Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Valeria Conway conducted an unannounced Case Management-Other inspection at the facility. As per an email received on 09/18/2025, the Woodland Hills North Regional Office was informed that the facility no longer had any residents and that it was officially closed. Upon arrival at 9:30 A.M., LPA Conway knocked on the door, but there was no response. LPA then contacted and spoke by phone with the Administrator, Karthiga Vijayakumar to inform them of the purpose of the visit, which was to verify and confirm rescission of the license. The licensee stated that the facility keys had been returned to the landlord as the lease had expired. Also, the Administrator reported that four (4) residents were relocated to other licensed facilities and that family members were notified of the closure and relocation arrangements for them. At 10:30 A.M. LPA observed a individual arriving at the facility who had access to the interior of the property. LPA approached this individual, who stated that the house is currently being used as a sober living rehabilitation home. Permission to enter was granted at 10:33 A.M. LPA conducted a tour of the physical plant and observed that no care or supervision was being provided. LPA found no evidence indicating that an Residential Care Facility for the Elderly (RCFE) was in operation. At 10:40 A.M. the administrator arrived at the facility and met in person with LPA Conway. The administrator stated that they no longer have access to the property and they are not affiliated to the sober living. Additionally, they surrendered the facility license for Home Sweet Home Duvali Dr, Facility # 565850199. Based on observations and the interview with the Administrator, it was determined that the facility is no longer operating as a licensed facility. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Teresa Camara conducted a required annual visit. LPA met with back-up administrator Karthik "Raj" Kanakaraj and explained the reason for the visit. Currently the facility administrator is listed as Gloria Valencia, however the back-up administrator stated the licensee submitted documents to CCL over a year ago to change the administrator to Karthiga Vijayakumar. The licensee will re-send all documents in order to change the administrator of record to the correct person. Beginning at 11:06 a.m., LPA along with the back-up administrator conducted the physical plant tour to ensure there are no health and safety hazards and the facility is in compliance with Title 22 Regulations. The fire extinguisher appeared fully charged and was purchased 4/1/2024. The hardwired smoke detectors and the carbon monoxide detector were tested and functional at the time of the visit. No fire clearance concerns were observed. KITCHEN: LPA observed the kitchen to be clean. Kitchen appliances appeared to be in operable condition. The facility has a sufficient supply of non-perishable and perishable food. Cleaning supplies are located in a locked cabinet under the kitchen sink. Knives are stored in a locked drawer. COMMON AREAS: This includes the living room and dining room areas. LPA observed common area to be clean and properly furnished at the time of the visit. An adequately screened fireplace was noted in the living room. The laundry room was locked and contained cleaning supplies. All required postings were observed in the common area. PPE and incontinence supplies are located in closets inside the facility. The garage was observed to be locked. LPA observed additional food, water, and cleaning supplies in the garage. BEDROOMS: There are seven (7) total bedrooms in the facility; six (6) bedrooms are designated for resident use and one (1) staff room. The staff room is kept locked. All resident rooms were observed to be furnished appropriately with clean linens and sufficient lighting. Report Continued on LIC 809-C Report Continued from LIC 809 BATHROOMS: There are three (3) bathrooms for resident use. One bathroom is located in the hall, one bathroom is a "Jack & Jill" type shared by bedrooms two (2) and three (3), and one is a private bathroom for bedroom one (1). Restrooms were observed to be equipped with nonskid surfaces and contain nonskid mats. Grab bars were observed in the bathrooms. The water temperature was measured in two bathrooms at 117.1 and 117.4 degrees Fahrenheit which is in regulatory compliance. OUTDOOR SPACE: The backyard has a covered patio area with patio furniture including a table and chairs for resident use. All passageways were observed to be clear. There were no bodies of water on the premises. RECORD REVIEW: Staff and resident records were reviewed for documents including, but not limited to: health screening, TB test, staff training records, fingerprint clearance, resident physician's report, needs and service appraisal, and personal rights. All four (4) staff files and six (6) resident files observed were in compliance with regulation. All training were observed to be complete. INFECTION CONTROL/EMERGENCY DISASTER PLANNING: LPA reviewed the facility's infection control practices and the facility's emergency disaster plan. The facility’s policies and procedures as it pertains to infection control are adequate. Emergency disaster drills are conducted quarterly. Emergency disaster plan was observed to be complete and updated annually, as required. MEDICATION REVIEW: Medications for two (2) residents were reviewed. Medications observed were labeled, stored, and properly documented at the time of the visit. Medications appear to be given as prescribed. INTERVIEWS: LPA interviewed two (2) residents and two (2) staff. No concerns noted. No deficiencies cited. Exit interview conducted. Nov 22, 2023Facility evaluation reportReport on file
Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kelly Dulek arrived at the facility unannounced to conduct a required annual visit at 11:45AM. LPA initially met with facility staff. Administrator was unavailable during today's visit, however Assistant Administrator Karthiga Vijayakumar arrived at the facility. Entrance interview conducted. Beginning at 11:59AM, the LPA, along with Assistant Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that facility is in compliance with Title 22 Regulations. The following was observed: Fire extinguisher is fully charged and purchased on 03/21/2023. Hardwired combination smoke and carbon monoxide detectors were tested at 01:46PM and all were functional at the time of the visit. No fire clearance concerns were observed. KITCHEN: The LPA observed the kitchen to be clean. Kitchen appliances appeared to be in operable condition. The facility has a sufficient supply of seven (7) days non-perishable and two (2) days perishable food. Cleaning supplies are located in a locked cabinet under the kitchen sink. Knives are stored in a locked drawer. COMMON AREAS: This includes the living room and dining room areas. LPA observed common area to be clean and properly furnished at the time of the visit. An adequately screened fireplace was noted in the living room. Locked laundry room was observed. All required postings were observed in the common area. GARAGE: Garage was observed and contained extra food, PPE and incontinence supplies, and emergency food and water. BEDROOMS: There are seven (7) total bedrooms in the facility; six (6) bedrooms are designated for resident use and one (1) staff room. The staff room is kept locked. All resident rooms were observed to be furnished Report Continued on LIC 809-C appropriately with clean linens, appropriate furnishings and sufficient lighting. BATHROOMS: There are three (3) bathrooms for resident use. One (1) is designated for shared resident use and two (2) are private restrooms. Restrooms were observed to be equipped with nonskid surfaces and contain nonskid mats. Grab bars were observed in the bathrooms. The water temperature was measured in the shared resident bathroom and measured in compliance with regulation. OUTDOOR SPACE: The backyard has a covered patio area with patio furniture including a table and chairs for resident use. All passageways were observed to be clear. There were no bodies of water on the premises. RECORD REVIEW: Staff and resident records were reviewed for documents including, but not limited to: health screening, TB test, staff training records, fingerprint clearance, resident physician's report, needs and service appraisal, and personal rights. All four (4) staff files and five (5) resident files observed were in compliance with regulation. Administrator Certificate was observed to be expired, however LPA reviewed the Administrator Certification website, which indicated all documents were received and the Administrator's Certificate is still pending at this time. All trainings were observed to be complete. INFECTION CONTROL/EMERGENCY DISASTER PLANNING: During today’s visit, the LPA reviewed the facility's infection control practices and the facility's emergency disaster plan. The facility’s policies and procedures as it pertains to infection control are adequate. Emergency disaster drills are conducted quarterly, with the last drill conducted on 09/30/2023. Emergency disaster plan was observed to be complete and updated annually, as required. MEDICATION REVIEW: Began at 01:35PM. Medications for two (2) residents were observed. All medications observed were labeled, stored, and properly documented at the time of the visit. INTERVIEWS: During today's visit, LPA attempted to interview two (2) staff and two (2) residents.
