Licensing Program Analyst (LPA) Gabriela Castro conducted an unannounced required annual visit using the Compliance and Regulatory Enforcement (CARE) Tool. LPA was greeted by facility staff explained the reason for the visit. Brad Garberich, Co-Administrator arrived shortly thereafter. The facility is licensed for 6 non-ambulatory residents ages 60 and over. Facility is approved for 6 hospice residents. During the inspection there were four (4) residents under hospice. Observations: Personal Rights postings (LIC 613C and Ombudsman), Complaint Poster (PUB 475), and nondiscrimination notice were observed in a common area. Residents had access to personal space, privacy, and adequate storage. No firearms/weapons were present. Physical Plant The facility is located in a residential neighborhood and consists of a single-story home. The home includes four (4) resident bedrooms and two (2) bathrooms, one of which is a private restroom. The facility also contains a combined living room and dining area furnished with living room furniture, a family room, a kitchen, a garage containing a washer and dryer, a front yard, and a large backyard. The front yard includes a long driveway with a walkway leading to the front entrance. Each resident bedroom was observed to contain the required furnishings, including a bed with mattress, linens, dresser, chair, and adequate lighting. Bathrooms were clean and equipped with required grab bars near the toilets and in the shower areas, as well as non-skid mats. Hot water temperatures measured within the required range of 105°F to 120°F. Cleaning supplies and other toxic substances were observed to be stored in locked cabinets and were inaccessible to residents. A Hoyer lift was available to assist residents with transfers and, according to the caregiver, is used only as needed. Extra linens and towels were available and stored in the hallway closet. Fire extinguishers were observed throughout the facility and were readily accessible for emergency use. The backyard provided shaded seating areas for residents to enjoy outdoor activities. All passageways and exits were observed to be clear, safe, and unobstructed. No bodies of water were observed on the premises. (Continued on LIC809C) Food Service: Refrigerators/freezers were maintained at proper temperatures (refrigerators maximum of 40 degrees °F and freezer 0-degree °C) with sufficient supply of 2-day perishable and 7 days non-perishable food. Fresh produce, proteins, and dry goods were stocked. Knives and were observed in a locked kitchen drawer. Health-Related Services & Records: Three (3) residents files were reviewed and contained current required documents Admissions Agreements, Pre-Placement Appraisals, Consents, Physician’s Reports with TB/ambulatory status and Rights acknowledgments. Three (3) residents’ medications were reviewed; medications were observed to be centrally stored in a locked hallway cabinet. MAR logs were observed to be current. Disaster Preparedness: Last fire/earthquake drill was conducted on July 9, 2026, with logs available. LIC 610D Emergency Disaster Plan was posted in the living room area. Emergency supplies (water, food, flashlights, batteries, first aid) were observed in the garage. Infection Control Plan was updated. Personnel Records & Training: Three (3) staff files were reviewed and included criminal record clearances, CPR/First Aid, required training and TB screenings. Administrator Certificate for Laurie Gallegos was valid through September 9, 2026. Insurance: Liability insurance was in compliance with an expiration date of September 18, 2026. The facility was observed to be operating in accordance with applicable Title 22 regulations. No deficiencies were observed or cited during today's visit. Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection visit. LPA was greeted by Caregiver Cherry Dipon and the purpose of the visit was explained. Administrator Laurie Gallegos was contacted by phone. The facility is licensed to serve six (6) non-ambulatory adults age range 60 and over. Bedrooms #2 and #4 approved for two (2) non-ambulatory each. Bedrooms #1 and #3 approved for one (1) non-ambulatory each. Garage not approved as a living area. The facility is a single-story home in a residential neighborhood of San Dimas. The home consists of living room, dining area, kitchen, family room, four (4) resident bedrooms, two (2) bathrooms, front and back yards. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: The front and backyard are well maintained and there are no pools or large bodies of water. There is a shaded area located in the backyard with sufficient seating for residents in care. Passageways and exits are free of obstruction. The water temperature was tested in bathroom #1 in the hallway and measured 111.2 degrees F and bathroom #2 water temperature measured 111.3 degrees F, which is within the required 105 degrees F - 120 degrees F. LPA observed grab bars and non-skid mats in both the bathrooms. continued on LIC 809C Resident bedrooms have all require furniture which includes, for each resident, a bed, a chair, night stand, a lamp, or lights sufficient for reading. Bed linens were clean and in good repair. There is closet space for clothing and other belongings. Hall closet contained extra clean linens. Interconnected smoke detectors were observed throughout the facility and are operable. There is a carbon monoxide detector located at the entrance of the home and another by the back bedroom. There are two (2) fire extinguishers located in the hallway and near the kitchen. Emergency drills are conducted quarterly, the last was conducted 07/04/25. Kitchen appliances are clean and were operating at the time of the visit. Sharps are locked in a kitchen drawer and are inaccessible to residents. Cleaning supplies and disinfectants are locked under the sink and are inaccessible to the residents. Sufficient supply of 2 days perishable and 7 days non-perishable foods were observed. Fireplace located in the living room is inaccessible with a covered screen. Dining room, family room and living room are clean and free of obstructions and have sufficient seating for residents in care. Medications are centrally stored in a cabinet in the dining room. Medications are documented properly and given as prescribed. First Aid kit was fully stocked with current manual. Three (3) resident and three (3) staff files were reviewed and were observed to contain required documentation. R#3 physician’s report indicates bedridden, facility does not have fire clearance for bedridden. Per California Code of Regulations, Title 22, an immediate $500 Civil Penalty was assessed for Fire Clearance. Per California Code of Regulations, Title 22, and California Health and Safety Code, deficiencies were cited during today’s visit. Licensing Program Analyst (LPA) Daniel Konishi made an announced visit and met with Licensee/Administrator, Laurie Gallegos conduct a announced Pre-Licensing evaluation. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: An application was submitted to Community Care Licensing Department (CCLD) on for an initial application of a Residential Care Facilities for the Elderly (RCFE) to serve adults ages 60 and over. Approved for six (6) ambulatory, of which five (5) may be non-ambulatory residents. Bedrooms#2 and 4 are approved for two (2) non-ambulatory. Bedroom #3 is approved for one (1) non-ambulatory and Bedroom #1 is approved for one (1) ambulatory hospice waiver granted for six (6) residents. Structure: Facility is a single-story home located in a residential area consisting of four (4) bedrooms, one (1) bathroom, kitchen, dining room, living room, family room, garage, and a backyard with seating and shade. Front entry and all exits have a wheelchair ramp. Operational Requirements: The facility did not have proof of liability insurance during today's visit and that they were instructed to obtain the liability insurance prior to accepting the first resident and will submit proof of that insurance to the department once it is obtained. Bedroom Clients: Bedrooms are equipped with a bed, night stand, chair, lamp, dresser, trash bins, and overhead lighting. Bathrooms: One (1) full bath equipped with working toilet, wash basins, bathtub/shower and One (1) full bath equipped with working toilet, wash basins, walk in shower. Linens & Hygiene Supplies: All beds had the required linen/supplies which include, pillowcase, mattress pads, fitted sheet, blanket and bedspreads. Adequate supply of linens is stored in linen closets. Emergency Phone Numbers, Exit Plan: Emergency numbers and Exit plans are posted and readily available for review. Three (3) fully charged fire extinguishers was observed. Facility has a land line telephone. Food Service: Dishes, cups, and flatware are stored in the kitchen cupboards, inspected and in good repair. Knives, cutlery, and other sharp kitchen utensils were observed locked and inaccessible. Adequate food supply is stored in the kitchen and consists of the following: 2-day perishables, and 7-day non-perishables. Emergency water supply was observed. Carbon Monoxide/Smoke Detectors: Where observed and tested. Appliances: Refrigerator, oven, microwave, dishwasher and washer/dryer are in good condition. The residence is equipped with central heating and air conditioning. Toxins: Cleaning supplies, and toxins are locked only accessible to staff. Water Temperature: Hot water was tested in all bathrooms, and kitchen sink. Water temperature was within normal limits 105 degrees Fahrenheit and not more than 120 degrees Fahrenheit. Medication, First-Aid Kit & Book: Designated centrally stored medications cabinet, and the first-aid kit has been inspected which has at least the following: tweezers, scissors, antiseptic, bandages, gauze, thermometer; including a current First Aid manual. Resident & Staff Files: Designated area for files will be in locked closet. Pools/Jacuzzi/Body of Water: No bodies of water were observed. Fire Clearance: Fire clearance was approved on 07/05/2024 for six (6) ambulatory, of which five (5) may be non-ambulatory residents. Bedrooms#2 and 4 are approved for two (2) non-ambulatory.Bedroom #3 is approved for one (1) non-ambulatory and Bedroom #1 is approved for one (1) ambulatory hospice waiver granted for six (6) residents. Component III: Component III was reviewed during inspection. Pre-Licensing is complete, and this facility has no deficiencies.
