Allegation investigated: Staff did not provide proper notice of rent increase to authorized representatives. Staff are falsifying documents.

Licensing Program Analyst (LPA) Mary Rico conducted an unannounced visit to investigate and deliver findings on the allegations listed above. LPA met with staff Sandy Flores and House Manager Rose Macdangdang, LPA explained the purpose of the visit. The investigation consisted of staff interviews, resident interviews and record review. The licensee was contacted and informed about today’s visit. For the allegation, Staff did not provide proper notice of rent increase to authorized representatives. During staff interviews, the Administrator informed LPA that the proper notice of the rent increase has been provided to all residents authorized representatives. During the record review, LPA observed that the proper documents were provided to residents and their authorized representatives.During resident interviews, residents were unable to collaborate on the allegation. Unsubstantiated For the allegation, Staff are falsifying documents. During staff interviews, three out of the three staff stated they have not falsified documents. During record review, LPA observed appropriate documents. Based on the evidence found during the investigation, the two (2) allegations listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Mary Rico made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with House Manager Rose Mary Macdangdang and staff Sandy Flores De Los Santos was granted entry to the facility. Licensed capacity is (6) current census (5). LPA was accompanied by House Manager Rose Macdangdang to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designate space for resident/staff files. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care .Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Facility has a variety of food available for residents. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (2) resident files for admission agreements, updated physician reports, and needs and services plans. LPA reviewed (2) resident medications and (2) hospice files. LPA also reviewed (2) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Ryan Gardner made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Administrator Noelia Garcia and was granted entry to the facility. At the time of the visit, there were three (3) staff present and five (5) residents present. The facility is a four (4) bedroom, two (2) bathroom home with a kitchen, a dining area, a living room area, and an attached garage. The facility is a Residential Care Facility for the Elderly (RCFE). The facility is licensed for a capacity of six (6) non-ambulatory residents, four (4) of the six (6) residents may be bedridden. The current census is five (5) residents. LPA was accompanied by Administrator to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to interior and exterior passageways. The facility is maintained at a comfortable temperature. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperature in the bathroom to be at 106.1 degrees F. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Postings such as the facility license, personal rights, the CCL complaint poster, labor laws, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. Medications are kept inside the medication cabinet in the kitchen inaccessible to residents. LPA discovered that Resident R1 had medications that were pre prepped and being stored in plastic cups. The facility will be issued a type A deficiency for storing R1’s medication outside of the original prescription bottle. Overall, the facility is clean and in good repair. Food Service: Non-perishable and perishable food supply is sufficient for the residents in care. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed three (3) residents files for admission agreements, updated physician reports, and needs and services plans. LPA discovered that three (3) out of three (3) residents files did not have a needs and services plan. The facility will be issued a type B deficiency for not having needs and services plans for the residents. LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. Medications/MARs records were audited and appeared to be dispensed and logged appropriately. Based on the observations made during today’s visit, two (2) deficiencies were cited per Title 22, Division 6, of the California Code of Regulations.
