On February 20, 2026, at 8:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by Caregiver (CG) Carlo Asinas. Licensee (LI) Eyob Kebede and Administrator (ADMIN) Regat Kebede arrived at the facility at a later time and LPA Kim explained the purpose of the visit. The facility is licensed to operate for six (6) nonambulatory residents and has a hospice waiver for five (5) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) staff bedrooms, three (3) bathrooms, living area, dining area, kitchen, outdoor covered patio, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with LI Kebede. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, and Resident Room 5. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 105.6 degrees F to 108.5 degrees F. A comfortable temperature of 70 degrees F was maintained in the facility. LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. Emergency food, emergency water, and emergency supplies are stored in the garage. The smoke detectors and carbon monoxide detectors were operable. A working telephone (714-921-9346) and video teleconferencing device dedicated to the residents both remain available. Last emergency drill was conducted on January 25, 2026, and are conducted quarterly. First aid kit is maintained and contains all the necessary elements. Liability insurance is effective June 30, 2025, and expires June 30, 2026. Fire extinguisher is charged, mounted in the kitchen, and was last inspected on March 4, 2025. LPA Kim conducted an audit of resident files (R1-R5), staff files (S1-S6), and medication and medication administration record that were in all in order and complete. LPA conducted interviews with three residents and two staff. No deficiencies were cited during the visit. On July 11, 2025, at 8:45 AM, Licensing Program Analyst (LPA) Edward Kim conducted a case management visit to follow up a request to add two bedrooms in the living area. LPA Kim was greeted and granted entry to the facility by Caregiver (CG) Ellein Bobis. Licensee Eyob Kebede arrived at the facility around 9:00 AM and LPA Kim explained the purpose of the visit. LPA Kim toured the indoor and outdoor of the physical facility with Licensee Kebede. LPA Kim observed where the facility is requesting to change in the living area to add two bedrooms. The facility is large enough to provide comfortable living accommodation and privacy for residents and staff. The common area that will be under construction, the living area, will be sufficient in space to promote and facilitate activities. The two new bedrooms will be large enough to reside in and to allow easy passage through the bed and other required furniture. On February 21, 2025, at 9:20 am, Licensing Program Analyst (LPA) Edward Kim conducted a continuation visit for a required 1-year annual visit. LPA Kim was greeted and gained entry by Caregiver (CG) Ellein Bobis. Assistant Administrator (AA) Eyob Kebede and Administrator (AD) Regat Kebede arrived to the facility. LPA Kim conducted an audit of resident records (R1-R4) and medication and medication administration record that were complete and in order. LPA Kim conducted staff interviews (S1-S2). First aid kit is maintained and contains all the necessary elements. The smoke detectors and carbon monoxide detectors were operable. A video teleconferencing device is dedicated to the residents remain available. Deficiencies were cited during this visit as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. LPA observed the following deficiencies: there is a staff room inside the family room that is not reflected in the facility sketch and there are three walls bolted on the ground with 14.75” space from the ceiling that is not reflected in the facility sketch. Assistant Administrator Yonathan Kebede arrived at the facility as the Designated Facility Representative and LPA Kim explained the purpose of the visit. The facility is licensed to operate for six (6) nonambulatory residents and has a hospice waiver for five (5) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, three (3) bathrooms, living area, dining area, kitchen, outdoor covered patio, and an attached two car garage. The following observations were made and these deficiencies will be addressed on a subsequent visit: 1) there is a staff room inside the family room that is not reflected in the facility sketch and 2) there are three walls bolted on the ground with 14.75” space from the ceiling that is not reflected in the facility sketch. Due to time constraints LPA will conduct the following on a subsequent visit: LPA will conduct record review for residents, an audit of medication and medication administration review, conduct staff interviews, check the smoke detectors and carbon monoxide detectors, review the internet device with video teleconferencing dedicated to the residents, and review the first aid kit on the next visit. No deficiencies were cited during today’s visit. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA met with Administrator Regat Kebede and explained the reason for the visit. The Administrator's certificate expires on 3/15/2024. LPA and Administrator toured the facility. Facility is a 5 bedroom home with 3 bathrooms, kitchen, living room with a room divider, dining room and a two car garage. LPA observed the fireplace in the living is room is screened. LPA observed the PUB 475 poster does not measure 20 X 26 inches, it is 12 X 16 inches. LPA observed a two day perishable and seven day non-perishable food supply in the kitchen. LPA observed the stove lights unassisted. The fire extinguisher in the kitchen is fully charged. The garage is kept locked and used for storage. LPA observed the cleaning supplies are kept locked in the garage. LPA observed all resident rooms were clean and had the required furnishings. The hot water measured 105.0 degrees Fahrenheit in all 3 bathrooms. All 3 bathrooms are clean and operational. LPA observed clean linens in the hall closet. The backyard has a covered patio with a seating area for residents. No bodies of water observed in the backyard. The exit gate is operational and is latched. No obstacles or hazards observed in the backyard. No obstacles or hazards observed in the facility. LPA inspected the first aid kit. The first aid kit does not have a current first aid manual but has all the other required elements. LPA reviewed 2 out of 2 staff files. Both staff have current CPR/First Aid training. Both staff only have 3 hours of current training. LPA reviewed 5 out of 5 resident files. LPA observed R1 did not have a signed physician's report (LIC 602A) and R3 who has been diagnosed with Dementia did not have a current physician's report. LPA observed all medications are kept locked in the hall closet. LPA reviewed 5 out of 5 resident medications. No discrepancies observed. LPA interviewed 2 staff. Residents declined to be interviewed. LPA consulted with Administrator concerning reporting requirements and facility record requirements. Violations are being cited per Title 22 division 6 of the California Code of regulations.
