Type of visit: Case Management - Deficiencies
On 03/06/2026 Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to facility in regards to complaint 56-AS-20251203115650. LPA met with Administrator Bene Molinitas and explained the purpose of today's visit. The department received a complaint on 12/03/2025 in regards to Resident #1 (R1). Per investigation, it was founded that R1 was admitted to facility with a stage 4 pressure injury. The facility did not follow proper reporting requirements and did not report or submit a written exception request for prohibited health care condition to licensing department. Based on the observations made during today’s visit, a deficiency was cited per Title 22, Division 6, of the California Code of Regulations. Allegation investigated: Unlawful Eviction. Staff is not providing night supervision to resident in care. Staff is not meeting resident dietary needs. Licensee is not accommodating care needs of the resident in care.
Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit for the purpose of delivering findings for the above allegations. LPA met with Staff Lorena Delacruz and explained today's visit. LPA contacted Administrator Restituto Calilung via telephone to inform of today's visit. On 12/03/2025, the department received a complaint with allegation in regards to unlawful eviction. Per facility records and interviews, Resident #1 (R1) requested to be sent to the hospital due to personal needs. LPA interviewed R1 who indicated facility did not unlawfully evict R1, R1 chose to leave facility. Second allegation alleges that staff is not providing night supervision to resident in care. LPA conducted (5) resident interviews. Per interviews, there was not enough evidence to corroborate that allegation of staff not providing night supervision may have or may have not occurred. Unsubstantiated Third allegation alleges that facility staff are not meeting dietary needs of residents in care. LPA conducted (5) resident interviews. 3 out of the 5 residents indicated dietary needs of residents are being met. LPA observed physician report for Resident #1 (R1) did not indicate special dieting needs. Fourth allegation alleges licensee is not accommodating care needs of the resident in care. LPA conducted (2) staff interviews and (5) resident interviews. Due to insufficient evidence, investigation did not reveal that licensee was not accommodating care needs for resident in care. Based on the evidence gathered during today’s investigation, the allegations listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, and this report (LIC9099) was discussed and provided to Staff Lorena Delacruz. LPA contacted Administrator Restituto Calilung to inform of investigation findings and conduct exit interview. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Raquel Hernandez made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Administrator Bene Molinitas and was granted entry to the facility. Licensed capacity is (6) current census (4). LPA was accompanied by Administrator Bene Molinitas to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. There was a designated space for resident/staff files. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Facility has a variety of food available for residents. Dishes, cups, and utensils were also stored properly. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (3) resident files for admission agreements, updated physician reports, pre-placement appraisals and needs and services plans. LPA reviewed (3) resident medications. No issues were observed. **Continuation on LIC809C** LPA also reviewed (3) staff files for First Aid/CPR certification, criminal record clearance, training's, and health screenings. No issues were observed. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced visit to deliver findings on the allegations listed above. LPA met with Administrator Bene Molinitas and explained the purpose of the visit. The investigation consisted of staff interviews, resident interviews and facility tour. For the allegation, Staff did not safeguard resident's personal items. LPA Hernandez conducted five (5) resident interviews and three (3) staff interviews. During resident interviews 5 out of the 5 residents stated the facility does safeguard and keep their personal belongings safe. During the staff interviews 3 out of the 3 staff stated they ensure resident's personal belongings are safe by allowing residents to keep belongings with them or in their room cabinet. Unsubstantiated For the allegation, Staff made inappropriate comments towards resident. LPA Hernandez conducted five (5) resident interviews and three (3) staff interviews. During resident interviews 5 out of the 5 residents stated staff do not speak inappropriate with them. During staff interviews 3 out of the 3 staff stated they do not make inappropriate comments towards any of the residents at the facility. For the allegation, Staff does not treat resident with dignity and respect. LPA Hernandez conducted five (5) resident interviews and three (3) staff interviews. During resident interviews 5 out of the 5 residents stated staff does treat them with dignity and respect. During staff interviews 3 out of the 3 staff stated they treat all their residents at the facility with dignity and respect. Based on the evidence gathered during today’s investigation, the allegations listed above are deemed UNSUBSTANTIATED. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Allegation investigated: Staff do not provide quality meals to residents in care. Staff do not ensure residents' rooms are kept clean.
On 04/18/2024 at 08:20 AM, Licensing Program Analyst (LPA) Melody Brown conducted an unannounced visit to the facility to commence a complaint investigation. LPA Brown was greeted and granted entrance by a staff member and LPA Brown met with House Manager Bene Molintas. LPA Brown identified herself and discussed the purpose of the visit and the elements of the allegations with House Manager Bene Molintas. Licensee/Administrator Restituto "Resty" Calilung arrived during the visit. LPA Brown conducted a quick tour of the facility, interviewed residents and staffs, and obtained facility records. The investigation consisted of file review and interviews with relevant parties. The first allegation indicates that Staff do not provide quality meals to residents in care. During the investigation, LPA Brown did not find evidence to corroborate the allegation. Interviews with four (4) of four (4) residents indicated that staffs at the facility are serving them good quality meals, and good portion of food for their breakfast, lunch and dinner. Residents interviewed revealed they don't have any complaints on their meals at the facility because staffs at the facility provide quality meals everyday. *** Continuation in LIC9099C *** Unsubstantiated Residents interviewed reported that staffs were informing them of the facility menu every week. LPA brown unable to interview Resident #1 (R1) as R1 moved out at the facility on 04/13/2024. Interviews with four (4) of four staff indicated that they are providing quality meals to their residents everyday and no incident happened at the facility that they served poor quality meals to their residents. Staffs interviewed added that they are informing their residents of their facility menu every week. During the facility visit on 04/18/2024, LPA Brown observed that staffs at the facility provided nutritious breakfast to their residents as residents were served rice porridge with milk, mixed fruit and a choice of drink. The second allegation indicates Staff do not ensure residents' rooms are kept clean. Interviews with four (4) of four (4) residents indicated that staffs at the facility are cleaning their room everyday and there's no incident that staffs at the facility did not ensure that their rooms are kept clean. Interviews with four (4) of four staff indicated that they are cleaning the residents room everyday and they always ensure that residents rooms are kept clean. Staffs interviews revealed that staffs are cleaning residents room twice a day, once in the morning and before bed time. During the facility visit on 04/18/2024, LPA Brown observed the resident rooms were kept clean. Based on the evidence, the allegation that Staff do not provide quality meals to residents in care (Allegation #1), and Staff do not ensure residents' rooms are kept clean (Allegation #2) are UNSUBSTANTIATED. Type of visit: Case Management - Deficiencies
On 04/18/2024 at 08:20 AM, Licensing Program Analyst (LPA) Melody Brown met with Licensee/Administrator Resty Calilung to initiate a Case Management Visit. The investigation consisted of observation, interviews, and a review of pertinent documentation. During the facility visit on 04/18/2024, Licensing Program Analyst (LPA) Melody Brown observed Staff #3 (S3) working at the facility, providing care and supervision to the residents in care without criminal background clearance. S3 reported to LPA Brown that S3 started working at the facility on 03/29/2024. Licensee/Administrator Restituto "Resty" Calilung confirmed to LPA Brown that the criminal background clearance for S3 was recently submitted pending processing. LPA Brown informed Licensee/Administrator Restituto "Resty" Calilung that deficiency will be issued and Civil Penalties were assessed during the facility visit with the amount of $500.00 and will continue to be assessed of $100.00 per day until corrected for failure to obtain criminal record clearance for S3. Licensee/Administrator Calilung removed S3 at the facility during the visit. In addition, during the quick tour of the facility, LPA Brown observed Resident #2 (R2) with full bed rail and per interview and documents review, R2's not on hospice and no Exception Letter was submitted and approved by Community Care Licensing Division (CCLD) for R2's full bed rail. Deficiency will be issued. Licensee/Administrator Restituto "Resty" Calilung removed the full bed rail during the facility visit on 04/18/2024. Type of visit: Required - 1 Year
0915: Licensing Program Analyst (LPA) Amy Goldenberg arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction, visit purpose and provided the facility caregiver with LPA identification and business card. 0936: Resident Records/Incident Reports/Personal Rights/Residents with Special Needs/Incidental Medial and Dental- LPA began review of resident records. Three (3) records were reviewed. LPA reviewed for admission agreement, medical assessment and TB test results, consent forms, identification and emergency information, appraisal needs and service plans, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. The facility is meeting documentation requirements. 1015: Physical Plant and Safety of Environment/Operational Requirements- LPA toured the facility inside and outside. LPA observe the facility to be clean and in good repair. The home is maintained at a comfortable temperature for the residents. Lighting is sufficient for safety and comfort. Water temperature measured 108 degrees F. Grab bars, non-slip mats are present in the restrooms. Laundry facilities and a locked cabinet is present for storing laundry soap and other chemicals. All outdoor and indoor passageways are free of obstruction. Night lights and emergency lighting is present. A locked area is provided for medications and sharp objects. There is a telephone working at this location. The LIC 610E, emergency disaster plan is maintained. LPA conducted Three (3) private resident interviews during the physical plant inspection. 1030: Personnel Records/Training/and Staffing-. LPA reviewed employee record for first aid certification, finger print clearance, personnel/job application, health screening and TB test results, criminal record statement, employee rights, training verification, and current administrator certification. The facility is meeting the documentation requirement. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. The facility has not exceeded its capacity limitation and the structure remains unchanged according to the approved floor plan. Smoke detectors and carbon monoxide detectors were tested and found to be operational. Fire extinguishers are tested or replaced annually. Receipts are on file. The facility is conducting emergency disaster drills, last done on 07/4/2023. 1100: LPA conducted two (2) staff interviews. 1115: LPA began preparation of this report. Based on the information received during this visit today, there are no deficiencies is being cited per Title 22, Division 6 of The California Code of Regulations.
