pm, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim met with Administrator(ADMIN) Regat Kebede. The facility is licensed to operate for six (6) nonambulatory residents and have a hospice waiver for five (5) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, one (1) staff bedroom, four (4) bathrooms, storage room, living area, dining area, kitchen, two outdoor covered areas, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with House Manager Regat Kebede. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, and Resident Room 6. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 115.7 degrees F to 120.0 degrees F. A comfortable temperature of 75 degrees F was maintained in the facility. LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were locked, stored, and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. Emergency food, emergency water, and emergency supplies are stored in the garage. The smoke detectors and carbon monoxide detectors were operable. A working telephone (714-693-9156) and video teleconferencing device dedicated to the residents both remain available. Last emergency drill was conducted on March 2, 2026 and are conducted quarterly. First aid kit is maintained and contains all the necessary elements. Liability insurance is effective June 30, 2025, and expires June 30, 2026. LPA Kim conducted an audit of resident files (R1-R6), staff files (S1-S6), and medication and medication administration record that were in all in order and complete. LPA conducted interviews with two residents and two staff. No deficiencies were cited during the inspection visit. On March 11, 2025, at 1:30pm, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by House Manager (HM) Eyob Kebede and LPA Kim explained the purpose of the visit to HM Kebede. Administrator (AD) Regat Kebede arrived at the facility around 1:50 PM. The facility is licensed to operate for six (6) nonambulatory residents and has a hospice waiver for three (3) residents. The facility is a single story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, one (1) staff bedrooms, four (4) bathrooms, storage room, living area, dining area, kitchen, outdoor covered patio, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with HM Kebede. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, and Resident Room 6. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 106.1 degrees F to 114.9 degrees F. A comfortable temperature of 72 degrees F was maintained in the facility. LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. The following bedrooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, and Staff Room 1. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 114.9 degrees F. A comfortable temperature of 75 degrees F was maintained in the facility. LPA Kim observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available and maintained properly. A working telephone (714-693-9156) remains available. LPA Kim reviewed the facility’s plan of operation, emergency and disaster plan, and fire/safety drill log. The facility conducts fire/safety drills quarterly. The facility has two (2) fire extinguishers that are charged and they were all serviced on February 9, 2024, smoke detectors, and carbon monoxide detectors were operable. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). Emergency food, emergency water and emergency supplies were stored in the garage. First Aid was maintained. LPA Kim conducted an audit of two (2) resident files (R1-R2), three (3) staff files (S1-S3), and medication and medication administration record were all in order and complete. LPA conducted two (2) staff interviews. A Technical violation was assessed during this inspection visit according to the California Code of Regulations (Title 22, Division 6, Chapter 8).
