Type of visit: Required - 1 Year
On 1/14/2026, Licensing Program Analyst (LPA) Valerie Flores conducted an unannounced 1-year required visit. LPA met with Administrator Chris Bundalian and LPA explained to Chris the purpose of the visit. A tour of the interior and exterior areas were conducted alongside Chris. LPA observed the following during today's visit: The facility is a single-story structure which consist of (3) three resident bedrooms, (3) staff bedrooms, (3) three bathrooms, office, living room, dining room, kitchen, garage, and backyard area. Facility's targeted population is a residential care facility for the developmentally disabled elderly. The facility provides the residents with laundry services. During the visit, LPA observed the washer and dryer to be fully functioning. The facility maintains social activities like crafts and board games located in the living room to encourage resident interaction. Resident bedrooms were observed to have the required bedding, furniture, seating, and functional lighting. LPA observed a closet which stored additional linen and towels. Indoor/outdoor passageways were kept free from obstruction. Facility maintained a two-day supply of perishable foods and (7) seven-day supply of non-perishable foods. Facility maintained emergency food and water in the garage. Knives, cleaning solutions, and other sharp items were stored in a locked cabinet. Medication was observed to be centrally stored in a locked closet in the hallway. LPA observed an additional hall closet which stored an over-supply of client hygiene products. The facility maintained at a comfortable temperature for the residents. LPA observed a covered patio that was fully furnished. The facility's foundation is at the top of a hill; for safety measures, the facility placed a gate around the perimeters of the backyard to ensure resident safety. Per Administrator, there are no firearms and/or ammunition on the premises. (4) Four out of (4) four resident records were reviewed. Records reviewed included but not limited to preplacement appraisals, admission agreements, personal rights, care plans, current IPP's, CDER, dental information, and medical assessments. (3) Three out of (3) three staff records were reviewed. Records reviewed included but not limited to health screenings, personnel records, background clearance, fingerprint clearance, employee rights, valid CPR/First-Aid Certification, and relevant training's to assist residents with activities of daily living. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kathleen Banrasavong arrived unannounced to conduct an annual inspection. Upon arrival LPA was greeted by facility staff and granted entry. LPA began inspection with introduction and visit purpose. Upon arrival LPA learned that four (4) residents live at this facility. There was three (3) residents at the facility during the inspection. There was one (1) staff members present. The Administrator, Christopher Bundalian was advised of the annual and came to conduct and completed the facility tour. Client Records/Incident Reports/Clients Rights Information: LPA reviewed client records. Four (4) records were reviewed. LPA reviewed for identification and emergency information, admission agreement, medical assessment, and TB test results, needs and service plans, placement, functional assessment, centrally stored medication/destruction records, safeguard for personal property/valuables, and personal rights notification. Personnel Records/Training/ Staffing/ Administration: LPA reviewed employee records. Two (2) records were reviewed. LPA reviewed employee records for first aid certification, criminal record clearance or an exemption, health screening and TB test results, employee rights, training verification, and current administrative organization. Christopher Bundalian, Administrator’s certificate expiration date was 08/11/2025. Food Service: Food prep areas are clean and organized. Food supply meets the requirement of one (1) week supply of nonperishable and two (2) day supply of perishables. Emergency food and water supply is present. There is a location for sharps in the kitchen. Physical Plant and Safety of Environment/Operational Requirements: LPA toured the facility inside and outside. LPA observed the facility to be clean and in good repair. The facility is maintained at 75 degrees for the client’s comfort. Lighting is sufficient for safety. Water temperature measured 108.0 degrees F. Laundry is done in the laundry room. There is a locked location for storing laundry soap, cleaning supplies and chemicals in the closet in the laundry room. All outdoor and indoor passageways are free of obstruction. Emergency lighting is available. There is a telephone working at this location. LPA dialed the facility’s landline number, which rang and was operable. The LIC 610, emergency disaster plan is maintained. There are no firearms at this facility. There is one (1) secured fireplace at this facility. There are zero (0) pools at the facility. There is one (1) secured gate that has a self-latching lock located on the northeast side of the house. LPA observed emergency supplies and one (1) first aid kit. The last emergency fire drill was conducted on 12/20/2024. Infection Control: The LPA observed the hand washing stations in the facility restrooms and kitchen had hand hygiene supplies and hand washing signs. LPA observed PPE equipment and cleaning supplies to do regular cleaning of the facility. LPA reviewed the facility's infection control plan which met department requirements. LPA reviewed staff records and found that staff had infection control training. Medications/Health Related Services/Incidental Medical Services: The medications are centrally stored. There is a locked cabinet allocated for medication storage. Centrally stored medication and destruction logs are maintained separately. LPA reviewed medication logs and observed that they were dispensed accurately. LPA made observation throughout the inspection process to assess if the facility remains in conformity with the State Fire Marshall regulations. LPA observed six (6) smoke detectors and one (1) carbon monoxide detectors throughout the facility. There were two (2) fire extinguishers on site, date charged was 12/10/2024. Pursuant to Title 22 of The California Code of Regulations Division 6, there are zero (0) deficiencies observed. Jan 24, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 1/24/2024, Licensing Program Analyst (LPA) Chinwe Nwogene arrived unannounced at the facility to conduct an annual inspection. LPA Nwogene met with Caregiver, Marian Bundalian and Administrator, Christopher Bundalian who were informed of the purpose of visit. At the time of visit there was three #3 staff and two #2 residents present. LPA was informed that the other two clients are at the Day Program. LPA toured the facility inside and out with Christopher Bundalian and Marian Bundalian. Tour included: Kitchen: LPA toured the kitchen and observed kitchen to be clean. Food is stored in a safe and healthful manner. Utensils and dishware are sufficient for the capacity. The refrigerator and stove are in working order. Sharps are stored in a locked kitchen cabinet, available only to authorized individuals. Trash cans has tight-fitting lid. Fridge, Freezer and all need appliances were present and shown to be in working condition and clean. Dining and Livingroom; LPA toured the dinning and Livingroom area. LPA observed area to be clean and furnitures in good condition. Temperature was 69 degrees Fahrenheit. Hallway: LPA toured the hallway and observed hallway to be clean with no pathway obstruction. LPA inspected the fire extinguisher and found it to be in compliance and record to be up to date. Carbon monoxide & smoke detector were tested and functioning properly. LPA observed additional linens and hygiene items. Medication: Medications were labeled and stored in separate bins inside of a locked medication closet and are distributed according to physician orders. The first aid kit was complete. Bathroom: LPA toured three #3 bathrooms and observed bathrooms to be clean and equipped with grab bar and non-slip mat. There is also a good number of personal toiletries available for the residents in care. The hot water measured at 113 degrees Fahrenheit. Bedroom; LPA toured #3 out of #3 resident bedrooms and observed bedrooms to be clean and furnished according to regulation, which includes proper furniture, dressers, chairs and lighting. Night lights were maintained throughout the facility. Garage and Laundry; LPA tour the garage and observed garage to be clean. Washing machine and dryer are all in good repair and sufficient for the census. Cleaning supplies will be stored away in the garage, inaccessible to clients. Backyard: LPA toured the backyard and observed backyard to be clean and furnitures in good condition. The backyard was free from obstruction and the side gates remain unlocked. No bodies of water were observed. Food Services: There are seven days non-perishable and two days of perishable food supply present, and all food was properly stored and available to residents. Fridge and Freezer are laugh enough to accommodate perishable foods. Records: All staff present have a criminal record clearance in file and are confirmed as being associated with the facility. Three staff and three residents' records were reviewed. All required postings were posted near the entryway and throughout the facility. The administrator certificate expires on 8/11/2025. Interview; Three staff were interviewed. No deficiencies noted at the time of visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Sara Martinez conducted an unannounced annual required visit. LPA was granted entry and met with Administrator, Christopher Bundalian, who was informed of the purpose of the visit. At the time of the visit there was three (3) staff and one (1) client present. The facility is a one story home with (6) bedrooms and (3) bathrooms with an attached garage, and a backyard. The facility does not contain any bodies of water, firearms, or ammunition on the property. The clients served are elderly adults 65 years of age and older. Clients are vendorized by Inland Regional Center (IRC). LPA conducted a tour of the interior and exterior, reviewed facility documents and conducted interviews. LPA observed the following: Infection Control: LPA observed the hand washing stations in the facility restrooms and kitchen had hand hygiene supplies and hand washing signs. PPE equipment and cleaning supplies were available to do regular cleaning of the facility. LPA reviewed the facility's infection control plan and staff training which met department requirements. Physical Plant: LPA observed the client bedrooms, bathrooms, and staff office. Physical plant, floors, windows, and doors were observed to be clean. Fixtures and furniture were present and were in good repair. The outdoor area was observed to be free of hazards and contained outdoor furniture and shaded area for clients. Laundry equipment was observed to be in good working condition. The sharp and dangerous objects were observed to be locked and inaccessible to clients. The smoke detector and carbon monoxide was operational, and the hot water temperature 116.4 F. Food Service: LPA observed facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. LPA observed the facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. Care & Supervision/Administration: Adequate staff are present for the supervision of clients during the visit. Facility sketch, exit routes, personal rights, complaint information and emergency phone numbers were found posted in the facility. The listed administrator possesses a current administrator's certificate. Record Review and Resident/Staff Files: LPA reviewed three (3) staff files and training. All staff have criminal clearance and updated training along with CPR/First Aid Certification. Three (3) client files were reviewed, and possessed all required paperwork. Health Related Services/ Incidental Medical Services: All client medication was locked in a hallway closet. LPA reviewed medications for three (3) clients and found all medication listed on MARS and all required labeling was found to be in place. Disaster preparedness: LPA reviewed the facility's emergency and disaster plan. LPA reviewed documentation showing the facility's last fire and earthquake drill was conducted on 08/03/2023 which met the department requirements. LPA observed all facility exits were clear from obstructions. LPA observed emergency supplies in the garage and entry way closet and first aid kit with all required items. No deficiencies were cited at the time of the visit.
